Herkinorin Exerted Neuroprotective Effects After Spinal Cord Injury by Suppression of NF-κB Pathway Activation.

The disruption of axonal continuity and neuronal loss at the injury site leads to structural disconnections as well as functional consequences in spinal cord injury. Herkinorin, a structurally unique non-nitrogenous opioid receptor agonist, demonstrates significant neuroprotective efficacy in models of ischemic brain injury and epilepsy. In this report, we aimed to explore the role and underlying mechanism of Herkinorin in axon regeneration and functional recovery in SCI. Oxygen-glucose deprivation and restoration (OGD/R) condition was used in cell line rat pheochromocytoma cells (PC12) and pr

Early diagnosis of Alzheimer's disease through handwriting analysis and deep learning: A review.

Alzheimer's disease (AD) is one of the most prevalent neurodegenerative disorders worldwide, requiring early identification for timely intervention and to slow disease progression. However, existing diagnostic approaches, while effective at later stages, remain limited in detecting early-stage AD. Handwriting analysis has recently emerged as a non-invasive, cost-effective, and ecologically valid digital behavioral biomarker that reflects neurocognitive impairment. This review examines the role of handwriting as a neurocognitive marker for AD, focusing on integrating deep learning methodologies

Common gait modulation by presence of hip assistance, individual motor strategies by assistance timing.

BACKGROUND: Hip-assist soft exosuits are tuned by shifting force-profile timing, yet it remains unclear whether responses to different force profiles generalize across individuals or are largely person-specific. METHODS: In 22 healthy adults walking at 1.1 m/s (two n = 11 cohorts), we compared a suited baseline (Assist-off) with eight timing-varied hip assist force profiles (A-H) parameterized by onset-peak-offset over the gait cycle, and evaluated prolonged exposure to a single profile. Lower-limb kinematics and electromyography (EMG) were measured. Cohort-level differences were tested using

Motor adaptation of gait to bilaterally added lower-limb mass in young adults.

BACKGROUND: Walking with bilaterally added lower-limb mass is a promising approach to improve gait function, but it alters the mechanics of the lower limbs. Consequently, the central nervous system needs to adapt and presumably form an internal model of the altered condition. However, this motor adaptation and de-adaptation process may jeopardize safe walking. This study aimed to analyze motor adaptation and de-adaptation in response to bilaterally added lower-limb mass. METHODS: Sixteen healthy female and male participants walked on a treadmill with and without weight cuffs attached to their

Age- and sex-related distribution of 30-second chair stand test performance across adulthood: Findings from the INSPIRE-T cohort.

BACKGROUND: Physical performance is an important indicator of health status across adulthood. In relatively high-functioning populations, bounded instruments such as the Short Physical Performance Battery (SPPB) may show ceiling effects, limiting discrimination at the upper end of the scale. The 30-second Chair Stand Test (30s-CST) provides a repetition-based measure of lower-extremity performance, but its distribution across the broader adult lifespan, particularly in younger adults, remains insufficiently characterized. This study aimed to characterize age- and sex-related 30s-CST performanc

The Association of Free Testosterone Levels With Frailty Metrics in Advanced Chronic Liver Disease.

BACKGROUND: Frailty is common in advanced chronic liver disease (AdvCLD). Key assessment tools for frailty in this population include the liver frailty index (LFI), the 6-minute walk test (6MWT), and the gait speed test (GST). This study investigates the relationship between free testosterone (FT) levels and these frailty measures in men with AdvCLD. METHODS: In this prospective cohort study of 197 male AdvCLD patients awaiting liver transplantation, frailty was assessed using the LFI, 6MWT, and GST. Participants were classified as having low or normal FT levels, and associations with frailty

Muscle mass gain and strength recovery after discharge of critical COVID-19 patients treated with mesenchymal stromal cell infusion: a randomized clinical trial.

INTRODUCTION: during the COVID-19 pandemic, patients with severe infection suffered from profound muscle loss and physical dysfunction. The immunomodulatory and anti-inflammatory properties of mesenchymal stromal cells (MSCs) could represent a new therapeutic strategy, reducing post-COVID-19 sequelae. This study aimed to evaluate the influence of MSC infusion on body composition, functionality, and the occurrence and severity of sarcopenia in critically ill COVID-19 patients after hospital discharge, correlating with clinical outcomes, viral load, and pro-inflammatory cytokines. METHODS: a pr

Upper body kinematics during walking and their relationship to fall risk after stroke.

