Sleep architecture in children with adenoidal hypertrophy.

AIM: Adenoidal hypertrophy (AH) in children is associated with obstructive manifestations like mouth breathing, snoring. Unfortunately, little is known regarding sleep architecture of AH in children. The purpose of this study was therefore undertaken to investigate the polysomnographic variables in children with AH. METHOD: 47 children with AH and 11 controls underwent nocturnal polysomnography. Sleep was scored manually according to the standard set by Rechtschaffen. RESULTS: In AH, stage 1 sleep percentage and rapid eye movement (REM) latency were increased significantly, while the sleep p

Body composition profiles and metabolic vulnerability in comorbid insomnia and sleep apnea (COMISA): A large-scale BIA analysis.

BACKGROUND: Comorbid insomnia and sleep apnea (COMISA) presents a greater clinical burden than obstructive sleep apnea (OSA) alone. We investigated the association between body composition and insomnia to identify the body composition phenotype of COMISA. METHODS: This cross-sectional study included 2915 patients with moderate-to-severe OSA (apnea-hypopnea index ≥15) who underwent polysomnography and body composition analysis. Participants were categorized into OSA (Insomnia Severity Index [ISI] < 15) and COMISA (ISI ≥15). Skeletal muscle mass (SMM), visceral fat area (VFA), and the SMM/VFA r

A Further Introduction to Dental Sleep Medicine.

Dental sleep medicine traditionally focuses on sleep-related breathing disorders, such as snoring and obstructive sleep apnea. However, everyday practice shows that also other sleep disorders touch on dentistry, including sleep-related orofacial pain, xerostomia, hypersalivation, gastroesophageal reflux disease and bruxism. A new definition, which covers all the diagnostic and treatment aspects of these disorders, has therefore been formulated for dental sleep medicine. This article describes why this development started and sketches the current state of affairs regarding the discipline of den

Pediatric sleep-wake disorders.

Sleep-wake problems are common during childhood and adolescence. They are of diverse cause, and can contribute significantly to alterations in behavior, cognition, and learning. Obstructive sleep apnea, central hypoventilation syndrome, narcolepsy, periodic hypersomnia, delayed sleep phase syndrome, restless legs syndrome, parasomnias, and sleep disruption consequent to psychiatric disorders are some of the commonly encountered conditions. Some aspects of sleep architecture and its organization change with age and maturation. Diagnostic criteria and sleep laboratory techniques and findings for

Adaptive servoventilation (ASV) in patients with sleep disordered breathing associated with chronic opioid medications for non-malignant pain.

BACKGROUND: Adaptive servoventilation (ASV) can be effective therapy for specific types of central apnea such as Cheyne-Stokes respiration (CSR). Patients treated chronically with opioids develop central apneas and ataxic breathing patterns (Biot's respiration), but therapy with CPAP is usually unsuccessful. There are no published studies of ASV in patients with sleep apnea complicated by chronic opioid therapy. METHODS: Retrospective analysis of 22 consecutive patients referred for evaluation and treatment of sleep apnea who had been using opioid medications for at least 6 months, had an apn

Sleep apnea is common in patients with coronary artery disease.

Sleep-disordered breathing (SDB) has a prognostic impact in patients with cardiac diseases. We included 257 patients with preserved left ventricular function and angiographically proven coronary artery disease (CAD). All patients underwent cardiorespiratory polygraphy. In 251 patients high-sensitive C-reactive protein and fibrinogen were measured. SDB was documented in 188 patients (apnea-hypopnea-index [AHI] 16.4+/- 1.9/h): 58 patients presented central sleep apnea (CSA) and 130 patients obstructive sleep apnea (OSA). All patients (73%) with SDB had higher blood fibrinogen levels than those w

Reduction of sleep apnea and bradycardia in preterm infants on oscillating water beds: a controlled polygraphic study.

The sleep and respiratory patterns of eight apneic preterm infants were polygraphically recorded for 24 hours. This polygraphic study was designed to test and extend our previous finding that gently oscillating water beds reduce apnea in premature infants. The infants who ranged in gestational age from 27 to 32 weeks and in birth weight from 1,077 to 1,650 gm served as their own controls, off and on the water bed. The 24-hour recordings were divided into four time blocks with the infant being placed on the water bed during alternate six-hour periods. Apnea was significantly reduced while the i

Evaluation of Sleep Apnea in Stroke Patients Using a Portable Smartphone-Based System.

