[Control of sexually transmitted diseases in Senegal].

STD are an important public health problem in numerous developing countries. Although satisfactory diagnostic and therapeutic means are available, the incidence of STD is increasing. Among the causes for this are sociological factors, like the weakening of customs and the family unit, and the development of tourism, which stimulates prostitution. There is a marked ignorance of the population in respect to STD. The Senegalese Government, WHO, and the Geneva-based Sandoz Institute launched in 1976 a joint programme of service, training and sesearch. The research aims at developing a preventive a…

[Schizophrenia and culture: reality and perspectives based on the Tunisian experience].

UNLABELLED: Despite notable advances in the field, schizophrenia is still considered a major public health problem on international level. The combination of frequency, chronicity and severity make it, for some authors, the "cancer of mental disorders". However, according to the many cross-cultural studies, in particular, the two mammoth research projects of the WHO (IPSS and DOSMED, it is now irrefutable that the course and outcome of schizophrenia are better in patients from developing countries. The reasons for better outcome and more favorable course of schizophrenia in developing countrie…

Vaccine antigen-based genotyping of Bordetella pertussis by direct Sanger sequencing of clinical samples in Peru from 2018 to 2019.

UNLABELLED: Despite widespread vaccination, pertussis (caused by Bordetella pertussis) persists in many countries, frequently causing outbreaks and severe cases in infants. The resurgence of pertussis may be due to genetic changes in the vaccine antigens of circulating B. pertussis strains. However, current typing methods, which depend on bacterial cultures, hinder our understanding of B. pertussis genotypes, especially in developing countries. This study aimed to analyze vaccine antigen-based genotypic variants (ptxP, ptxA, fim3, and prn) of B. pertussis in Peru from 2018 to 2019 via direct S…

Pediatric AIDS and perinatal HIV infection in Zaire: epidemiologic and pathologic findings.

HIV infection in women and children is a special problem in Zaire and in other countries where heterosexual transmission is predominant. Nearly half of the cases of HIV infection are in women 15 to 30 years old and as many as seven infected infants may be born each year. Whether or not infected at birth, these children have mothers, and often fathers, who are infected and likely to die while they are still very young. Such orphaned children, as well as those whose families cannot provide adequate food and health care, add to the problematic economies of developing countries. The problems of ch…

Zinc in child health and disease.

Zinc deficiency is common in children from developing countries due to lack of intake of animal foods, high dietary phytate content, inadequate food intake and increased fecal losses during diarrhea. Zinc has a fundamental role in cellular metabolism, with profound effects on the immune system and the intestinal mucosa. Zinc supplementation has shown significant benefits in prevention and treatment of diarrhea and pneumonia. Routine zinc supplementation given to low birth weight babies for a year has resulted in substantial reduction in mortality. Zinc deficiency may have adverse effects on ph…

Determinants of compliance with iron supplementation: supplies, side effects, or psychology?

Iron deficiency anemia affects over 2 billion people. Particularly at risk are pregnant women and young children. Although distribution of iron supplements is practised in many antenatal care programs in developing countries, it has often been alleged that pregnant women do not take them. Poor compliance arises not only because of patient behavior but also from factors out of the patient's control. This paper presents the results of a review of the literature on medical compliance to determine whether iron supplementation is different from other medications, to assess the known levels of compl…

Triumph of Pneumococcal Conjugate Vaccines: Overcoming a Common Foe.

Pneumococcal conjugate vaccine (PCV) has reduced the burden of pneumococcal disease by the near elimination of vaccine serotypes from countries giving a booster dose at >9 months of life. Herd protection, induced by interruption of pneumococcal vaccine type transmission has protected children too young to be immunized, children and adults with underlying risk conditions for invasive pneumococcal disease, and the elderly. PCV has rolled out in most poor countries, but millions of children remain un-immunized especially in middle income countries because of cost constraints. These are being met

Hepatitis E, the neglected one.

Hepatitis E virus (HEV) infection is a worldwide disease. It is the first cause of acute viral hepatitis in the world with an estimated 20 million cases every year and 56 000 deaths. In developing countries, hepatitis E is a waterborne infection. In these countries, HEV genotypes 1 and 2 cause large outbreaks and affect young subjects with a significant mortality rate in pregnant women and patients with cirrhosis. In the developed countries, HEV genotypes 3 and 4 are responsible for autochthonous, sporadic hepatitis and transmission is zoonotic. HEV can cause neurological disorders and in immu…

Esophageal atresia and tracheoesophageal fistula: success and failure rates in the United Arab Emirates.

