Oral rehabilitation of a bulimic patient: a case report.

Bulimia nervosa is among the most common health problems in contemporary society. It is a self-induced weight loss syndrome associated with distinct dental manifestations involving physical and psychologic symptoms. It is characterized by recurrent binge-purge episodes that occur at least once a day. This article describes the complete-mouth rehabilitation of a bulimic patient with a generalized enamel erosion of her dentition and a poor esthetic appearance. Porcelain-fused-to-metal restorations were used as the definitive treatment. Good esthetics and high self-esteem were the final results.

Eating disorders and sexual abuse.

This review examines the current debate on the role that sexual and physical abuse may play in predisposing to eating disorders in women. Despite some discordant opinions, clinicians agree that the experience of abuse in early childhood may be important for understanding the complex genesis of the eating disorders of some women. Three groups of studies are presented: those in which no connections emerge between sexual abuse and eating disorders, those in which a strong link is present and those in which the results refer to a multifactorial interpretative model. Some of the main symptoms, such

The observer perspective: biased imagery in social phobia, agoraphobia, and blood/injury phobia.

Clark and Wells' (1995): 'A cognitive model of social phobia'. In Social phobia: Diagnosis, assessment, and treatment (pp. 69-93), R. G. Heimberg, M. R. Liebowitz, D. A. & F. R. Hope (eds.); cognitive model of social phobia proposes that social phobics generate a negative impression of how they appear to others. This impression often occurs in the form of an image from an "observer" perspective in which social phobics can see themselves as if from another person's vantage point. This study investigated the specificity of the observer perspective among patients with social phobia, agoraphobia,

Soft tissue profile: a survey of African-American preference.

Soft tissue profile is a critical area of interest in the development of an orthodontic treatment plan. This study was designed to elicit the current soft tissue profile preference of African-American respondents. Results indicate that: (1) African-American male and female subjects prefer relatively flat profiles, with varied fullness of the lips. (2) African-American male and female subjects prefer the male subjects to have more protrusive profiles than the female subjects. (3) Respondents do not always correctly select their own profile type. (4) More than a third of the respondents desire a

The interpersonal and psychological functioning of women who experienced childhood physical abuse, incest, and parental alcoholism.

Questionnaires assessing childhood physical abuse (CPA), childhood incest (CI), and parental alcoholism (ACOA) were completed by 253 college women from introductory psychology classes at a large midwestern university. The relationship between these variables and the level of depression, self-esteem and involvement with physically abusive, sexually assaultive, sexually coercive, and chemically dependent partners was assessed. Support was found for an additive model of trauma that predicted a relationship between number of childhood traumas and adult outcomes. Limited support was found for a spe

Therapy outcome of two treatment models for bulimia nervosa: preliminary results of a controlled study.

Beyond the reduction of the core symptoms, different modification patterns are expected due to differing emphases in two alternative types of treatment for bulimia nervosa: inpatient analytic and outpatient systemic therapy. The initial results of a study with a waiting-list control group are reported. Eating disorders of the bulimic women definitely improved in both therapy groups, the results for the inpatient group (n = 27) indicate a basic change in the attitude towards eating.

Adaptation in families with a member requiring mechanical ventilation at home.

Current health care trends indicate that ventilator-dependent patients increasingly will be discharged to home after shorter hospitalizations. The purpose of this study was to determine how care givers adapt to having ventilator-dependent adults at home. Twenty families were interviewed in their homes by using an eight-question semistructured interview guide. The Roy model was used as a framework to analyze these data. Two instruments, the Family Coping Scale (F-COPES) and the Family APGAR, were administered to measure family coping and function. Patients, aged 18 to 74 years, required 24-hour

Quality of object relations versus interpersonal functioning as predictors of therapeutic alliance and psychotherapy outcome.

The purpose of the study was to compare an interview measure of quality of object relations to questionnaire and interview measures of recent interpersonal functioning with respect to the prediction of therapeutic alliance and psychotherapy outcome. The sample consisted of 64 patients who had received approximately 20 sessions of short-term individual psychotherapy within a controlled, clinical-trial investigation. Ratings of therapeutic alliance were provided independently by the patient and the therapist after each session. Outcome measures, which were provided by three sources (patient, the

Personal and social resources in children of patients with bipolar affective disorder and children of normal control subjects.

The authors examined the personal resources (social problem-solving ability, internal locus of control, self-esteem, and self-perceived competence) and social resources (social network structure and support) in 23 children of patients with bipolar affective disorder (probands) and 33 children of normal control parents. Positive resource profiles were related to psychiatric well-being in the offspring. Nondisordered probands, in particular, demonstrated a strikingly positive profile of personal resources as well as a wide range of peer, sibling, and other kin supporters. Disordered probands had

Periodic leg movements in sleep: effect of clonazepam treatment.

Periodic leg movements in sleep were diagnosed in 8 patients with insomnia and in 12 patients with hypersomnia. Polysomnography did not reveal any difference in the number of leg movements in these groups, the mean duration of a leg movement, or the number of activity epochs. Those with insomnia, however, had a shorter intermovement interval than those with hypersomnia, with more leg movements in each activity epoch. Treatment with 0.5 to 2 mg clonazepam improved subjective complaints and decreased the number of leg movements without affecting the intermovement interval or movement duration.

