Streptococcal Necrotizing Fasciitis with Toxic Shock Syndrome and Rapid Fatal Outcome.

INTRODUCTION: Streptococcal necrotizing fasciitis (NF) is a serious soft tissue infection with rapid progression of inflammatory process among superficial or deep fascia, systemic host response to infection leading to toxic shock syndrome (TSS), and multiple organ failure. Lethality is high. CASE OUTLINE: A 46-year-old male without co-morbidities was admitted to the Emergency Department with redness, swelling and pain on his right lower leg. He became sick two days earlier with malaise, chills and shivering. On admission he was hypotensive, anuric, with erythematous rash on his face, neck and

Acute necrotizing fasciitis in Egyptian patients: a case series.

Hyperacute soft tissue infection is an uncommon infectious entity, which mostly affects immunocompromised individuals, mainly diabetics and poses difficult diagnostic and therapeutic management decisions. This study addresses the presentation, evaluation and management of 37 diabetic patients with acute necrotizing fasciitis treated throughout the period between August 1993 and July 2006 by the main author. Extremities, trunk and perineum were the most commonly involved sites with an incidence of 35%, 30% and 27%, respectively. All patients presented with cellulitis, which was associated with

[Repair and functional reconstruction of war wounds of limbs].

From 1979 to 1994, reparative and reconstructive surgery were used to repair the war injuries of skins, bones, blood vessels and nerves of the limbs in 800 cases. A systematic clinical study was carried out. Many new operative methods were used and the results of treatment were good. Innovations and modifications were made in technique. In 120 cases of war injuries having soft tissues defects including skin and muscles, various tissue transplantations were used with the hope to accomplish one-staged repair of the defect and reconstruction of motor function of muscle. To those infections of bon

Preliminary arteriovenous fistula for free-flap reconstruction in the diabetic foot.

We present our experience at the American University of Beirut Medical Center with two diabetic patients suffering from large necrotic and infected foot ulcers. Both patients were ambulatory at the time of presentation despite their extensive wounds and were believed to have a useful limb with adequate protective sensation worth saving. Below-knee amputation was prevented in both cases by successful soft-tissue coverage of the ulcers using microvascular composite-tissue transfer a few days after performing a preliminary arteriovenous fistula with a long vein graft loop. The flap vessels were a

Major peripheral veins injuries.

The injury was severe in wounded limb patients of this series who suffered from an associated major peripheral vein trauma. The presence of such an injury weighed heavily on the prognosis. Thirty eight patients with major peripheral veins injuries are reviewed. The injury had resulted from war wounds, work or road accidents. The superficial femoral vein was the most frequently injured vein. Associated injuries were frequently noted: soft tissues injuries in 35 patients, fractures in 33, arterial injuries in 32 and peripheral nerve injuries in 22 patients. Shock was more often present and more

Treatment of the infected total knee replacement combining DAIR with continuous local antibiotic perfusion: a preliminary case report series.

BACKGROUND: Continuous local antibiotics perfusion (CLAP) involves placing a bone marrow needle and a double-lumen tube in the infected area, through which an antibiotic at an appropriate concentration is continuously administered. CLAP has shown good outcomes in patients with bone and soft tissue infections. However, the use of CLAP for late infection after total knee arthroplasty (TKA) is rarely reported. We addressed the gap in the treatment of prosthetic joint infection (PJI) using intra-joint antibiotics perfusion (iJAP) combined with debridement, antibiotics, and implant retention (DAIR)…

Infected nonunion of the long bones.

The problems in infected nonunion include multiple sinuses, osteomyelitis, bone and soft tissue loss, osteopenia, adjacent joint stiffness, complex deformities, limb-length inequalities, and multidrug-resistant polybacterial infection. Bone gap and active infection are the crucial factors relating to treatment and prognosis. Gaps larger than 4 cm likely cannot be effectively bridged by corticocancellous bone grafting. If the limb has intact distal circulation and sensation, limb salvage and reconstruction generally is preferable to amputation. The fracture generally unites if adequate debridem

Biological Features Implies Potential Use of Autologous Adipose-Derived Stem/Progenitor Cells in Wound Repair and Regenerations for the Patients with Lipodystrophy.

A paradigm shift in plastic and reconstructive surgery is brought about the usage of cell-based therapies for wound healing and regeneration. Considering the imitations in the reconstructive surgeries in restoring tissue loss and deficiency, stem cell-based therapy, in particular, has been expected to pave the way for a new solution to the regenerative approaches. Limitations in the reconstructive surgeries in restoring tissue loss and deficiency have paved the way for new regenerative approaches. Among them, adipose-derived stem/progenitor cells (ADSCs)-based therapy could be the most promisi

Reconstruction of the Burned Nose Using an Expanded Skin-Grafted Forehead Flap, Delayed Turnover Flaps, and Indocyanine Green Perfusion Assessment.