INTRODUCTION: Falls are highly prevalent after stroke. Although trunk control is critical for dynamic stability during walking, the contribution of upper body (head, neck, thorax, spine) kinematics to fall risk discrimination and prediction remains unknown. This study examined whether trunk kinematics during walking differentiate people with stroke (PwS) with high versus low fall-risk and whether these measures improve fall risk classification. METHODS: Fifty sub-acute PwS from a public 3D gait dataset were analysed. Participants walked barefoot at self-selected speed while full-body kinemati

Prolonged Anti-Zic4 Antibody-Positive Cerebellar Degeneration Following COVID-19 Infection.

BACKGROUND: Although anti-Zic4 antibodies are associated with paraneoplastic cerebellar degeneration, predominantly in small-cell lung carcinoma, their role in postinfectious autoimmunity remains poorly understood. CASE REPORT: Here, we report a 53-year-old woman who developed progressive cerebellar degeneration following coronavirus disease 2019 (COVID-19), characterized by isolated anti-Zic4 antibody positivity in the absence of malignancy. Two months after developing mild respiratory symptoms, she developed dysarthria, imbalance, and gait ataxia. Neurological examination revealed gaze-evok

Diagnostic approach to episodic ataxia types 1 and 2: a proposed algorithm for limited resource-settings.

BACKGROUND: Episodic ataxias (EA) comprise a heterogeneous group of genetic conditions with spells of gait difficulty and imbalance, for which the main causes are EA1 (KCNA1 gene) and EA2 (CACNA1A gene). While EA1 may respond to some antiepileptics and EA2 responds to acetazolamide, no guideline exists to inform decision-making in settings where genetic testing is unavailable. OBJECTIVES: We sought to determine distinguishing clinical features between EA1 and EA2 and propose an algorithm based on our findings. METHODS: Systematized literature review to identify individuals with confirmed pat

Canine paroxysmal dyskinesia: distribution of etiological subtypes and structural and reactive causes in a retrospective tertiary care cohort.

INTRODUCTION: Canine paroxysmal dyskinesia is an episodic movement disorder characterized by transient dystonia, dyskinesia and tremor in dogs with preserved consciousness, yet its differentiation from focal epilepsy remains challenging and a structured diagnostic framework comparable to that used in epilepsy is lacking. METHODS: This retrospective, single-center study evaluated the distribution of etiologies in dogs presenting with paroxysmal dyskinesia to a tertiary referral center between 2012 and 2023. Medical records were systematically screened, and dogs were included if episodic abnorm

A novel deep intronic EIF2AK3 variant disrupts splicing and causes Wolcott-Rallison syndrome.

AIM: Deep intronic variants can disrupt splicing and cause monogenic disease but are missed by routine genetic testing. This study assessed the contribution of deep intronic variants to Wolcott-Rallison syndrome (WRS), a recessive disorder characterized by early-onset diabetes and progressive multisystem disease caused by loss-of-function EIF2AK3 variants. METHODS: We investigated a cohort of 116 individuals referred to the Exeter Genomics Laboratory for genetic testing who had diabetes diagnosed at ≤2 years and at least one additional feature consistent with WRS: hepatic dysfunction, skeleta

A multidimensional analysis of fall risk among older adults in India: evidence from the longitudinal ageing study in India (LASI).

BACKGROUND: Falls are a major public health concern among older adults in India. While earlier studies using LASI data have identified some risk factors, they did not account for key geriatric syndromes, physical performance, and functional status. This study aims to comprehensively examine the determinants of falls among older adults using nationally representative LASI Wave 1 data. METHODS: We analysed data from 26,780 community-dwelling individuals aged ≥ 60 years from LASI Wave 1. Falls were self-reported. Covariates included socio-demographics, comorbidities, ADL/IADL, cognition, depress

Humans and platyrrhines share a pattern of articular variance.

Previous work has identified a pattern of articular shape variation in modern human limb joints, in which convex joint surfaces exhibit constrained variance relative to their more variable concave conarticulars. This pattern is notable given that joint morphogenesis is influenced by local biomechanical environments, which vary substantially across joints and throughout development. Whether this biased variance pattern reflects generalizable developmental or biomechanical constraints, or instead represents a human-specific phenomenon, remains unclear. Here, we test the generalizability of this

Effects of First Metatarsophalangeal Osteoarthritis on Plantar Pressures Across Multiple Activities.

BACKGROUND: The first metatarsophalangeal joint (MTPJ1) is one of the sites most often affected by osteoarthritis (OA). The condition contributes to activity restrictions and reduced quality of life along with biomechanical changes that affect the joint and beyond. Despite this, previous studies have presented limited evidence of alterations in plantar pressures during level walking. In this case-control study, we hypothesized that activities that may stress the joint to a greater extent than standard gait may reveal greater differences in plantar pressures between those with MTPJ1 OA and heal
