Chest injuries in children.

Chest injuries of varying severity occur commonly in children, much more as a result of motor vehicle accidents and falls from a height than from penetration. The author reviews the injuries that occur with reference to mechanism, immediate and late treatment and monitoring. The differences between the injuries of children and those of adults stem from the greater resilience of most structures in children; bony injuries are not necessarily extensive even when disruptions of the lung and blood vessels are serious. Pneumothorax may occur alone or with other injuries. A chest tube should be inser

Cystic duplications of the esophagus and neurenteric cysts.

This paper reviews the past 25 years of experience with these lesions at The Hospital for Sick Children, Toronto, during which 19 infants and children were treated. There were 8 females and 11 males. Nine of these patients were under 1 year of age at presentation, 5 were between the ages of 1 and 10 years, 5 were older than 10 years. Eleven presented with symptoms referrable to the chest, and six with predominantly neurological symptoms. Two asymptomatic children were referred because of chest masses found incidentally on chest x-rays. Fifteen chest masses were noted; the other four had neurol

The use of different induction and maintenance chemotherapy regimens for the treatment of advanced yolk sac tumors.

Four children with yolk sac tumor were treated with an aggressive combination chemotherapy program. Three children had presacral primary tumors, one having pulmonary metastases, and one had a testicular primary tumor with pulmonary metastases. Three children were treated when they had measurable disease, and one had no measurable disease. The chemotherapy program consisted of a 6-wk induction period with vincristine (VCR), cis-diamminedichloroplatinum (DDP), and bleomycin. Maintenance therapy consisted of VCR, actinomycin D, and cyclophosphamide (cytoxan) every 3-4 wk as tolerated. Treatment w

Immunological markers and DNA content in a case of giant lymph node hyperplasia (Castleman's disease).

Little is known about the immunologic characteristics of the cells in giant lymph node hyperplasia (GLNH). For this reason, cell surface markers and intracytoplasmic immunoglobulins were determined on a case of GLNH. In addition, cellular DNA content was determined by flow analysis. A 59-year-old male underwent thoracotomy for a posterior madiastinal mass, which was entirely excised. Histologically, the mass was diagnosed as GLNH with features of both the hyaline-vascular and plasma-cell types. Preoperatively, the patient had a broad-based hypergammaglobulinemia with an increase in serum IgG.

1-Deamino-8-d-arginine vasopressin: a new pharmacological approach to the management of haemophilia and von Willebrands' diseases.

1-Deamino-8-d-arginine vasopressin (D.D.A.V.P.) infusion causes a marked increase in factor-VIII (antihaemophilic-factor)-related properties in patients with moderate and mild haemophilia and von Willebrand's disease (vWd). The possibility was therefore evaluated that such an autologous factor-VII response might be haemostatically effective, allowing patients to undergo surgery without plasma concentrates. 0.3 microng/kg of D.D.A.V.P. given before dental surgery and repeated in the early postoperative period was followed by a two to three fold rise in factor-VIII coagulant activity (VII C.A.)

Haemodynamic and respiratory changes accompanying haemorrhagic shock in thoracotomized dogs.

Haemorrhagic shock was induced after thoracotomy by bleeding into a reservoir and the reduction of systemic blood pressure to 40 mm Hg. After two hours the blood was retransfused. The haemodynamic and respiratory changes due to the haemorrhage and retransfusion were noted during the two hour shock period and one hour after transfusion. The results were as follows: 1. Pulmonary mean and systemic mean pressure during haemorrhage and retransfusion do not change in parallel; 2. there is a rapid rise in pulmonary arterial [PA] and pulmonary capillary [PC] pressure during retransfusion; 3. cardiac o

Inefficacy of "therapeutic" serum levels of digoxin in controlling the ventricular rate in atrial fibrillation.

Although therapeutic and toxic serum concentrations of digoxin have been established, there is sparse information permitting correlation of drug level with clinical effect. This study was undertaken to assess the radioimmunoassay serum digoxin levels in 30 patients with acute atrial fibrillation (38 determinations) and 30 patients with chronic atrial fibrillation (54 determinations). Those with chronic fibrillation were subdivided into those in clinically stable condition (14 patients), and those seriously ill and in clinically unstable condition (16 patients). Slowing of ventricular rate in p

Robot-assisted thoracoscopic fissureless fissure-last approach versus conventional interlobar method for left lower lobectomy in lung cancer: a comparative study of short-term outcomes.