The management of esophageal atresia (EA) and tracheoesophageal fistula (TEF) has improved markedly over the years, with a current overall survival of 80%-90%. This however, is not the case in developing countries, where mortality continues to be high. The results of treatment of 41 consecutive cases EA and/or TEF have been analyzed to determine factors pertinent to the outcome. The distribution of anomalies and a postoperative survival of 86.8% were comparable to those from developed countries. Aspiration pneumonia was unusually frequent in our patients (78%), but did not adversely influence …

The epidemiology and control of hepatitis A.

Hepatitis A virus (HAV) infection is endemic in developing countries and remains a public health problem elsewhere. A total of 7430 cases was notified in England and Wales in 1991. Children and young adults were most frequently affected. Risk factors include travel to countries of high endemicity, contact with an infected person and shellfish consumption. Person-to-person spread is common within families, closely knit communities and schools (especially nurseries). The provision of safe water and sanitary disposal of faeces are prerequisites for control of HAV throughout the world. In develope…

Prevalence of Hepatitis B Among Migrants Living in High-Income Countries With Low/Intermediate HBV Prevalence-A Systematic Review and Meta-Analysis.

The World Health Organisation targeted the Hepatitis B virus (HBV) for elimination as a public health threat by 2030. To achieve this goal, equitable access to HBV testing and treatment services is critical. Migrants usually experience multiple barriers to accessing HBV services. Understanding HBV prevalence among this population can support planning for HBV elimination. This systematic review aimed to estimate HBV prevalence among migrants in high-income countries with low/intermediate HBV prevalence. PubMed, Scopus, PsycINFO and Cochrane Library were searched for peer-reviewed studies publis

Trends of stillbirth, early neonatal and perinatal mortality rates and sociodemographic differentials of perinatal mortality rates in 17 low- and middle-income countries: A modeling study.

BACKGROUND: Stillbirths and early neonatal deaths account for a substantial proportion of global child mortality. Reliable measurement of these outcomes remains challenging in settings where civil registration systems are incomplete and household surveys are the primary data source. However, survey-based estimates are often affected by underreporting and misclassification between stillbirths and early neonatal deaths. We aimed to apply a statistical adjustment approach to improve estimates of stillbirth, early neonatal, and perinatal mortality trends and to assess socioeconomic inequalities in

Predictors of Liver Steatosis Progression Among People Living With HIV in Low- and Middle-Income Countries: A Multicentre Longitudinal Study.

INTRODUCTION: Liver steatosis is increasingly recognized as a major comorbidity in people living with HIV (PLWH), with growing evidence suggesting an increased risk of progression to advanced liver disease. We assessed predictors of liver steatosis progression over 36 months in PLWH from low- and middle-income countries. METHODS: Between 2020 and 2023, we enrolled PLWH aged ≥40 years who had been on antiretroviral therapy (ART) for ≥6 months at nine HIV clinics in the IeDEA Sentinel Research Network (SRN) across the Asia-Pacific, the Americas and African IeDEA regions, and followed them prosp

Tracking funding disparities in global health aid with machine learning.

Reducing the global burden of disease is crucial for improving health outcomes. However, misalignment between health aid and country-level disease burden leaves vulnerable populations without the necessary support for major health challenges, particularly in the least developed countries. Here, we develop a machine learning pipeline using large language models to track flows in official development assistance (ODA) earmarked for health and identify aid-burden misalignment. We classified 3.7 million development aid projects from 2000 to 2022 (USD  ~ 332 billion) into 17 major categories of comm

Effectiveness of culturally appropriate antenatal education packages to improve birth preparedness and complication readiness in low- and middle-income countries: a systematic review.

BACKGROUND: Birth preparedness and complication readiness (BPCR) remain essential components of maternal survival strategies in low- and middle-income countries (LMICs). Suboptimal antenatal education contributes significantly to delayed recognition of obstetric danger signs and late care-seeking. Delays in recognising obstetric danger signs and seeking care contribute to high maternal mortality in LMICs. Culturally tailored antenatal education packages have been increasingly used to improve BPCR; however, evidence across LMICs has not been systematically synthesised. OBJECTIVE: To synthesise

Sustainability of infectious disease elimination: a scoping review on integrating treatment and surveillance within health systems.

BACKGROUND: Global infectious disease elimination programmes have historically relied on vertical, donor-driven approaches. However, amid reduced global funding and increased emphasis on country ownership and sustainability, interest in infectious disease-specific activities integrated into routine health systems is growing. Yet evidence on how integration has been implemented remains fragmented. This scoping review takes a multi-disease approach to assess the extent and nature of evidence on the integration of infectious disease elimination and eradication programmes into routine health syste
