[Scimitar syndrome in an adult - partial anomalous pulmonary venosus connection].

Scimitar syndrome is a type of partial anomalous pulmonary venous connections, in which part or even the entire right lung is typically drained by right pulmonary veins that anomalously drain into the inferior vena cava. The affected lung is often hypoplastic. The severity of clinical signs and symptoms depend on the degree of a left-to-right shunt and the presence of pulmonary defects. We present a 60 year-old women who has been asymptomatic for a long time.

Coronary arteriovenous fistula.

Congenital coronary arteriovenous fistulas represent abnormal communication between coronary arteries and any cardiac chambers, pulmonary artery, coronary sinus, superior vena cava or pulmonary vein. We reported an interesting case of very long and tortuous coronary arteriovenous fistulas originated from the left circumflex coronary artery, which drained into bronchial arteries in right lower lung field diagnosed by coronary angiography and multidetector computed tomography.

Caval agenesis with a hypoplastic left kidney in a patient with trauma on warfarin for deep vein thrombosis.

Congenital anomalies of the inferior vena cava (IVC) are rare, but recognized, causing deep venous thrombosis. We present a case of a 50-year-old patient with trauma who suffered an intracranial hemorrhage secondary to a fall while on anticoagulation for deep vein thromboses. Venous return from the lower extremities was determined to be through dilated lumbar venous collaterals into the azygous and hemiazygous systems. A second interesting anatomic finding was a hypoplastic left kidney.

Cryptogenic stroke in the presence of venous bronchopulmonary collateralisation.

We report the case of a 37-year-old man with a history of Hodgkin lymphoma and paroxysmal atrial fibrillation, seen because of a transient ischaemic attack with symptoms of temporary aphasia. Transoesophageal echocardiography could not detect a patent foramen ovale. However, CT thorax and flebography showed bilateral occlusion of both subclavian veins and the superior vena cava with an important collateral venous network, most likely responsible for this rare case of acquired cryptogenic stroke.

Long-term survival after salvage surgery for invasive thymoma with intracardiac extension.

We report a case of successful salvage surgery for invasive thymoma initially judged to be unresectable that did not respond to sequential chemoradiotherapy. The patient underwent en bloc resection of the tumor, superior vena cava, upper portion of the right atrium (RA) and intracardiac neoplastic thrombus with the aid of a cardiopulmonary bypass without cardiac arrest. The patient is disease free 8.5 years after radical thymectomy and subsequent resection of 2 second primary lung adenocarcinomas.

Hemoptysis complicating Scimitar Syndrome: from diagnosis to treatment.

We report the case of a 24-year-old patient with known scimitar syndrome presenting with hemoptysis. Multidetector CT angiography showed the scimitar vein draining the right lung to the inferior vena cava associated with right lower lobe bronchopulmonary sequestration. The presence of hemoptysis associated with scimitar syndrome is likely secondary to an anomalous systemic arterial blood supply. We treated this patient successfully with transcatheter occlusion of the anomalous feeding arterial vessels.

Teddy bear in the heart.

In a patient with native aortic valve endocarditis, transoesphageal echocardiography yielded a teddy bear appearance which is not reported so far. A perivalvular abscess (right ear), the superior vena cava in cross section (left ear) and the dilated (post-stenotic) aortic root (face) made up the teddy bear. This was not a cuddlesome toy but an ominous sign. The genesis of perivalvular abscess as well as the role of transoesphageal echocardiography in its diagnosis and treatment are briefly reviewed.

[Right atrial and right ventricular thrombus in a patient with hepatic carcinoma - a case report].

A case of a 62-year-old male with suspected aortic dissection is presented. Echocardiography did not confirm the initial diagnosis and revealed the presence of thrombus in the inferior vena cava, right atrium and right ventricle. Computerised tomography, followed by histological examination of liver biopsy, revealed hepatic carcinoma. The patient received anticoagulation and unfortunately died 14 days later. Symptomatology of the right-heart thrombus and thrombo-embolic complications in patients with neoplastic disorders are discussed.

Successful resection of a primary liposarcoma in the anterior mediastinum in a child: report of a case.

Primary liposarcomas of the mediastinum are very rare. We report on a 13-year-old girl who presented with a huge mediastinal tumor. The tumor was extirpated by a median sternotomy with a right thoracotomy. The tumor included the superior vena cava in the anterior mediastinum. It therefore probably originated from the anterior mediastinal fat tissue, possibly from the thymus. A pathological examination revealed myxoid liposarcoma. At 35 months postoperatively, the patient has not shown any recurrence.

[Agenesia of the canal of Arantius. A case report].

Looking for the etiology of hydramnios which became symptomatic at 21 weeks' gestation ultrasonography revealed a hepatic vascular abnormality without other symptoms. The suspected diagnosis was agenesis of the ductus venosus with creation of a high grade arteriovenous shunt between the umbilical vein and the inferior vena cava. Agenesis of the ductus venosus may be one expression of the different possible systemic-portal-umbilical abnormalities. Physiological consequences vary according to the type of substitutive anastomoses.

