Lower subscapular nerve transfer for axillary nerve repair in upper brachial plexus palsy.

BACKGROUND: The potential to utilize the lower subscapular nerve for brachial plexus surgery has been suggested in many anatomical studies. However, we know of no studies in the literature describing the use of the lower subscapular nerve for axillary nerve reconstruction to date. This study aimed to examine the effectiveness of this nerve transfer in patients with upper brachial plexus palsy. METHODS: Of 1340 nerve reconstructions in 568 patients with brachial plexus injury performed by the senior author (P.H.), a subset of 18 patients underwent axillary nerve reconstruction using the lower

OBJECTIVE: To review the advances in the diagnosis and treatment of obstetric brachial plexus palsy (OBPP). METHODS: The incidence, risk factors, classification, and imaging tests of OBPP and indication, technique, and results of surgery were reviewed and summarized. RESULTS: The incidence of OBPP is not declining in recent years. Birth weight of ≥4 kg, forceps delivery, and prepregnancy body mass index of ≥21 are considered to be major risk factors, and caesarean section delivery seems to be a protective factor. Neurophysiological investigations can be applied to qualitative diagnosis of OB

Timing of forearm arterial repair in the well-perfused limb.

The purpose of this study was to evaluate the difference in the outcomes of patients with a well-perfused hand who were taken to the operating room (OR) within 6 hours of forearm arterial injury vs patients who were treated in a semiurgent fashion. A review of 201 consecutive patients who were taken to the OR with forearm-level radial or ulnar arterial injury identified 26 patients who had a well-perfused hand at presentation in whom hemostasis was achieved within 15 minutes with manual pressure and no signs of hemodynamic instability. Outcomes of interest included short-version Disabilities o

Anatomic Reconstruction of Acute Posterior Shoulder Instability With Reverse Hill-Sachs Lesions: A Report of Two Cases.

Posterior shoulder dislocation associated with a reverse Hill-Sachs lesion is an uncommon injury that may result in persistent instability when the humeral head defect is engaged. Management depends on lesion size, engagement, chronicity, and reconstructibility; both non-anatomic procedures and anatomic reconstruction strategies have been described for acute lesions within the joint-preserving range. We present two cases of acute posterior shoulder instability with engaging reverse Hill-Sachs lesions treated with a combined arthroscopic-open approach. In both patients, arthroscopic posterior l

A novel concealed posterior axillary approach for scapula glenoid fractures - a multicenter clinical study.

Glenoid fractures of the scapula can be treated via anterior deltopectoral, Judet, modified Judet, or posterior minimally invasive approaches; however, these may result in soft tissue injury, hematoma formation, nerve damage, or inadequate internal fixation stability. While arthroscopy minimizes soft tissue dissection, it may pose challenges in visualizing inferior glenoid fragments. To address these potential complications, we designed a new posterior axillary approach, the Tian Yun-Wu Dankai approach (T-W approach), in the treatment of certain glenoid fractures of scapula. We retrospectively

Identifying key factors associated with subscapularis tendon tears and developing a risk prediction model to assist diagnosis.

BACKGROUND: There are still some challenges in diagnosing subscapularis (SSC) tendon tears as accurately as posterosuperior rotator cuff tears on MRI. The omission of SSC tendon tear can lead to muscle atrophy, fatty infiltration, and increased tearing accompanied by aggravated shoulder pain and loss of function. An effective non-invasive evaluation tool will be beneficial to early identification and intervention. The study aims to identify sensitive predictors associated with SSC tendon tears and develop a risk prediction model to assist in diagnosis. METHODS: Data on 660 patients who receiv

Morel-Lavallee Lesion Associated with Subtrochanteric Femur Fracture in a Young Female Resulting in Extensive Soft-Tissue Necrosis and Hypertrophic Non-union Managed with Staged Surgeries and Exchange Nailing: A Rare Injury Pattern and Review of Literature.

INTRODUCTION: Interferential screws and anchors have played a major role in fixation of soft tissue to bony tunnel. With advent of time, there developed metal anchors, biodegradable anchors, bioinert anchors, etc. Biodegradable anchors had complications such as reactive synovitis, cyst formation, soft-tissue inflammation, and local osteolysis. Polyether ether ketone (PEEK) which was biologically inert and radiolucent was introduced to overcome the disadvantages of anchors. We present a case of non-suppurative tissue reaction to PEEK anchor following rotator cuff repair. CASE REPORT: A 58-year

Inferior subluxation of humeral head after osteosynthesis for greater tuberosity fracture.

BACKGROUND: Inferior subluxation of the humeral head is frequently observed immediately after surgery for proximal humerus fractures; however, the incidence and risk factors of inferior subluxation after osteosynthesis for isolated greater tuberosity fractures remain unsolved. Additionally, the postoperative course of inferior subluxation has not been elucidated. The purpose of the present study is to identify the predictors for the occurrence of postoperative inferior subluxation by multivariate analysis and investigate the postoperative shift of inferior subluxation and its effect on surgica
