The aim of this study was to assess echocardiographically the occurrence of cardiac abnormalities in patients with acute reactive arthritis (ReA). Eighteen consecutive adult patients with acute ReA were studied by the use of two-dimensionally guided M-mode and Doppler echocardiography. Aortic or mitral regurgitation were both detected in one of the patients. Mild to moderate left ventricular dilatation was observed in five patients, in four of whom the duration of acute ReA was over 6 months. OBJECTIVE: To evaluate the effect of local application of ice on duration and severity of acute gouty arthritis.
METHODS: Nineteen patients with acute gout were enrolled and randomized into 2 groups. Group A (n = 10) received topical ice therapy, oral prednisone 30 mg PO tapered to 0 over 6 days and colchicine 0.6 mg/day. Eight patients underwent surgery on 15 feet for rheumatoid forefoot problems. Thirteen of the 15 feet that were operated upon had an attempt to preserve the hallux metatarsophalangeal joint while resectional arthroplasty was performed on the lesser MP joints. In this controlled study, hearing and middle ear functions were investigated in 37 patients with rheumatoid arthritis (RA) and 35 controls in order to study the prevalence and the nature of hearing loss in RA. The prevalence of the hearing impairment was significantly higher in the RA group, and the majority was bilateral (P<0.001). The effects of somatostatin on the development of adjuvant arthritis induced by Mycobacterium butyricum were studied. Rheumatoid arthritis (RA) causes local and systemic bone loss due the increased bone resorption. Because the bisphosphonates are potent inhibitors for bone resorption, these drugs will be the major regimen to treat RA-associated osteoporosis. In addition, some reports suggest that bisphosphonates may modulate the pathogenesis of RA itself. Further study of bisphosphonates may help improve the treatment of RA.

We compared power Doppler sonography to laboratory indices of disease activity in patients with knee arthritis to determine the clinical relevance of hypervascularity. Eight healthy volunteers and 22 patients with symptoms and signs of knee arthritis were studied. Presence or absence of hypervascularity, synovial thickening, effusion, and Baker's cysts were recorded. Two cases are presented of unusual cutaneous lesions associated with rheumatoid arthritis in underlying joints. The lesions were evanescent, erythematous and violaceous partly macular and partly indurated plaques, with a livedo-like pattern of erythema at the edge in one case. Histological changes were identical in the two cases. The major features were dilated, dermal lymphatics containing aggregates of inflammatory cells, mainly histiocytes, with adjacent perivascular lymphoid aggregates. An appropriate name for this reaction would appear to be cutaneous histiocytic lymphangitis.

Occipitocervical fixation remains a surgical challenge. A Y-plate in combination with transarticular screw fixation provides a simple and economically favorable solution to this problem. By integrating the transarticular screws into the Y-plate immediate postoperative reliable stabilization is achieved. Clinical results confirm the reliability and the low rate of pseudarthrosis with this surgical technique.