[Congenital arterio-venous coronary fistula associated with patent ductus arteriousus: description of a clinical case and review of the literature].

Congenital coronary artery fistulae are rare anomalies (0.27-0.4% of all congenital heart defects) and consist of a communication between a coronary artery and a cardiac chamber, a great artery or the superior vena cava. The association of these congenital anomalies with other congenital cardiovascular defects is unusual. The purpose of this paper is to report a case of congenital coronary fistula between the anterior descending coronary artery and the pulmonary artery associated with patent ductus arteriosus and to review the literature on the subject.

Budd-Chiari syndrome: successful treatment by percutaneous transluminal angioplasty.

Budd Chiari syndrome is a rare disorder. It is caused by obstruction of the hepatic veins. The prognosis is usually dismal, except in the rare case where the obstruction is due to either webs in the hepatic veins or membranous obstruction of the inferior vena cava. Such cases, more common in the Orient, can be treated by percutaneous transluminal angioplasty. We report a case of Budd Chiari syndrome, due to a membranous obstruction in the right hepatic vein, successfully treated by percutaneous transluminal angioplasty.

Unusual primary locations of hydatid disease. Case report.

Two unusual cases of primary hydatid disease are described. In one of them a retroperitoneal cyst, presenting as a palpable abdominal mass, was strongly adherent to the inferior vena cava and dislocated the right kidney. Partial cystectomy and drainage were performed. The other cyst, of the right ischiorectal fossa, was initially misinterpreted as a perineal of sciatic hernia. It was completely removed. No other site of hydatid disease was found and the patients remain well 3 and 4 years postoperatively.

Coronal sonography: a review of abdominal applications.

Many abdominal structures are optimally imaged in the coronal plane because of their unique shape or environment. Routine application of coronal ultrasound scanning to the spleen, kidneys, and liver is advocated. Adjunctive coronal images of the biliary tract, inferior vena cava, aorta, and retroperitoneum complement or frequently provided diagnostic information unavailable in routine views. Inadequacy of acoustic windows or limitations of tissue-beam interaction encountered in transverse and sagittal projections may be overcome with coronal scans.

[Anuria caused by perianeurysmal retroperitoneal fibrosis. Medical treatment. A case report].

After 3 sessions of haemodialysis, renal function returned to normal, but the bilateral ureteral obstruction recurred 11 months later with signs of compression of the inferior vena cava. Corticosteroid therapy in doses of 0.5 mg/kg/day resulted in disappearance of the pyeloureteral dilatation at intravenous urography and regression of the fibrosis on CT sections. From a review of the literature the authors describe the clinical symptoms, diagnostic methods and main treatments of perianeurysmal retroperitoneal fibrosis, a not uncommon disease.

[Value of cineradiography in the study of Budd-Chiari syndrome. Value of various angiographic technics. Apropos of 8 cases].

Eight cases of surgically proven Budd Chiari disease, have been explored pre- operatively by cine-angiography. This procedure is very safe and simple, and allows a dynamic study of the intra- and extra-hepatic pathways. The authors are reviewing the different modalities of radiologic evaluation of Budd-Chiari disease, including inferior vena cava phlebography, opacification of supra-hepatics veins, and coeliac arteriography. They propose a flow sheet in the radiological work-up of these patients.

Use of a split-sheath vein introducer for subclavian venipuncture in the placement of silicone catheters for chronic venous access.

The technique of venous cutdown for placement of silicone catheters can be difficult, uncomfortable for the patient, and very time-consuming. Adapting a technique used for the placement of permanent transvenous pacemaker leads, subclavian venotomy and a split-sheath dilator can be used for direct placement of large-bore catheters into the superior vena cava. This approach decreases patient discomfort and the likelihood of infectious or hemorrhagic complications of silicone catheter placement, and has greatly simplified and shortened this procedure.

[Basics on the application technics of infusions].

For the application of infusion solutions the subclavian catheter is best suited. The sometimes serious complications resulting from it can only be avoided by exact insertion technique and observation of the contraindications. In rare cases the vena-cava-inferior catheter serves as an alternative access. For short-term infusions and in the case of non-occurring circulation centralisation arm and leg venipuncture can be applied. Due to frequent complications and difficulties arising during insertion the brachial vein catheter is contraindicated.

Exercise training and vascular cell phenotype in a swine model of familial hypercholesterolaemia: conduit arteries and veins.

What is the central question of this study? Does endurance exercise training cause anti-atherogenic effects on the endothelium in a swine model of familial hypercholesterolaemia (FH), and how are these effects distributed across veins, arteries and multiple vascular territories within each system? What is the main finding and its importance? Coronary artery endothelium-dependent vasomotor function was depressed in sedentary FH pigs compared with sedentary control animals, and exercise training did not change vasomotor function within FH. In systemic conduit arteries and veins, few effects of F
