Changing trends in infectious disease in heart transplantation.

BACKGROUND: During the past 25 years, advances in immunosuppression and the use of selective anti-microbial prophylaxis have progressively reduced the risk of infection after heart transplantation. This study presents a historical perspective of the changing trends of infectious disease after heart transplantation. METHODS: Infectious complications in 4 representative eras of immunosuppression and anti-microbial prophylaxis were analyzed: (1) 38 in the pre-cyclosporine era (1978-1980), (2) 72 in the early cyclosporine era (1982-1984), where maintenance immunosuppression included high-dose cyc

Vaginal tuberculosis in an elderly kidney transplant recipient.

Female genital tuberculosis (FGTB) is extremely rare in post-menopausal women. A 59-year-old woman developed vaginal tuberculosis one year after receiving a kidney transplant from a living donor. Her complaints included abdominal pain, fever, and weight loss. Furthermore, her tuberculin skin test was negative. She was successfully treated with quadruple anti-TB therapy for 6 months.

Allogeneic hematopoietic cell transplantation for adults with ALL.

ALL is a heterogeneous disease, and outcomes vary by patient age, immunophenotype and clinical, cytogenetic and molecular features. Modern treatment strategies use a risk-adapted approach. The optimal post-remission therapy for adults with ALL remains unclear. Available data indicate no consensus as to whether there is an advantage to allogeneic hematopoietic cell transplant (Allo-HCT) over chemotherapy for the consolidation of adults with ALL with standard risk features while in the first CR (CR1). However, Allo-HCT is recommended in CR1 for patients with high-risk ALL, for those in a second

[Long-term results after liver transplantation].

Liver transplantation has been reported to reach excellent results for selected indications. We analysed the results of liver transplantation in our centre over a period of 23 years, with a total of 2,114 consecutive liver transplants in 1,773 patients (eras I-III 5.5 years each, era IV 6.5 years). Overall 20-year survival after liver transplantation was 29.8%. The most frequent leading causes of death were infections of various origins (30%), tumour recurrence (14.2%), and pneumonia (8.4%). The most frequent leading causes for graft loss were infection of various origins (19.6%), initial nonf…

Successful cord blood transplantation for mycosis fungoides.

A 26-year-old female diagnosed as mycosis fungoides (MF, clinical stage IV) was treated with single-agent chemotherapy, multi-drug chemotherapy and unrelated bone marrow transplantation with reduced-intensity conditioning (engraftment failure), resulting in failure. Unrelated cord blood transplantation (CBT) as second transplantation following myeloablative conditioning brought complete remission (CR), but relapse of MF occurred 3 months after transplantation. However, discontinuation of immune suppressant led to the regression of MF regions and to second CR that continued for more than 23 mon

Significance of central perivenulitis in pediatric liver transplantation.

Central perivenulitis (CP) encompasses dropout of zone 3 hepatocytes, red blood cell extravasation, and perivenular mononuclear inflammation. In the liver transplant setting, CP can occur in isolation or it can occur in association with portal-based disease such as acute cellular rejection (PB-ACR). Some CP is also thought to be a manifestation of chronic rejection, particularly when accompanied by zone 3 fibrosis. Prior studies of CP in pediatric liver allografts have been hampered by lack of protocol biopsies and low rates of histologic follow-up. We studied 62 consecutive liver allografts f

Safety of minimal immunosuppression in liver transplantation for hepatoblastoma.

BACKGROUND: Despite aggressive chemotherapy, recurrence of disease remains the leading cause of death after liver transplantation (LTx) for hepatoblastoma (HB). Unfortunately, little is known about the effects of immunosuppression on recurrence and posttransplant outcomes. We hypothesized that minimal immunosuppression can be safely used in these recipients. METHODS: In 2004, we adopted a minimal immunosuppression regimen using daclizumab induction and tacrolimus monotherapy. Kaplan-Meier survival curves were generated. RESULTS: From 2004 to 2006, 6 children underwent primary LTx for HB with

Liver transplantation for primary and metastatic liver cancers.

Liver transplantation for hepatic malignancies has emerged as a well-documented and proven treatment modality. However, early unsatisfactory results emphasized that only a highly selected patient population would benefit from transplantation. Currently, 15% of all liver transplants performed are for hepatocellular carcinoma (HCC). There is no controversy about the fact that liver transplantation for HCC in the adult population yields good results for patients whose tumour masses do not exceed the Milan criteria. It remains to be determined whether patients with more extensive tumours can be re…

Helicobacter pylori in solid-organ transplant recipient.

PURPOSE OF REVIEW: Helicobacter pylori (H. pylori) infection induces a range of upper gastrointestinal diseases including chronic gastritis, peptic ulcer disease, mucosa-associated lymphatic tissue (MALT) lymphoma, and gastric adenocarcinoma with marked individual variability. In the present review, we survey the published reports on H. pylori infection in organ transplant recipients and summarize findings about the prevalence of H. pylori infection and outcomes such as peptic ulcer and MALT lymphoma induced by H. pylori infection after transplantation. RECENT FINDINGS: The prevalence of H. p

Should all unrelated donors for transplantation be matched?