OBJECTIVES: Incomplete interlobar fissure complicates lobectomy and raises the risk of prolonged postoperative air leak. Although the fissureless fissure-last approach can reduce air leak in thoracoscopic surgery, its applicability in robot-assisted thoracoscopic surgery for left lower lobectomy remains uncertain. This study evaluated the feasibility of the fissureless fissure-last approach by comparing perioperative outcomes with those of the conventional interlobar approach. METHODS: This single-center retrospective study included 53 patients with primary lung cancer who underwent robot-ass

CA19-9-producing esophagogastric junction cancer with nodal recurrence outside the planned dissection field: a case report.

INTRODUCTION: The biological behavior of CA19-9-producing esophagogastric junction cancer (EGJC) remains poorly understood, although some reports suggest that CA19-9 expression reflects a more aggressive subtype. PRESENTATION OF CASE: We report a 64-year-old man with Siewert type II EGJC, presenting with 3.5 cm of esophageal invasion and markedly elevated serum CA19-9 levels (11 630 U/mL). Following neoadjuvant chemoradiotherapy, thoracoscopic Ivor Lewis esophagectomy with lower mediastinal lymphadenectomy was performed. Histopathology confirmed a CA19-9-producing adenocarcinoma. Twenty month

Hybrid Minimally Invasive Management of Thoracic Esophageal Perforations: Technical Feasibility in Selected Patients.

BACKGROUND: Thoracic esophageal perforation (EP) is a life-threatening emergency. While minimally invasive techniques have been successfully utilized, the heterogeneity of clinical presentations (perforation location, delays in treatment, tissue quality, etc.) has prevented the establishment of a standardized management approach. We present our experience with a minimally invasive management protocol combining thoracoscopy and endoscopy. METHODS: Between December 2023 and January 2025, 5 patients with thoracic EP underwent a thoracoscopic approach. Primary closure via direct suturing, lavage,

Unveiling Hidden Variations in Lung Fissures: A Cadaveric Perspective on Surgical and Diagnostic Challenges.

Introduction Anatomical variations in lung fissures arise from incomplete obliteration of intersegmental planes during embryogenesis, resulting in incomplete, accessory, or absent fissures. These variants pose challenges in thoracic surgery, increasing risks of air leaks, prolonged operative times, and conversion to thoracotomy, while complicating radiological interpretation. Materials and methods This descriptive cross-sectional study aimed to characterise the patterns and completeness of major lung fissures in adult cadaveric lungs and to compare these features between the right and left sid

Redo Aortic Root and Total Arch Replacement for a Giant Ascending and Arch Aneurysm After a Previous Double Mechanical Valve Replacement.

Redo replacement of the ascending aorta and aortic arch after previous double mechanical valve replacement is rare and technically challenging. Surgical re-entry is particularly hazardous in the presence of giant aneurysms because dense adhesions, distorted mediastinal anatomy, and prosthetic valves increase the risks of complications. We report the successful management of a giant compressive aneurysm using a staged strategy incorporating pre-sternotomy left ventricular (LV) apical decompression. A 63-year-old man with previous mitral commissurotomy and double mechanical mitral-aortic valve r

Successful lung-preserving tractotomy to treat intrathoracic wood impalement after high-fall trauma: a case report.

BACKGROUND: Thoracic impalement injuries caused by wooden objects are extremely rare and present unique diagnostic and therapeutic challenges owing to the radiolucent nature of wood and the risk of organic contamination. Lung-preserving tractotomy is a valuable, but infrequently reported technique for managing penetrating lung trauma caused by wooden foreign bodies. CASE PRESENTATION: A 39-year-old healthy arborist fell from a height of approximately 20 m during tree felling. On presentation, the patient was hemodynamically stable, with a small anterior chest wound and subcutaneous emphysema.
