Chronic intestinal pseudoobstruction associated with fetal alcohol syndrome.

Alcohol acts as a teratogen in the fetus, resulting in prenatal or postnatal growth failure, characteristic facial dysmorphic features, and central nervous system dysfunction. The toxic effects of alcohol on the developing brain are well recognized, but gastrointestinal neuropathy has not been described in fetal alcohol syndrome (FAS). Five children with FAS presented in infancy with signs and symptoms suggestive of chronic intestinal pseudoobstruction. They were not able to sustain adequate caloric intake by mouth, and all required prolonged special methods of alimentation. We performed antro

Cognitive and behavioral deficits associated with parental alcohol use.

OBJECTIVE: To review and synthesize the scientific literature on cognitive and behavioral deficits associated with parental alcohol use and to highlight areas for future attention. METHOD: Studies of children of alcoholic parents (generally fathers) and of children prenatally exposed to alcohol were reviewed, focusing on cognitive and behavioral findings. Relevant animal studies were also reviewed. RESULTS: Large numbers of children may be affected by parental alcohol use. Prenatal alcohol exposure is frequently associated with specific cognitive and behavioral deficits. Children of alcoholi

Cognitive deficits in nonretarded adults with fetal alcohol syndrome.

Persons with fetal alcohol syndrome (FAS) who are not mentally retarded often have difficulty qualifying for special educational and vocational services. In this pilot study, 16 nonretarded young adults with FAS were divided into two groups--one with average to above-average IQ and one with borderline to low-average IQ. Participants in both groups manifested clear deficits on neuropsychological measures sensitive to complex attention, verbal learning, and executive function. The frequency and severity of cognitive impairment demonstrated in both FAS groups were greater than what would have bee

Rapid clinical evaluation of anosmia. The alcohol sniff test.

BACKGROUND: Smell impairment affects 1% to 2% of Americans and leads to frequent physician visits. Olfactory testing is available in chemosensory centers, but not as part of a routine cranial nerve examination. The alcohol sniff test (AST), which uses the standard 70% isopropyl alcohol pad, was developed as a quick, reliable measure of olfactory function. METHODS: Sixty-four patients and 36 healthy control subjects (N = 100) were tested with the AST and with a standard butanol threshold test. RESULTS: The AST reliably, consistently, and correctly measured olfactory function. CONCLUSIONS: Th

Alcohol induces formation of morphine precursors in the striatum of rats.

Dopamine-derived alkaloids, the tetrahydroisoquinolines (TIQs), are suspected to play a role in the pathogenesis of alcoholism. The present study describes the alcohol induced formation of the S-enantiomer of Tetrahydropapaveroline and Norcoclaurine in the rat brain. These compounds are of special interest since both were found as being intermediates in the biosynthesis of morphine in the opium poppy. The concentration of both TIQs were determined in different brain regions of Wistar rats after 6 and 18 months alcohol consumption ad libitum. Gas chromatography/mass spectrometry was used for th

[Acute kidney insufficiency in an alcoholic patient with liver cirrhosis].

We report on a 67-year-old patient who was hospitalized because of worsening of general condition, generalized edema and ascites with decompensated ethylic cirrhosis of the liver. On hospital admission renal failure was present which had developed within 5 months. In spite of intensive treatment with diuretics and repeated ascites punctures, fluid repletion reappeared repeatedly. Renal function remained stable for the first 10 days but worsened thereafter rapidly.

Thiamine deficiency as predisposition to, and consequence of, increased alcohol consumption.

It was found that the activity of the marker thiamine-dependent enzyme, transketolase (TK), was decreased (down to 61-79% of control) in blood, liver and brain of inbred rats following a 6-month consumption of 15% ethanol as their only source of drinking fluid. After ethanol withdrawal, the enzyme activity was gradually restored, but did not reach the control values until 1 month following cessation of alcohol consumption. Moreover, in rats preferring ethanol, the decrease of TK activity was more pronounced than in water-preferring rats. Another experiment showed that thiamine deficiency induc

Severe metabolic acidosis in the alcoholic: differential diagnosis and management.

1. A chronic alcoholic with severe metabolic acidosis presents a difficult diagnostic problem. The most common cause is alcoholic ketoacidosis, a syndrome with a typical history but often misleading laboratory findings. This paper will focus on this important and probably underdiagnosed syndrome. 2. The disorder occurs in alcoholics who have had a heavy drinking-bout culminating in severe vomiting, with resulting dehydration, starvation, and then a beta-hydroxybutyrate dominated ketoacidosis. 3. Awareness of this syndrome, thorough history-taking, physical examination and routine laboratory an

Corneal melting in a chronic alcoholic contact lens wearer.

