Exploring the mechanism of transcranial direct current stimulation combined with aerobic exercise in improving working memory of post-stroke patients with cognitive dysfunction: An event-related potentials study.

BACKGROUND: Post-stroke cognitive impairment (PSCI) often involves working memory deficits. This study explored whether combining transcranial direct current stimulation (tDCS) and aerobic exercise (AE) improves working memory in PSCI patients and investigated the underlying neurophysiological mechanisms using event-related potentials (ERPs). METHODS: A total of 60 PSCI patients were randomly assigned to three groups: tDCS (2.0 mA anodal stimulation over left dorsolateral prefrontal cortex, 30 min/day, 5 days/week for 4 weeks), AE (moderate-intensity, 60-70 % maximum heart rate, 30 min/day wi

Outcomes of targeted axillary radiation therapy with omission of axillary dissection in early breast cancer patients with one or two positive sentinel lymph nodes and extracapsular extension.

PURPOSE: Axillary dissection has been shown to be equivalent to axillary radiotherapy in the AMAROS trial; however, extracapsular invasion of sentinel lymph nodes was not considered among the evaluated variables. The clinical significance of extracapsular extension (ECE) in one or two positive sentinel lymph nodes remains under investigation. This study aims to evaluate the impact of targeted axillary radiation therapy while omitting completion axillary lymph node dissection (ALND) in the presence of extracapsular extension. METHODS: A retrospective study was conducted between 2016 and 2023 i

Changes in knowledge and attitudes towards mental disorders in a Chinese sample population-a web-based approach.

BACKGROUND: On October 10, 2022, The Lancet Psychiatry launched a global campaign aimed at ending the stigma surrounding mental illness, reinforcing the need for continued efforts to combat this pervasive issue. We previously surveyed knowledge and attitudes towards mental illness in a Chinese sample population. Seven years later, we aimed to conduct a study to reassess public perceptions of mental health and the level of mental illness literacy within the community today. METHODS: A cross-sectional survey was designed and administered via a web-based chat application (WeChat) from March to S

Sedation-related adverse events in gastrointestinal endoscopy (CH-ESC study): study protocol for a prospective, observational cohort study in a Chinese tertiary hospital.

BACKGROUND: Sedation is commonly employed during gastrointestinal endoscopy (GIE), yet sedation-related adverse events (SRAEs)-particularly cardiopulmonary complications-remain a critical safety concern. While large cohort studies from Western countries have identified key risk factors such as advanced age, obesity, and high ASA classification, data on SRAEs within the Chinese population are scarce. This study aims to assess the prevalence, characteristics, and risk factors associated with SRAEs in a Chinese tertiary hospital setting. METHODS/DESIGN: This prospective, observational cohort stu

Drink lemonade! Implementation of drinking 2 h before elective orthopedic surgery to reduce prolonged fasting.

BACKGROUND: Prolonged fasting before surgery remains common despite guidelines recommending liquid fasting for 2 h and solid fasting for 6 h preoperatively. Consuming carbohydrate-rich drinks until 2 h before surgery improves patient well-being and reduces postoperative complications and insulin resistance. Given the persistence of implementation barriers, including logistical challenges and insufficient patient education, this study aimed to implement drinking up to 2 h before elective orthopedic surgery. METHODS: The Grol and Wensing implementation approached was followed in a before-after

Design of the OverCool study: Lung-conservative liquid ventilation for the induction of Ultra-Rapid Cooling after Cardiac Arrest (OverCool).

BACKGROUND: The therapeutic window within which induced hypothermia might be effective after cardiac arrest is still unknown. In animal cardiac arrest models, early induction and faster cooling are independently associated favorable outcome. However, induction of Ultra-Rapid Therapeutic Hypothermia (achieving core body temperature of 33.0 ± 0.5 °C within 60 min after the start of the procedure) still need to be evaluated in the human setting. Total liquid ventilation with temperature-controlled breathable liquids provided such rapid cooling (i.e. >15 °C/h cooling rate) in both small and large

Pilot study to assess the safety and efficacy of human acellular dermal matrix for Chiari surgery.

