[Surgical treatment of epilepsy: outcome of various surgical procedures in adults and children].

Surgical treatment of drug-resistant epilepsy is being performed in a growing number of adults and children. The objective of this report is to review and evaluate the published literature related to the outcome of epilepsy surgery. Surgical procedures were classified as "curative", which included temporal and extratemporal resections, as well as hemispherotomy and stereotactic radiosurgery, and as "palliative", which mainly included callosotomy and multiple subpial transections. Data obtained from the literature suggest that after temporal lobe surgery, 68 percent of the adult patients, on av

[Ruptured frontopolar artery aneurysm: two cases].

We report two very rare cases of subarachnoid hemorrhage due to a ruptured frontopolar artery aneurysm in the cingulate sulcus. Only two surgically treated cases have been reported. Case 1: A 69-year-old woman suffered headache and was admitted to our hospital the next day. Computed tomography (CT) revealed focal subarachnoid hemorrhage (SAH) in the interhemispheric fissure. Cerebral angiography on admission failed to show an aneurysm, but follow-up study on day 14 revealed a small aneurysm in the cingulate sulcus at the origin of the frontopolar artery arising from the callosomarginal trunk.

Redundancy gain in the acallosal brain.

The authors measured simple reaction time (RT) to visual stimuli, presented either singly to 1 or the other visual field or in bilaterally presented pairs, to 2 women with callosal agenesis. The stimuli were either white against a black background or gray against an equiluminant yellow background. RTs to bilateral pairs were decreased beyond predictions based on a simple race between independent unilateral processes, implying interhemispheric neural summation. This effect was enhanced under equiluminance in the participant M.M. whose anterior commissure was within normal limits, but not in the

[Difficulties in stereotactic biopsies of brain tumors].

MATERIAL: 28 stereotactic biopsies of organic brain processes (brain tumours) were performed in the years 1997-2000 in the Department of Neurosurgery, Medical University of Warsaw. In this series the lesions were located in corpus callosum in 5 patients, in basal nuclei in 9, and deeply in the white matter of cerebral hemispheres in 14. METHOD: The Baklund biopsy kit and Leksell's stereotactic frame were used, target localisation was based on the CT scan. Histological verification was based on hematoxillin--eosin staining, completed with histochemical evaluation if necessary. In 9 patients in…

OBJECTIVES: To study the types of lesion found on magnetic resonance (MR) in cerebral palsy (CP) and their correlation with a the type of CP, gestational age at birth and aetiology. PATIENTS AND METHODS: We made a retrospective review of all patients over one year in age, diagnosed as having spastic CP, who were seen in a paediatric neurology clinic between 1987 and 1998. The MR were evaluated blindly using data from the clinical history. RESULTS: Seventy two patients fulfilled criteria for inclusion in the study. The following results were obtained (all associations were statistically signi

Treatment of seizures in subcortical laminar heterotopia with corpus callosotomy and lamotrigine.

Focal and generalized cortical dysgeneses are sometimes seen on the magnetic resonance images (MRI) of patients with epilepsy. Subcortical laminar heterotopia are bilateral collections of gray matter in the centrum semiovale that resemble a band or "double cortex" on MRI. We studied one male and two female patients with subcortical laminar heterotopia who had moderate to severe developmental delay, early-onset epilepsy, and medically refractory seizures. Atonic, atypical absence, tonic, myoclonic, complex partial, and generalized tonic-clonic seizures were recorded. Interictal and ictal electr

Bilateral effects of unilateral visual cortex lesions in human.

We studied the vision of 12 patients with unilateral lesions of the visual cortex. All had a VI-type scotoma located in the contralateral visual fields, as expected, and visual acuity of 20/30 or better. Our aim was to test the hypothesis that they also had a visual deficit in their ipsilesional or 'good' visual fields. The first experiment tested the subjects' ability to respond to transient signals presented at unpredictable temporal intervals and spatial locations amongst many spatially random and identical distracter elements. The results showed that, compared with controls, the lesion gro

Transcallosal inhibition in cortical and subcortical cerebral vascular lesions.

The excitability of the motor cortex after transcranial magnetic stimulation was investigated in 10 patients with purely subcortical, and in 22 patients with cortical-subcortical cerebrovascular lesions. In the first investigation we applied magnetic double stimuli over both motor cortices with different inter-stimulus intervals. The first (conditioning) stimulus was applied to the affected hemisphere and the second stimulus (test stimulus) to the unaffected side. The responses of the first dorsal interosseal (FDI) muscle, contralateral to the test stimulus, were recorded after applying the te

Protelomeric sequences are deleted in cases of short arm inverted duplication of chromosome 8.

We present a patient with a de novo inverted duplication of the short arm of chromosome 8. Molecular analysis confirmed the cytogenetic suspicion of a simultaneous deletion of the tip of the short arm and indicated the maternal origin of the abnormality. This deletion made no detectable contribution to the phenotype of the patient which was comparable to that of previous cases of 8p duplication. Similar investigations of inverted duplications involving other chromosomes may reveal unexpected deletions with significant phenotypic consequences.

Neuropsychological demonstration of splenial interhemispheric disconnection in a case of "optic anomia".

A patient is described whose neuropsychological deficit corresponded to the notion of "optic aphasia" or, better, optic anomia. The functional deficit was undirectional, i.e. there was a severe impairment in the naming of visually presented stimuli without impairment in the comprehension of the corresponding names. The splenial disconnection was demonstrated by intact cross modal matching within the right hemisphere as well as preserved left-handed tactile naming, which is processed via mid-callosal pathways. The psychological interpretation is discussed in terms of visual and tactile semantic

Delayed transcallosal conduction to the lesioned sensorimotor cortex in multiple Sclerosis: A combined TMS 7 T-MRI study.

BACKGROUND: In multiple sclerosis (MS), demyelination and degeneration of transcallosal pathways impair interhemispheric communication. While white matter damage is well documented, the impact of cortical lesions on transcallosal conduction remains unclear. OBJECTIVE: To determine whether cortical lesions in the sensorimotor hand area (SM1‑HAND) contribute to impaired transcallosal motor interaction using ultra‑high‑field MRI and transcranial magnetic stimulation (TMS). METHODS: Twenty healthy controls (HCs) and 38 MS patients underwent 7 T structural and diffusion‑weighted MRI. Structural s
