Two cases of enteric myiasis in man. Pseudomyiasis and true intestinal myiasis.

Dipterous larvae were found in the appendices from two postmortem examinations. The single maggot in the first case was not immediately identified in the sections. However, when the posterior end of the larva was recovered from the unsectioned portion of the appendix, it was identified as a maggot of the genus Sarcophaga. The first case was believed to be a case of "pseudomyiasis,+ i.e., the accidental entrapment of a swallowed larva passing through the digestive tract. In the second case, the appendix contained numerous larvae with great variation in sizes and stages of development of the mou

Heartburn. The role of radiology.

There is currently controversy as to the importance of the radiologic demonstration of a hiatal hernia, reflux, or both as the explanation of heartburn. It is clear, however, that clinical-radiologic correlation requires additional observations such as the straightness of the potential path for reflux, the presence of a contractile esophagogastric region, the degree of extrinsic compression of the cuff of the stomach within the hiatus, the size of the hernia, and the peristaltic activity of the body of the esophagus. Vigorous or water-swallowing maneuvers to demonstrate reflux are unreliable i

[Report of a cased of carcinoma of the hypopharynx successfully treated by immediate pharyngogastrostomy (author's transl)].

The treatment of carcinoma of the lower part of the pharynx and upper oesophagus presents many difficulties. One of the outstanding problems being that of repair of the defect after excision of the pharyngo-oesophageal portion of the alimentary tract. This paper describes an operation in which the mobilized stomach is transposed after blunt dissection of the oesophagus via a posterior mediastinal tunnel without thoracotomy to a position high in the neck, with anastomosis of the fundus to the pharynx at the level of vallecula. This type of oesophagectomy was first tried bei Turner 1936 for carc

Association Between Texture-Modified Diet Intake Rate and Functional Outcomes After Acute Stroke: A Retrospective Observational Study.

BACKGROUND/OBJECTIVES: Texture-modified diets (TMDs) are commonly prescribed after stroke, but the clinical significance of the amount actually consumed remains unclear. We examined the association between TMD intake rate and functional status at discharge and 90 days after stroke. METHODS: This retrospective single-center study included 406 patients with ischemic stroke or intracerebral hemorrhage who received TMDs for suspected or confirmed dysphagia between 2012 and 2017. Overall TMD intake was calculated as the meal-count-weighted mean consumption proportion across four TMD levels and ana

Measurement properties of instruments for post-extubation dysphagia in critically ill patients: A systematic review.

OBJECTIVES: Post-extubation dysphagia (PED) is a common but often under-recognised complication in critically ill patients following invasive mechanical ventilation. Systematic assessment of PED in routine ICU practice is not common. This systematic review aimed to identify clinical instruments used to assess PED in critically ill adult patients and to evaluate their measurement properties. METHODS: This review was conducted in accordance with the COSMIN methodology for systematic reviews of measurement instruments. A comprehensive evidence search was performed in multiple databases and other

ZJUSXH Model: A Multilayer Model for Predicting 90-Day Neurological Outcome After Endovascular Thrombectomy in Acute Large Vessel Occlusion Stroke.

OBJECTIVES: Stroke is a leading cause of mortality and disability worldwide, particularly acute ischemic stroke (AIS) due to large vessel occlusion (LVO). Although endovascular intervention has proven effective, individual outcomes vary considerably. This study aimed to identify key prognostic factors and develop a predictive model for neurological recovery in patients with LVO-AIS following endovascular thrombectomy (EVT). METHODS: A total of 191 LVO-AIS patients who underwent EVT at the Neurosurgery Department of Zhejiang University Shaoxing Hospital from January 2022 to June 2023 were incl

Effects of a Standardised Medical-Dental Collaborative Protocol on Acute Stroke Rehabilitation: A Multicentre Prospective Cohort Study.

BACKGROUND: Acute stroke often causes complications such as aspiration pneumonia, which can be prevented through oral hygiene. However, evidence supporting individualised oral care remains limited. We developed a multidisciplinary oral function management protocol and evaluated its benefits in patients with acute stroke, including its impact on aspiration pneumonia. METHODS: In this prospective multicentre cohort study, data from 1616 patients with acute stroke admitted to participating hospitals between 31 July 2017, and 27 January 2021 were analysed. Hospitals implemented our oral function

Palliative External Beam Radiation Therapy for Dysphagia in a 101-Year-Old Man With Esophageal Adenocarcinoma.

