OBJECTIVE: To evaluate 10-year trends in pharmacologic management of young children with gastroesophageal reflux disease (GERD) or pediatric feeding disorder (PFD), a population at risk for over prescribing.
STUDY DESIGN: Single-center, retrospective cohort study of children ≤2 years diagnosed with GERD or PFD between January 2014 and December 2023. Prescriptions were searched for proton pump inhibitors (PPI), H2-receptor antagonists (H2RA), cyproheptadine, erythromycin, metoclopramide, or prucalopride, and procedures were searched for intrapyloric botulinum injections. The change over time i

Ménière's disease (MD) and vestibular migraine (VM) are two common vestibular disorders with significant clinical overlap in their symptomatic presentations, including vertigo, hearing loss, tinnitus, and aural fullness. Although distinct diagnostic criteria exist for each, this symptomatic similarity often makes early-stage differentiation challenging. While recent studies have found potential biomarkers for MD and VM, their diagnostic utility remains limited by small sample sizes and lack of standardized validation protocols. This necessitates continued reliance on a synthesis of established

Delirium is an acute disturbance of attention, arousal, and cognition that fluctuates in severity and is a common yet under recognized complication in hospitalized patients with Parkinson's disease. In Parkinson's disease, symptoms such as rigidity, bradykinesia, hallucinations, and slowed thinking overlap with the clinical features of delirium, which can obscure its onset in hospitalized patients. Variability in diagnostic criteria further complicates accurate recognition and prevalence estimates. This mini review summarizes current evidence on the prevalence, risk factors, and consequences o

BACKGROUND: Individuals with spinal cord injury or disease (SCI/D) experience disproportionately high rates of recurrent urinary tract infections, which are often complicated by atypical symptoms and delayed diagnoses. Patient-centered tools, like the Urinary Symptom Questionnaires for Neurogenic Bladder (USQNB), have been developed to support symptom assessment yet remain underused. Generative artificial intelligence tools such as ChatGPT may offer a more usable approach to improving symptom management by providing real-time, tailored health information directly to patients.
OBJECTIVE: This

The 21st Century Cures Act opened Medicare Advantage (MA) enrollment to people with end-stage renal disease (ESRD). Previously, most Medicare beneficiaries with ESRD were only permitted to enroll in traditional Medicare. The purpose of our study was to determine how MA plans responded to this policy change. We conducted 48 interviews with representatives from MA plans, dialysis provider organizations, and chronic kidney disease (CKD) care management companies. One major theme that emerged from our interviews was MA plans partnered with CKD care management companies to manage the care of ESRD p

PURPOSE OF REVIEW: To examine recent developments in shared decision making (SDM) interventions for advanced chronic kidney disease (CKD) and end-stage kidney disease (ESKD). Given the complexity of treatment decisions and low patient engagement despite available options, SDM is a critical approach to improve treatment initiation and engagement.
RECENT FINDINGS: Three recent SDM interventions were identified: DART (Decision-Aid for Renal Therapy), a decision support patient-centered video tool for older patients with stages 4-5 CKD that significantly reduced decisional conflict and improved t

A 47-year-old married woman presented with abdominal discomfort, distension, and nausea following a two-day episode of constipation accompanied by reduced urine output. Upon her admission, a physical assessment indicated tachycardia and abdominal sensitivity, while imaging examinations corroborated gastrointestinal perforation along with signs of acute diffuse peritonitis. The patient had a prior history of Crohn's disease and had been off medication for over a year. The initial surgical intervention entailed bowel resection and adhesiolysis; however, postoperative complications arose, prompti

Atypical Scheuermann's disease is a rare spinal disorder characterized by progressive kyphotic deformity and, in rare instances, spinal cord compression. Unlike classic Scheuermann's disease, which affects the thoracic spine, atypical variants may involve the thoracolumbar or lumbar regions, leading to neurological compromise. This report presents a rare case of atypical Scheuermann's disease with spinal cord compression, its surgical management, and a review of relevant literature. A 24-year-old male presented with progressive lower limb weakness, gait instability, and back pain for 6 months.

Central serous chorioretinopathy (CSCR) is characterized by serous detachment of the neurosensory retina and is considered part of the pachychoroid spectrum. Vogt-Koyanagi-Harada (VKH) disease, in contrast, is characterized by bilateral granulomatous panuveitis, exudative retinal detachments, and choroidal thickening. As both are primarily diseases of the choroid, the rare clinical overlap between atypical CSCR and VKH disease can complicate diagnosis. We report a case of atypical CSCR mimicking VKH disease, highlighting the diagnostic and treatment challenges. A 50-year-old woman presented wi