Previous analyses have suggested that hematopoietic cell transplantation (HCT) from an unrelated donor results in better survival if the patient is younger and, possibly also if the donor is younger. Additionally, survival is improved if HCT is performed during early disease stage and if the recipient and possibly the donor are cytomegalovirus (CMV) seronegative. Equivocal data have been published comparing bone marrow vs. (granulocyte-colony stimulating factor) G-CSF-stimulated peripheral blood stem cells for transplantation. A randomized trial is underway by the Blood and Marrow Transplant C

[The incidence of tertiary hyperparathyroidism after renal transplantation].

INTRODUCTION: Tertiary hyperparathyroidism (3 degrees HPT) is a condition characterized by autonomous secretion of parathyroid hormone (PTH) which develops as a consequence of secondary hyperparathyroidism in patients with chronic renal failure. Severe cases of 3 degrees HPT are treated with parathyroidectomy (Ptx). The purpose of this study was to uncover the incidence of 3 degrees HPT and frequency of Ptx after renal transplantation (Tx). Furthermore, we wanted to identify parameters that predicted the development of 3 degrees HPT and the prognosis. MATERIALS AND METHODS: The study included

[Stem cell transplantation in the treatment of gastrointestinal diseases].

The stem cells with self-renewal ability are capable to form one or more cell types. They will be in the target of cell and gene therapy because of their multipotency and easy retrieval. Application of adult mesenchymal, neuronal, epidermal and haematopoietic stem cell can be favourable in the treatment of cardiac (myocardial infarction), bone (osteoarthritis), neurological (Parkinson's, Alzheimer's) and hematological (hemophilia, thrombocytopenia) disorders. Authors summarize the knowledge in connection with their application in the therapy of gastrointestinal diseases. Haematopoietic stem ce

[Dilutional hyponatremia, hepatorenal syndrome and liver transplantation].

Cirrhosis is a chronic, progressive disease characterized by complications associated with portal hypertension and liver failure. Renal function disorders are a common complication in patients with cirrhosis and are associated with high morbidity and mortality and poor prognosis. Renal function alterations in these patients include sodium and water retention and renal vasoconstriction. Sodium retention causes the formation of ascites and edema, solute-free water leads to dilutional hyponatremia, and renal vasoconstriction gives rise to the development of hepatorenal syndrome (HRS). Due to thei

Emerging trends in transplantation of inherited metabolic diseases.

Allogeneic hematopoietic stem cell transplantation (HSCT) can prolong life and improve its quality in patients with inherited metabolic diseases. HSCT offers a permanent source of enzyme replacement therapy and also might mediate nonhematopoietic cell regeneration or repair. Unrelated cord blood is an exciting newer graft source for treatment of patients with these fatal disorders, providing increased access to donors and significant clinical efficacy, particularly when transplantation is performed in early stages. Pre-transplant performance status is highly predictive of overall survival.

Reversibility of cachexia after bilateral lung transplantation.

BACKGROUND: Cachexia is one of the terminating syndromes in pulmonary disease, and cachectic patients have an increased mortality even after lung transplantation (LTx). Our study assessed weight changes in cachectic patients after LTx compared to normal weight LTx recipients, and investigated differences in gender, chronic obstructive pulmonary disease (COPD), cystic fibrosis (CF), and primary pulmonary hypertension (PPH). METHODS: We prospectively examined 46 patients before and serially 3, 6, 12 and 24 months after LTx (25 female, 21 male; median age at transplantation 46.9+/-13.9 years, n=

Choledocho-choledochostomy in deceased donor liver transplantation.

Biliary complications following deceased donor liver transplantation occur with an incidence of approximately 5-10%. The most common type of biliary reconstruction in whole-organ deceased donor liver transplantation remains the choledocho-choledochostomy, which creates an anastomosis between the donor and recipient common bile ducts or common hepatic ducts. Key elements in performing a successful choledocho-choledochostomy include ensuring that bile ducts have adequate blood supply and avoiding mechanical trauma or tension on the anastomosis. Techniques including ductoplasty and spatulation ca

The time is now: formation of true transplant centers.

With the advances in the complexity of medical care there is a need to change the way we are organized in order to deliver that care in the most patient-friendly, efficient manner. This need is perhaps most compellingly manifest in the field of whole organ transplantation. Yet we are trying to deliver medical care with an organizational structure that was developed over 100 years ago when medical practice was considerably different. We argue for the development of a vertically integrated organizational structure in academic health centers for certain disciplines like transplantation. While thi

Variant techniques for liver transplantation in pediatric programs.

INTRODUCTION: Several variant techniques have been developed as alternatives to whole liver transplantation to improve size matching, timing, or simply to increase the pool of donors. The aim of this study was to assess the requirements of these techniques and their outcomes in a pediatric transplant program. PATIENTS AND METHOD: A retrospective analysis of children on the waiting list in the last 4 years was carried out. Data of patients who died while on the waiting list (WL) were recorded. Transplanted patients were divided according to the type of graft: whole liver, split, living donor a

Rectal carcinoid tumor in a renal transplant patient.

Malignancy represents a leading cause of morbidity and mortality in patients with a long-term surviving graft. Carcinoid tumor is a common primary endocrine tumor in the general population that is rare in transplant recipients. Our present report focused on a 48-year-old man who received immunosuppressive therapy based on cyclosporine and steroids. Twelve years after renal transplantation, he suffered watery diarrhea and abdominal discomfort. Colonoscopy showed a hard, sessile mass at 5 cm from the anal verge; endoscopic ultrasound showed a 13-mm homogenous hypoechoic mass. Upon endoscopic bio