Reconstruction of the burned nose is challenging because defects may involve external skin, internal lining, and structural support within scarred or previously grafted tissues. We report the case of a 35-year-old man who sustained an approximately 20% total body surface area flame burn involving the face, neck, chest, and upper extremities, requiring acute excision and split-thickness skin grafting. He later developed a severe postburn nasal deformity with loss of the lower third of the nose, airway compromise, and deficiency of lining, support, and external soft tissue. Both the forehead and

Antioxidant efficiency of Sarcophyton crude extract against gentamicin toxicity in male albino rats.

Gentamicin is an antibiotic widely used in treating bacterial infections. However, it's clinical interest is limited by it's toxic side effects on vital organs. Sarcophyton soft coral is a source of natural products with a range of bioactivities. As such, the objective of this work was to assess how well Sarcophyton extract mitigated the gentamicin toxicity in rats. Four equal groups, each with five adult male albino rats, were randomly assigned: the control group, the Sarcophyton group given a Sarcophyton extract dose of 200 mg/kg/day orally for seven days, the gentamicin group receiving an i

Sigmoid colon cancer presenting as a large abdominal mass accompanied by abscess and rupture: a case report and literature review.

INTRODUCTION AND IMPORTANCE: Colon cancer presenting as a large abdominal mass accompanied by abscess and rupture is rare and prone to be misdiagnosed and delayed. In addition, the treatment plan is not clear when combined with abdominal wall metastasis. CASE PRESENTATION: A 79-year-old woman presented with a large abdominal mass accompanied by abscess and rupture. It was misdiagnosed as a soft tissue infection in a local hospital, and after a comprehensive examination, it was diagnosed as sigmoid colon cancer with abdominal wall metastasis and abscess formation. The patient underwent a one-s

Soft tissue reconstruction with omental free flap in complex upper extremity injuries: report of 13 cases.

Although there is a wide list of free flaps options for soft tissue reconstruction of complex upper extremity injuries, the omental flap has some useful anatomical and biochemical advantages. We report 13 patients who underwent hand or upper extremity reconstruction with omental free flaps. Nine patients had extensive tissue damage, resulting with digital cyanosis and hypothermia, and some of them with areas of cutaneous necrosis, or avulsed tissues with tendons and bones exposed or infected. The remaining four patients had minor extensive tissue damage without circulatory problems. Patient's

Tissue engineering: revolution and challenge in auricular cartilage reconstruction.

External ear reconstruction for congenital deformity such as microtia or following trauma remains one of the greatest challenges for reconstructive plastic surgeons. The problems faced in reconstructing the intricate ear framework are highly complex. A durable, inert material that is resistant to scar contracture is required. To date, no material, autologous or prosthetic, is available that perfectly mimics the shapely elastic cartilage found in the ear. Current procedure involves autologous costal cartilage that is sculpted to create a framework for the overlying soft tissues. However, this i

Choreography of Lyme Disease Spirochete Adhesins To Promote Vascular Escape.

The Lyme disease spirochete Borrelia burgdorferi sensu lato can cause a multitude of clinical manifestations because of its ability to disseminate into any organ system via migration through soft tissue, the lymphatic system, and the circulatory system. The latter is believed to constitute the predominant pathway for dissemination to distal sites from the inoculating tick bite. In spite of its importance, the hematogenous dissemination process remains largely uncharacterized, particularly due to difficulties studying this process in a living host and the lack of an in vitro system that recapit

Free tissue transfer to the traumatized upper extremity: Risk factors for postoperative complications in 282 cases.

BACKGROUND: Complex traumatic upper extremity injuries frequently possess compromised local vasculature or extensive defects that are not amenable to local flap reconstruction. Free tissue transfer is required to provide adequate soft tissue coverage. The present study aimed to evaluate risk factors that contribute to postoperative complications and flap loss in complex upper extremity reconstruction. METHODS: Retrospective chart review was performed for all patients undergoing free tissue transfer for upper extremity reconstruction from 1976 to 2001. Data collected included patient demograph

Antibiotic Therapy in Dentistry.

Dental caries, pulpal necrosis, trauma, and periodontal diseases can result in dental infections which could have severe consequences that affect both soft and hard tissues of the oral cavity. Dental infections commonly present with symptoms of pain, fever, and swelling. Surgical and endodontic treatments are the early management of infected teeth, followed by antibiotic therapy. Some alternative methods also exist for treating infection such as low-level laser therapy and photodynamic therapy. Antibiotics are generally used in dental procedures to treat odontogenic infections, nonodontogenic
