Pectus excavatum is the most frequent deformity of the thorax. Many surgical procedures have been used for correcting this chest wall deformity. The Nuss procedure is an instant full correction using a rigid stainless steel bar, but some patients experience acute pain. This study follows patients with severe sinus bradycardia (40 beats per minute for 6 seconds or greater) in order to evaluate mortality and the effectiveness of permanent pacemaker insertion. Severe sinus bradycardia was noted on a 24-hour Holter in 95 patients. There were 64 males and 31 females with a mean age of 69 +/- 10 years. All were available for follow-up at 26 +/- 13 months. We report a case of a 49-year-old female who presented with atypical chest pain. An electrocardiogram-gated coronary computed tomography angiography revealed the absence of the left main coronary artery segment with direct origin of the left anterior descending artery and the left circumflex artery from the left coronary sinus having separate ostia. The case highlights a rare anomaly of coronary artery origin with procedural implications.

We report the case of an immunocompromised patient admitted for chest pain secondary to pericardial effusion. Pericardiocentesis liquid analysis revealed an Aspergillus pericarditis. Advanced multimodality imaging later revealed invasive aspergillosis. The patient was treated with pericardectomy and long-term antifungal therapy. Novel teaching points are as follows: Pericardectomy is an efficient treatment for Aspergillus pericarditis. Disease control can be obtained without local relapse with pericardectomy and long-term antifungal therapy.

A 39-year-old woman presented with chest pain and elevated troponin levels. Cardiac catheterization demonstrated no coronary artery disease, with findings consistent with atypical variant takotsubo cardiomyopathy. Further workup showed elevated plasma fractionated metanephrines suggestive of pheochromocytoma. Abdominal imaging showed a right adrenal mass, with surgical excision and histopathological evaluation confirming a pheochromocytoma. Pheochromocytomas are a rare cause of takotsubo-like syndrome with an atypical takotsubo variant rarer still.

A 50-year-old man presented with right chest pain and persistent increased parathyroid hormone after parathyroidectomy for parathyroid carcinoma. 68Ga-DOTATATE PET/CT and 18F-FDG PET/CT were performed consecutively for restaging to determine subsequent treatment strategy. An osteolytic lesion on the right sixth rib showed increased uptake on both 68Ga-DOTATATE and 18F-FDG PET/CT. Subsequent biopsy confirmed it to be a brown tumor.

Pleural effusion is a rare manifestation in synovitis-acne-pustulosis-hyperostosis-osteitis (SAPHO) syndrome, which is characterized by the presence of osteoarticular lesions and dermatological involvement. We herein report a 71-year-old man with pleural effusion resulting from SAPHO syndrome. He was successfully treated using corticosteroids and has experienced no recurrence for one year. We should consider SAPHO syndrome when encountering cases of anterior chest pain and pleural fluid.

We discuss a case of secondary syphilis with pulmonary involvement in a 45-year-old man who tested positive for HIV. He presented with dyspnoea, chest pain and a rash on his limbs and torso. A CT showed multiple bilateral necrotic lung nodules. A diagnosis of pulmonary syphilis was made due to his respiratory symptoms and imaging, his serological, histopathology findings, and the resolution of symptoms on treatment with benzathine penicillin.