PURPOSE: We report a case of bilateral keratitis in a 43-year-old alcoholic patient who wore soft daily wear contact lenses. METHODS: After consecutive negative cultures of corneal and conjunctival scrapings and after observing no improvement following antibiotic therapy, the patient was clinically diagnosed with vitamin A deficiency. RESULTS: After 20 days of treatment with vitamin A, the patient's cornea completely healed. Laboratory results confirmed the clinical diagnosis. CONCLUSIONS: When the ophthalmologist is confronted with corneal melting in a known alcoholic or drug-user, it is i

Naltrexone increases the latency to drink alcohol in social drinkers.

We investigated the effects of naltrexone (NTX) on alcohol drinking, urge to drink alcohol, and alcohol-induced sensations and mood states in social drinkers consuming alcohol ad libitum in a cocktail bar. Sixteen college-age men and women participated in a double-blind, placebo-controlled, within-subjects, cross-over study. Subjects were tested during each of three drug conditions: NTX, 50 mg/ day, po; inactive placebo; and no drug. Each treatment condition lasted 8 to 11 days. Small groups of subjects consumed alcohol ad libitum during three 2-hr evening drinking sessions, separated by appro

Visual pathway hemorrhage associated with alcohol-induced coagulopathy.

Hemorrhage confined to the optic pathways is rare. We describe a patient with diffuse hemorrhage of the intracranial optic nerves, optic chiasm, and optic tracts, attributed to alcohol-induced coagulopathy. The hemorrhage resolved completely as the patient's vision improved, and subsequent neuroimaging studies showed atrophy of the affected structures.

IgA deposition in the skin of patients with alcoholic liver disease.

In patients with alcoholic liver disease, IgA deposits are often found in the liver, kidneys and skin. The present study was undertaken to determine the specificity, sensitivity and characteristics of IgA deposition in the skin of a group of alcoholic patients with or without overt liver disease, and compare these with findings in non-alcoholic cirrhotics. Twenty-six out of 28 patients with alcoholic cirrhosis had IgA deposition in the skin. In contrast, only 6 out of 11 patients who were alcoholic without any clinical liver disease and 3 out of 13 patients with non-alcoholic cirrhosis of the

[Alcohol-induced hallucinosis. Clinical aspects, pathophysiology and therapy].

In recent years, little research has been focused on alcohol hallucinosis. The psychopathology of alcohol hallucinosis (vivid acoustic hallucinations, paranoid symptoms and fear) resembles paranoid schizophrenia, but other organic mental disorders have to be excluded too. Prognosis is usually good, but in 10-20% of cases alcohol hallucinosis tends to become chronic. Possible pathophysiological mechanisms involved in the development of the syndrome are changes in dopaminergic transmission or other neurotransmitter systems and neuronal membranes, elevated levels of betacarbolines and an impaired

Treatment of postoperative lymphoceles with percutaneous drainage and alcohol sclerotherapy.

PURPOSE: To assess the efficacy and safety of percutaneous catheter drainage combined with alcohol sclerosis in the treatment of postoperative lymphoceles. PATIENTS AND METHODS: Thirteen patients with 14 postoperative symptomatic lymphoceles were treated. Drainage catheters were inserted under ultrasound (n = 13) or computed tomographic (n = 1) guidance. Lymphocele sclerosis was performed by instilling 10-100 mL of absolute alcohol into the lymphocele cavity and aspirating the alcohol after 30 minutes. Sclerosis sessions were carried out one to three times per day, usually three times per wee

Alcohol use among injured sets of drivers and passengers.

Crash report and blood alcohol concentration (BAC) data were linked for 109 injured driver/passenger pairs admitted to a Level I trauma center. Among those occupants, 47 drivers (43%) (mean BAC, 147 mg/dl) and 45 passengers (41%) (mean BAC, 127 mg/dl) were BAC+. No occupant was BAC+ in 57 crashes (52%); both were BAC+ in 40 (37%); and only one was BAC+ in 12 (11%). When both occupants were BAC+, the driver had the higher BAC in 68% of cases, and when one was BAC+, it was the driver 58% of the time. In 6 additional alcohol-related crashes with one driver and two passengers, the "wrong" occupant

Men, women & alcohol: a distinctly unequal & dangerous relationship.

This article illustrates how women who abuse alcohol are at greater risk physically, emotionally and socially than their male counterparts. As a result, I recommend that clinical assessment and underwriting of women, who abuse or have the potential to abuse alcohol, change to reflect the increased risks which these women face.
