Which is the optimal surgical strategy for the terrible triad of the elbow?

OBJECTIVE: To compare and rank the clinical outcomes of commonly used surgical strategies for the terrible triad of the elbow (TTE) using a network meta-analysis (NMA). METHODS: Major databases were searched from inception to September 2025 for comparative studies evaluating different surgical approaches for TTE. Primary outcomes were the Mayo Elbow Performance Score (MEPS) excellent-good rate and complication incidence. Secondary outcomes included elbow flexion-extension range of motion (ROM), forearm rotation ROM, and fracture healing time. A frequentist NMA framework was applied to estimat

Factors in redisplacement of paediatric distal radius fractures.

Redisplacement after manipulation can occur in distal radius fractures. We performed a retrospective study of 73 patients undergoing manipulation with or without K-wire fixation. From the preoperative, intraoperative and postoperative films we measured anteroposterior and lateral angulation and translation. Reangulation of each fracture was calculated. Factors demonstrating significant correlation with reangulation were age, initial radial translation, ulna fracture, distance from fracture to physis and inadequate correction of translation or angulation. This emphasizes the importance of adequ

Unrecognized traumatic atlanto-occipital dislocation.

This report describes the case of a 67-year-old male victim of an auto-pedestrian accident who was alert and neurologically intact on initial presentation. The patient denied head or neck pain and was without evidence of craniocervical injury. A lateral c-spine radiograph did not show any abnormal findings. The patient deteriorated during his trauma evaluation and was refractory to resuscitation. At autopsy, he was found to have an atlanto-occipital dislocation (AOD). The case is discussed and the anatomic, clinical, and radiographic features of atlanto-occipital dislocation are reviewed.

Ankle fracture treated with the AO principle--an experience with 116 cases.

One hundred and sixteen cases of ankle fractures were classified by the AO system and treated by open reduction and internal fixation in accordance with the AO principles. The stabilization of the lateral malleolus was emphasized. Ninety-six cases were available for functional assessment with an average follow-up of 39 months. The results were good or excellent in 90 per cent of type A fractures and in 70-80 per cent of type B and C fractures. There was no death or non-union. Local complications included wound infection (8.6 per cent) and skin necrosis (3 per cent). This study illustrates the

Arthroscopically assisted temporary trans-articular headless screw fixation for perilunate injuries: A retrospective study.

BACKGROUND: Perilunate injuries are rare high-energy wrist injuries whose open treatment carries a risk of capsular stiffness and carpal devascularisation. Arthroscopically assisted reduction with temporary fixation aims to limit this morbidity, and temporary trans-articular headless compression screws may provide more rigid scapholunate and lunotriquetral control than Kirschner wires. We asked what the mid-term clinical, radiographic and patient-reported outcomes of this technique are, using the contralateral wrist as a paired control, and whether they differ by injury pattern. METHODS: Retr

Total hip arthroplasty in paediatric patients with cerebral palsy: mid- to long-term outcomes of a retrospective cohort.

INTRODUCTION: Total hip arthroplasty (THA) is commonly performed in adults with excellent outcomes. Its use in the paediatric population remains rare due to concerns regarding potential damage to the growth plate, pelvic biomechanical changes during growth, leg-length discrepancies, and implant longevity. Although modern prostheses have an estimated lifespan of approximately 30 years, durability remains a major concern in young patients. However, in selected cases, particularly in spastic patients suffering from chronic painful hip dislocation, medical options may be exhausted, and conservativ

The use of dual mobility cups in revision total hip arthroplasty for failed large head metal-on-metal bearings.

PURPOSE: Revision of failed large head metal-on-metal (MoM) total hip arthroplasty (THA) is a challenging procedure particularly to reconstruct acetabular bone defect due to osteolysis and to achieve hip stability due to soft tissue damages, both potentially caused by adverse reaction to metal debris (ARMD). This study aimed to evaluate the outcome of dual mobility cup (DMC) constructs in revision THA for failed large head MoM bearings with a special attention to the occurrence of dislocation or re-revision. METHODS: Between 2015 and 2019, 57 patients (64 THAs, 41 men, mean age = 65 ± 10 year

Clinical Outcomes of Total Hip Arthroplasty With the Anterolateral Modified Watson-Jones Approach for Displaced Femoral Neck Fractures.

OBJECTIVE: In recent years, many studies have reported good results with total hip arthroplasty (THA) for displaced femoral neck fractures (FNFs). However, no study has reported the clinical outcomes of the anterolateral modified Watson-Jones THA (MWJ-THA) for displaced FNFs. This study aimed to investigate the clinical results of THA for displaced FNFs at our hospital and to discuss the advantages of MWJ-THA over THA with other approaches for displaced FNFs. METHODS: Forty-three patients who underwent MWJ-THA for displaced FNFs were included in this study. Patient characteristics, preinjury

Minimally Invasive Fixation of a Distal Fibula Fracture With an Intramedullary Nail.

BACKGROUND: Advantages of using an intramedullary (IM) nail over a lateral plate for fixation of distal fibula fractures include the ability to allow for earlier weightbearing and return to sport, smaller incisions, and decreased hardware prominence. When comparing a fibular nail with an IM screw, the nail allows for syndesmosis fixation through the device and imparts rotational stability. Long-term studies support the viability and clinical outcomes of using an IM nail. In this technique video, a case is presented to illustrate the use of an IM nail for fixation of a distal fibula fracture.

Idiopathic Unilateral Intracranial Condylar Dislocation: A Case Report.

Intracranial condylar dislocation to the middle fossa is rare, as it is not reported often. Known cases have an etiology, identified as erosion of the glenoid cavity from joint prostheses and/or traumatic events. As such, this case aims to offer a predisposing reason for the idiopathic condylar dislocation to the middle cranial fossa with nonfunctional limitations.
