UNLABELLED: In the Department of Surgery, University of Turku, 310 patients underwent parietal cell vagotomy for duodenal (268 patients) or pyloric-prepyloric ulcer (42 patients) in the years 1973-82. The male/female ratio was 4/1 and mean age 43 years. There was no mortality. Splenic injury led to splenectomy in 2.6%. A relaparotomy for intraabdominal bleeding was done in 1%. No case of minor curve necrosis occurred. During the follow-up of 3-9 (mean 5) years 9 patients had died of unrelated causes and 29 could not be traced leaving 272 patients for study. With the carrageenin food oedema assay, we studied in rats the effects of indomethacin on inflammation, gastric ulcers and lipid peroxide levels. Our results show a positive correlation between ulceration and plasma lipid peroxides. We suggest that the ulceration could be due to inhibition of prostacyclin synthetase by lipid peroxides.

Based upon the experience with the treatment of 2312 patients with ulcer disease of the stomach and duodenum the authors analysed the major tasks which must be solved for obtaining better immediate and long-term results of the surgical treatment. In 1880 (81,3%) of the patients with a gastric ulcer and a complicated ulcer of the duodenum the stomach resection gave favorable outcomes in 98,6%. Fifty patients undergoing elective vagotomy for the treatment of chronic duodenal ulceration have been investigated pre-operatively and again 3 months postoperatively to determine the extent and severity of associated gastro-oesophageal reflux. A prospective trial of extemporaneous cytology has been conducted in 113 patients with gastric ulcer. The diagnostic effectiveness was 97% but the yield of traditional biopsy in 13 patients with malignant ulcer was 52%. Cyto-guided biopsy, performed in 46 patients (8 with malignant ulcer), increased the yield to 97%.

Remote results within the terms of from 1 to 3 years in 60 patients and the working ability in 73 patients subjected to selective proximal vagotomy (SPV) or SPV with drainage of the stomach were studied. Functional disorders were observed much more rarely after the isolated SPV. The use of benzohexasonium and electrostitmulation of the stomach in the early postoperative period allows preventing the development of functional disorders. The SPV ensures an early recovery of the working ability.

A multicenter trial of oral ranitidine 150 mg bid was conducted in 41 patients with duodenal and 30 with gastric ulcers. Patients were randomly allocated in double-blind fashion to 4 wk treatment with either ranitidine or placebo, after which all unhealed patients were given 4 wk on the active drug without breaking the original allocation code. After 4 wk of treatment the healing rate associated with ranitidine was significantly superior to that of placebo in both duodenal and gastric ulcer patients.
