BACKGROUND: Cognitive-motor interference, as measured by dual-task walking (performing a mental task while walking), affects many clinical populations. Ankle-foot orthoses (AFOs) are lower-leg splints prescribed to provide stability to the foot and ankle, as well as prevent foot drop, a gait deficit common after stroke. AFO use has been shown to improve gait parameters such as speed and step time, which are often negatively impacted by dual-task walking.
OBJECTIVES: Our objective was to establish whether AFOs could protect against cognitive-motor interference, as measured by dual-task walking…

BACKGROUND: During gait, the human ankle both bends with ease and provides push-off forces that facilitate forward motion. The ankle is crucial for support, stabilization, and adapting to different slopes and terrains. Individuals with lower limb amputation require an ankle-foot prosthesis for basic mobility.
METHODS: Inspired by the role of the ankle-foot in an able-bodied gait, the 3D printed Compliant and Articulating Prosthetic Ankle (CAPA) foot was designed. It consists of four articulating components connected by torsion springs and produces forces that are dependent on the ankle angle.…

BACKGROUND: It is well documented in the literature that fungal infections are common in diabetic foot ulcers (DFUs). This has led to an overuse of antifungal agents, namely fluconazole, with a consequent risk of emergence of resistance to this drug. Previous studies have shown a 3.9% prevalence of fluconazole resistance in DFU, but limited data exist regarding the change in resistance pattern over the last decade.
OBJECTIVES: Our aim was to study the prevalence of resistance to fluconazole in patients with DFU and culture-proven fungal infections.
MATERIALS AND METHODS: We retrospectively s…

OBJECTIVE: Morphological changes of the abductor hallucis muscle (AbH) in flexible flatfoot (FF) individuals influence regulations of the medial longitudinal arch (MLA). Prolonged and repeated stretching of AbH in flexible flatfoot may cause changes in muscle reflex properties and further influence postural performance. However, AbH muscle reflex under different postural conditions have never been examined. The purpose of this study was to investigate differences in AbH H-reflex and postural performance between individuals with normal foot (NF) alignment and FF under prone, double-leg stance (…

BACKGROUND: Foot ulceration is the most frequently recognized lower extremity complication in diabetic patients. Predicting wound healing is an essential step in the management of diabetic foot ulcers (DFUs), as it is estimated that early detection and appropriate treatments may prevent up to 85% of amputations. Toe systolic blood pressure (TBP) is a quick and portable bedside assessment and is less affected by medial sclerosis of arteries present in the diabetic population compared to other measurements like ankle-brachial index. This systematic review seeks to evaluate the sensitivity and sp…

BACKGROUND: Recently, the study of walking gait has received significant attention due to the importance of identifying disorders relating to gait patterns. Characterisation and classification of different common gait disorders such as foot drop in an effective and accurate manner can lead to improved diagnosis, prognosis assessment, and treatment. However, currently visual inspection is the main clinical method to evaluate gait disorders, which is reliant on the subjectivity of the observer, leading to inaccuracies.
RESEARCH QUESTION: This study examines if it is feasible to use commercial o…

Hand, foot, and mouth disease (HFMD) is a common viral childhood illness, that has been a severe public health concern worldwide, particularly in the Asia-Pacific region. According to epidemiological data of HFMD during the past decade, the most prevalent causal viruses were enterovirus (EV)-A71, coxsackievirus (CV)-A16, CV-A6, and CV-A10. The public health burden of CV-A10-related diseases has been underestimated as their incidence was lower than that of EV-A71 and CV-A16 in most HFMD outbreaks. However, cases of CV-A10 infection are more severe, and its genome is more variable, which has ale…

Diabetes Mellitus is a serious systemic illness that has an epidemic-like increasing prevalence in the United States, as well as the rest of the world. With the increasing number of people with diabetes comes the higher incidence of diabetes-related complications. One of these known complications, diabetic foot ulcers (DFU), has an estimated lifetime incidence of 15% in diabetics. Having a DFU increases the risk of infection, amputation, and even death, which is why prompt treatment and surveillance of such ulcers is imperative. Multiple organizations and journals have recently published best …

Current practice for diabetic foot ulcers (DFU) screening involves detection and localization by podiatrists. Existing automated solutions either focus on segmentation or classification. In this work, we design deep learning methods for real-time DFU localization. To produce a robust deep learning model, we collected an extensive database of 1775 images of DFU. Two medical experts produced the ground truths of this data set by outlining the region of interest of DFU with an annotator software. Using five-fold cross-validation, overall, faster R-CNN with InceptionV2 model using two-tier transfe…

Diabetic patients with foot infections were evaluated over a 5-year period from April 2005 to March 2010. Cultures were obtained from 92 patients after surgical debridement. All of the patients were classified as "severe" diabetic foot infections (DFIs) and PEDIS grade 4. Wound specimens were collected by bone biopsy and sent to San Francisco VA Medical Center Microbiology Department for aerobic and anaerobic cultures. Among the 92 cases, the study resulted in a total of 410 pathogens, which 203 pathogens were from bone cultures and 207 pathogens were from soft tissue cultures. 74% of cases we…

AIM: To determine the most appropriate duration of antibiotic therapy for diabetic foot infections (DFIs).
METHODS: Using a clinical pathway for adult patients with DFIs (retrospective cohort analysis), we created a cluster-controlled Cox regression model to assess factors related to remission of infection, emphasizing antibiotic-related variables. We excluded total amputations as a result of DFI and DFI episodes with a follow-up time of <2 months.
RESULTS: Among 1018 DFI episodes in 482 patients, we identified 392 episodes of osteomyelitis, 626 soft tissue infections, 246 large abscesses, 3…