Chowchilla revisited: the effects of psychic trauma four years after a school-bus kidnapping.

A 4-year follow-up study of 25 school-bus kidnapping victims and one child who narrowly missed the experience revealed that every child exhibited posttraumatic effects. Symptom severity was related to the child's prior vulnerabilities, family pathology, and community bonding. Important new findings included pessimism about the future, belief in omens and prediction, memories of incorrect perceptions, thought suppression, shame, fear of reexperiencing traumatic anxiety, trauma-specific and mundane fears, posttraumatic play, behavioral reenactment, repetitions of psychophysiological disturbances

Special problems of women in psychotherapy.

Feminist critics have failed to acknowledge the usefulness of Freud's neutral observations about female sexual development. This paper is an attempt to refute the prejudiced, incorrect view of the modern psychoanalytically oriented psychotherapist as sexist in orientation. Several detailed case examples are utilized to illustrate the interpretive treatment of excessive passivity in women patients with a variety of neurotic difficulties.

Pattern of Agreement Among Medications Used During Pregnancy as Recorded in Self-Report and Administrative Claims From California.

BACKGROUND: Medication usage prior to conception and over the duration of pregnancy is common. Two frequent sources that record such medications are self-report and administrative databases. Our goal was to examine the pattern of agreement among medications reported in each source. METHODS: We linked all interviewed subjects recruited for the Birth Defects Study To Evaluate Pregnancy exposureS (BD-STEPS) in California to their Medi-Cal insurance claims. BD-STEPS is a population-based case-control study including livebirths, stillbirths, and terminations with a major birth defect and livebirth

Business Versus Ethics? Thoughts on the Future of Business Ethics.

INTRODUCTION: Behavioral determinants can enable or hinder motivation towards registration and donorship and, subsequently, action or inertia towards organ donation. Nevertheless, there is limited information about the role of self-efficacy in relation to organ donation awareness and presumed consent among individuals and their families. The aim of this study was to explore knowledge, attitudes and general self-efficacy as behavioral determinants for organ donation among rural primary care attendants, in order to tailor awareness strategies for reversing inertia within an opt-out system. Healthy and Sustainable Food Choices in European Households: A Cross-Cultural Study of Knowledge, Practices, Intentions, and Barriers.

Background: Growing concerns about health and sustainability are accelerating the need to transform food systems to promote well-being and reduce environmental impacts. Despite increasing policy efforts, evidence remains limited on self-perceived knowledge, reported practices, intentions, and perceived barriers related to healthy and sustainable eating among European households with children. Establishing healthy eating habits during childhood is essential for long-term health, with parents playing a pivotal role in shaping these behaviors. Understanding the factors and barriers related to foo

Gene expression networks regulated by human personality.

Genome-wide association studies of human personality have been carried out, but transcription of the whole genome has not been studied in relation to personality in humans. We collected genome-wide expression profiles of adults to characterize the regulation of expression and function in genes related to human personality. We devised an innovative multi-omic approach to network analysis to identify the key control elements and interactions in multi-modular networks. We identified sets of transcribed genes that were co-expressed in specific brain regions with genes known to be associated with p

Self-management strategies used by 'high functioning' individuals with bipolar disorder: from research to clinical practice.

INTRODUCTION: Bipolar disorder (BD) is a complex mental illness that results in substantial costs, both at a personal and societal level. Research into BD has been driven by a strongly medical model conception, with a focus upon pathology and dysfunction. Little research to date has focused upon strategies used to maintain or regain wellness in BD. Here, we present results from a qualitative study of self-management strategies used by a Canadian sample of 'high-functioning' individuals with BD. The aims of the present paper are two-fold: (1) To provide a description of the self-management stra

"In Situ" Orbital Correlations.

ConspectusOrbital correlation diagrams are central to chemistry. Based on the symmetry compatibility and orbital overlap amplitude, they link the energy-ordered frontier molecular orbitals (MOs) of reactants and products and have long been a powerful and essential tool for understanding chemical interactions (reactions) and molecular properties. The frontier MOs typically include the highest occupied MOs (HOMOs) and the lowest unoccupied MOs (LUMOs), along with a few nearby orbitals of the reactants. However, it is also known that some reactions cannot be well explained with a few frontier MOs

Prevalence, associated factors, and machine learning-based prediction of infertility among married women in North Shewa Zone, Ethiopia: a cross-sectional study.

BACKGROUND: Infertility, defined as the inability to conceive after 12 months of regular unprotected sexual intercourse, is an important public health concern with significant social, psychological, and marital consequences. In this study, infertility was based on self-reported time to conception rather than clinical diagnosis; no clinical examination or male factor assessment was conducted, and outcome misclassification is therefore possible. Despite its importance, community-based evidence in Ethiopia remains limited. This study aimed to estimate the prevalence of infertility, identify facto
