We evaluated the diagnostic yield of event loop recorders in ambulatory patients referred for palpitations, dizziness or syncopal events and in whom a previous Holter recording was not diagnostic. A total of 96 patients were studied, 50 (52%) were men and 46 (48%) were women with a mean age of 37+/-10 years. Mean duration of the recording was 5.2+/-2.3 days. The purpose of this study was to evaluate longterm mortality in 44 patients undergoing electrophysiologically guided therapy for sustained monomorphic ventricular tachycardia. We applied the following modified response criteria: non-inducibility or slowing of induced ventricular tachycardia to < or = 150 bpm. On this basis, 25/44 (57%) patients were classified as responders, and 19/44 (43%) as non-responders. This paper describes the history of an 81-year-old female suffering from a giant dissecting aortic aneurysm with concealed perforation within the thorax. The patient had suffered from arterial hypertension for about 10 years and had been treated with thiazide. Nine months prior to admission the patient was in a state of collapse, and ultrasound examination revealed an intra-abdominal aortic aneurysm.