PURPOSE: Although there may be benefits to adult patients who have had Chiari surgery when duroplasty is indicated, there are also more risks involved. The complications derived from a non-hermetic dural closure in the posterior fossa can be significant, mainly cerebrospinal fluid (CSF) leakage, meningitis and pseudomeningocele. We explored the option of utilizing a different duroplasty that we typically utilized in order to reduce these risks. The aim of this study is to assess the safety and efficacy of two duraplasties used for Chiari malformation (ChM) surgery: Tutopatch®, and a human acel

Comparative analysis of continuous pericapsular nerve group block and supra-inguinal fascia iliaca compartment block for postoperative analgesia in total hip arthroplasty: a randomized controlled trial.

BACKGROUND: Effective postoperative pain management is essential for recovery after total hip arthroplasty. While opioids are commonly used, their adverse effects necessitate alternative strategies. Peripheral nerve blocks are increasingly utilized, including the pericapsular nerve group block and the supra-inguinal fascia iliaca compartment block. However, comparative data on their efficacy remain limited. This randomized controlled trial evaluated the analgesic effectiveness of these two techniques. The primary outcome was postoperative pain scores at rest and with movement. Secondary outcom

Treatment with PCSK9 inhibitors influences microRNAs expression and changes of arterial wall properties: a randomized controlled trial.

BACKGROUND: MicroRNAs (miRNAs) are involved in the synthesis of proprotein convertase subtilisin-kexin type 9 (PCSK9), one of the regulators of low-density lipoprotein cholesterol (LDL-C) metabolism, and are directly involved in the atherosclerotic process. The aim of this study was to verify whether treatment with PCSK9 inhibitors (PCSK9i) and changes in the expression of miRNAs involved in PCSK9 metabolism are associated with arterial wall properties in stable post-myocardial infarction (MI) patients with insufficiently regulated LDL-C levels and significantly increased Lp(a) levels. Difference in Knee Joint Structure and Lower Extremity Functioning Among People with Varying Severity of Knee Osteoarthritis: A Cross-Sectional Study.

Osteoarthritis (OA) is a highly prevalent musculoskeletal condition. It is reported that knee OA progressively affects lower-extremity functioning. Evidence is lacking on when there is a substantial decline in function and whether this trend differs between males and females and disease severity. This study aimed to examine influence of gender and increasing grade of knee OA on lower-extremity function. Following Institutional Ethical Committee approval and signed-informed consent, 115 participants (57-males, 58-females), aged 40-65 years, with diagnosed knee OA (grade 1-4) were recruited. Low

Impact of nano-selenium supplementation add-on sertraline on depressive symptoms and oxidative stress in patients with major depressive disorder: a triple-blind randomized controlled trial.

Major depressive disorder (MDD) is a prevalent psychiatric condition associated with increased oxidative stress, which may contribute to its pathophysiology. Elevated malondialdehyde (MDA) levels and reduced total antioxidant capacity (TAC) and glutathione peroxidase (GPX) activity have been observed in individuals with MDD. Nano-selenium, a novel formulation with enhanced bioavailability and antioxidant potency compared to conventional selenium, may help modulate these oxidative stress biomarkers. In this randomized, triple-blind, placebo-controlled trial, 50 adults newly diagnosed with MDD r

Effectiveness and safety of the four-step versus six-step milk ladder in children with IgE-mediated cow's milk protein allergy: protocol for an open-label randomised controlled trial.

INTRODUCTION: Introducing baked milk into the diet of children with cow's milk protein allergy (CMPA) has been shown to potentially accelerate the development of tolerance to non-heated milk. However, there is no standardised milk ladder (ML) protocol, and different scientific societies across countries recommend varying versions. This study aims to assess the effectiveness and safety of the four-step ML (4-ML) compared with the six-step ML (6-ML) in children with IgE-mediated CMPA. METHODS AND ANALYSIS: We will perform an open-label randomised trial with two parallel arms in two departments