Sleep apnea is common but often underdiagnosed after stroke, hindering rehabilitation and functional recovery. Access to sleep studies remains limited for stroke patients, partly due to the inconvenience of current diagnostic tools. The widespread availability and built-in sensors of smartphones make them powerful solutions for mobile health applications, including sleep monitoring. This study aims to detect and evaluate sleep apnea in post-stroke patients using a smartphone and extract multimodal digital biomarkers to characterize their sleep patterns. For that, overnight sleep tests were con

Upper airway findings in patients with nocturnal breathing disorders.

This prospective open study was performed to examine the relation between pathologic findings of the upper respiratory tract and the types of noncentral sleep disorders (rhonchopathy, obstructive sleep apnea). 312 men and 274 women aged between 35-75 years attended our outpatient department for relief of their nocturnal breathing disorder. About 1/3 of the patients suffered only of habitual rhonchopathy and 2/3 were obstructive. No findings of the head and neck examination could predict the degree of oxygen desaturation, although pathologies of the nose (73.68%), the soft palate (94.1%) and na

Polysomnographic comparison of sleep in children with obesity and normal weight without suspected sleep-related breathing disorder.

Short sleep and obstructive apneas/hypopneas have been shown to be associated with childhood obesity. Still, few studies have compared sleep in children with obesity, without suspected sleep disordered breathing and normal weight peers by objective sleep measures and compared results with subjective parent assessment of sleep. Children with obesity aged 7-13 years (N = 44) and a matched group of normal weight children (N = 42) completed clinical polysomnography (Embla A10 Recording System). Parents scored their children's sleep on the Children's Sleep Habits Questionnaire (CSHQ). Mann-Whitney

Interactions of central and autonomic nervous systems in patients with sleep apnea-hypopnea syndrome during sleep.

PURPOSE: Sleep apnea-hypopnea syndrome (SAHS) is an independent risk factor for various cardiovascular and cerebrovascular diseases, but the underlying relationship of its physiological subsystems remains unclear. Thus, we aimed to investigate the effect of SAHS on central and autonomic nervous system (CNS-ANS) interactions during sleep. METHODS: Thirty-five patients with SAHS and 19 healthy age-matched controls underwent overnight polysomnography. The absolute spectral powers of five frequency bands from six EEG channels and ECG morphological features (HR, PR interval, QT interval) were calc

Central Sleep Apnea Alters Neuronal Excitability and Increases the Randomness in Sleep-Wake Transitions.

OBJECTIVE: While most studies on Central Sleep Apnea (CSA) have focused on breathing and metabolic disorders, the neuronal dysfunction that causes CSA remains largely unknown. Here, we investigate the underlying neuronal mechanism of CSA by studying the sleep-wake dynamics as derived from hypnograms. METHODS: We analyze sleep data of seven groups of subjects: healthy adults (n = 48), adults with obstructive sleep apnea (OSA) (n = 29), adults with CSA (n = 25), healthy children (n = 40), children with OSA (n = 18), children with CSA (n = 73) and CSA children treated with CPAP (n = 10). Effectiveness of Sleep Apnea Detection Based on One vs. Two Symmetrical EEG Channels.

Typically, two symmetrical EEG channels are recorded during polysomnography (PSG). As a rule, only the recommended channel is used for sleep stage scoring or sleep apnea detection, and the other for backup. Concurrently, there are many works demonstrating the asymmetry in brain activity. The aim of this work was to compare the accuracy of sleep apnea detection with the use of features obtained from one (C3-A2 or C4-A1) versus these two symmetrical EEG channels. To this end, the relevant data from the PhysioBank database (25 whole-night PSGs) were used. The same methodology of feature extractio

Analysis of differential photoplethysmography signal patterns in apnea and hypopnea.

Objective.Photoplethysmography, a non-invasive optical technique that measures changes in blood volume in the microvascular bed of tissue, offers a promising approach for monitoring physiological changes during sleep. This study evaluates differential photoplethysmography signal patterns that can distinguish between apneas vs hypopneas, which are key features of sleep-related breathing disorders.Approach.We analyzed data from 263 severe (apnea hypopnea index ⩾30) obstructive sleep apnea patients, using recordings from the Multi-Ethnic Study of Atherosclerosis. PLMS is associated with low arousal threshold and favorable upper airway patency in patients with OSA.

OBJECTIVES: Periodic limb movements during sleep (PLMS) in patients with obstructive sleep apnea (OSA) increases the complexity of disease management. However, pathological mechanisms linking OSA and PLMS have remained unclear. This study aims to compare the endotypic traits of sleep apnea in patients with combined PLMS (OSA-PLMS) versus those with OSA alone. METHODS: A total of 1008 adults with moderate-to-severe OSA were prospectively recruited from a single sleep center at which they underwent in-laboratory full-night polysomnographic studies. Periodic limb movements were manually scored,
