Laminoplasty versus laminectomy and fusion for multilevel cervical myelopathy: a meta-analysis of clinical and radiological outcomes.

OBJECT Posterior cervical surgery, expansive laminoplasty (EL) or laminectomy followed by fusion (LF), is usually performed in patients with multilevel (≥ 3) cervical spondylotic myelopathy (CSM). However, the superiority of either of these techniques is still open to debate. The aim of this study was to compare clinical outcomes and postoperative kyphosis in patients undergoing EL versus LF by performing a meta-analysis. METHODS Included in the meta-analysis were all studies of EL versus LF in adults with multilevel CSM in MEDLINE (PubMed), EMBASE, and the Cochrane library. A random-effects m

Symptomatic thoracic spinal cord herniation: case series and technical report.

BACKGROUND AND IMPORTANCE: Idiopathic spinal cord herniation (ISCH) is an uncommon condition located predominantly in the thoracic spine and often associated with a remote history of a major traumatic injury. ISCH has an incompletely described presentation and unknown etiology. There is no consensus on the treatment algorithm and surgical technique, and there are few data on clinical outcomes. CLINICAL PRESENTATION: In this case series and technical report, we describe the atypical myelopathy presentation, remote history of traumatic injury, radiographic progression, treatment, and outcomes o

Intraoperative neurophysiologic monitoring in spine surgery. Developments and state of the art in France in 2011.

Intraoperative spinal cord monitoring consists in a subcontinuous evaluation of spinal cord sensory-motor functions and allows the reduction the incidence of neurological complications resulting from spinal surgery. A combination of techniques is used: somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), neurogenic motor evoked potentials (NMEP), D waves, and pedicular screw testing. In absence of intraoperative neurophysiological testing, the intraoperative wake-up test is a true form of monitoring even if its latency long and its precision variable. A 2011 survey of 117 Fre

Hemihyperplasia-multiple lipomatosis syndrome (HHML): a challenge in spinal care.

A 15-year-old girl developed a progressive paraparesis over a period of six months, secondary to spinal cord compression by a lipomatous mass and anomalies of the vertebral column. Clinically, a right hemihyperplasia affecting the trunk and lower limb was evident, as well as a right convex lumbar scoliosis. CT and MRI demonstrated severe spinal cord compression resulting from intraspinal lipomatosis, overgrowth of right facet joints (T8 to L5), and kyphoscoliosis. Surgical decompression was undertaken. A lumbar scoliosis of 48 degrees was partially corrected by means of dual-rod instrumentatio

Suspended laminoplasty.

OBJECT: Cervical laminoplasty is a recognized technique commonly used for multilevel posterior cervical decompression, and it is favored over laminectomy for maintaining spinal stability. Traditional hinge techniques, however, limit lateral exposure on one side and can limit dural exposure. The authors present their experience with a modified laminoplasty technique incorporating complete laminectomy and placement of titanium miniplate instrumentation. This method allows wide bilateral posterior decompression and unobscured dural access. METHODS: Twenty-eight patients (mean age 57 years) under

[Parkinsonism and Camptocormia with focal spinal myopathy: case report and responsiveness to treatment].

INTRODUCTION: Camptocormia is characterised by extreme flexion of the thoracolumbar spine. It suffered an increase during walking and it is relieved in supine position. Camptocormia has been described in psychogenic disorders, but in other diseases, including Parkinson's disease as well. It has been recently described several cases with focal spinal myopathy, and we present a patient with this clinical association. CASE REPORT: This 82-year-old man had a 6-year history of parkinsonian symptoms, mostly of rigid-akinetic type. He was in stage 4 on Hoehn & Yahr scale, and he had reached 62 point

[Systems analysis of X-ray films of the vertebral column].

The systems analysis of spinal X-ray films (SASXF) developed by the author solves the problem of an integral and, concurrently, detailed study of the spine of a patient on the basis of routine X-ray study. The examination of the patient gave rise to a patient's systems spinal model that considers the individual features of the vertebral column, which are most significant for a manual therapist's work. The standards of a X-ray study, a protocol form, and examination tools are described. An example of systems spinal model is given. The experience accumulated during studies of more than 2000 pati

Risks associated with spinal manipulation.

The aim of this systematic review was to summarize the evidence about the risks of spinal manipulation. Articles were located through searching three electronic databases (MEDLINE, EMBASE, Cochrane Library), contacting experts (n =9), scanning reference lists of relevant articles, and searching departmental files. Reports in any language containing data relating to risks associated with spinal manipulation were included, irrespective of the profession of the therapist. Where available, systematic reviews were used as the basis of this article. All papers were evaluated independently by the aut

Vascular anatomy anterior to lumbosacral transitional vertebrae and implications for anterior lumbar interbody fusion.

BACKGROUND CONTEXT: Anterior approaches to the lumbosacral spine afford the ideal window to the disc for interbody fusion. Vascular injuries represent the most feared complications of such approaches. Unfortunately, the combination of more procedures being performed, more surgeons at the beginning of the learning curve and less invasive techniques of approach combine to increase the risk of vascular injury in the face of altered vascular anatomy. PURPOSE: To assess cases in which vascular anatomy significantly altered the surgical approach to the lumbosacral junction. STUDY DESIGN/SETTING: C

Clinical outcomes after cervical spine fusion.

The advent of sterile technique, modern anesthesia, and organized industrial society have allowed for great advances and widespread use of cervical arthrodesis for a variety of disorders. This article defines expected outcome for cervical arthrodesis used to treat degenerative disease, trauma, deformity, and a variety of other disorders.

Operative treatment of degenerative cervical disk disease.

The surgical treatment of degenerative cervical disk disease should be considered only after an adequate trial of conservative management has failed. When surgery is contemplated, the decision must be based on sound indications and the operative strategy and approach must address the specific abnormality responsible for the patient's symptom complex.

Cervicogenic headache: a personal view.

Cervicogenic headache describes pain referred along the upper three cervical roots and is a common source of headache in the over fifties. A review of clinical features, anatomy, and investigations using neural blockade shows several clinical pictures. Diseases of the cranio-vertebral junction, e.g. syringomyelia, and tonsilar descent, can produce mechanical or hydrodynamic head and neck pain. Pre-existent migraine and tension headache may be precipitated or aggravated by cervical spondylosis, but the anatomical mechanism is unclear. Headaches which arise from and are primarily caused by disor