Esophageal adenocarcinoma in centenarians is rare, and treatment options in this age group are limited. Dysphagia is often the predominant symptom that affects quality of life (QoL), and options for treatment and palliation are limited when patients are not candidates for chemoradiation and endoscopic interventions fail. We present the case of a 101-year-old male patient with symptomatic distal gastroesophageal junction (GEJ) adenocarcinoma who presented with progressive dysphagia, aspiration, and weight loss. Esophagogastroduodenoscopy (EGD) was performed to confirm the diagnosis and to place

Structural design of dysphagia-oriented double emulsions via internal-external phase coordination to modulate texture, rheology, and oral lubrication.

Dysphagia is a swallowing disorder that poses serious nutritional and safety challenges for older adults and clinical patients. Traditional thickening techniques negatively affect mouthfeel, resulting in limited tunability of texture and lubrication. In this study, a dual-parameter strategy was developed to form dysphagia-oriented double emulsions (DEs, W1/O/W2). The internal aqueous phase (ϕ = 0.10-0.25) and the external xanthan gum‑sodium alginate matrix (XG: SA = 1:7-9:1) of DEs were modulated. In combination with IDDSI flow and fork-drip tests, texture, rheology, and oral tribology measure

Age better discriminates early dysphagia than cough function, respiratory muscle strength, or trunk control after mild stroke.

This retrospective study aimed to identify factors that best discriminate the presence of early dysphagia after mild stroke. We performed a retrospective analysis of prospectively collected routine-care data from consecutive first-ever subacute stroke patients. We included 140 first-ever subacute stroke patients greater than 18 years or older, within 3 weeks of stroke onset, with hemiparesis and a Mini-Mental State Examination score greater than 24, recruited from an inpatient rehabilitation unit. Dysphagia was assessed using the Volume-Viscosity Swallow Test and videofluoroscopy, with swallow

Efficacy of scalp acupuncture for post stroke dysphagia: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Stroke persists as a paramount global health priority, maintaining persistently elevated incidence and mortality metrics across epidemiological registries. Post-stroke dysphagia, serving as a critical determinant of functional prognosis, imposes multidimensional burdens spanning aspiration pneumonia risk, nutritional compromise, and psychosocial well-being. This neurogenic deglutition disorder has consequently catalyzed clinical innovation in rehabilitation modalities, with scalp acupuncture emerging as a promising neuromodulatory intervention. METHODS: We conducted a systematic s

Clinical utility of voluntary cough peak expiratory flow outcomes to screen aspiration status in head and neck cancer survivors.

PURPOSE: Peak expiratory flow of voluntary cough (PEF) is a physiologic metric indexing an individual’s physiologic capacity to defend the airway by expelling tracheal aspirate. PEF can be obtained in an objective, quick and inexpensive manner in busy clinical settings, with demonstrated discriminant ability to detect unsafe swallowing in neurogenic populations. This study examined the discriminant ability of voluntary cough PEF to classify aspiration status in head and neck cancer (HNC) patients post-radiotherapy (RT). METHODS: This was a secondary analysis of a prospective clinical trial of

DoE-Optimized Minitabs: Fabricating a Promising Modified-Release Sprinkle Formulation.

INTRODUCTION: Dysphagia is a challenging medical condition that significantly affects individuals' swallowing function, quality of life, and overall well-being, especially in geriatric and pediatric populations. Sprinkle formulations are emerging as a broadly accepted solution to this challenge. However, such formulations are quite challenging to formulate as modified-release delivery systems. The present study aims to resolve this issue by formulating modified-release minitabs of the model drug etoricoxib as a sprinkle formulation. METHODS: A fabricated metal mold and punch system was used t

Maximum pharyngeal constriction area: an independent predictor of sarcopenic dysphagia after esophageal cancer surgery.

The exact mechanism of sarcopenic dysphagia following esophagectomy, which is associated with reduced pharyngeal contraction, remains unclear. This study measured the maximum pharyngeal constriction area normalized (MPCAn) as a potential indicator of post-esophagectomy sarcopenic dysphagia. Videofluoroscopic examination of swallowing study was conducted in 134 patients with suspected dysphagia following esophagectomy, defined as a score of ≥3 in the 8-point penetration-aspiration scale. The area under the receiver operating characteristic curve of MPCAn was applied to detect dysphagia in compa
