A 52 yrs old male presented with multiple, painless, firm nodules over extremities, which were mimicking benign neoplastic lesion. Fine needle aspiration was performed from three such nodules which revealed chalky white aspirate. After staining the smears with H&E and Giemsa stains, smears show amorphous pink material, needle shaped crystalline structures, many macrophages and foreign body type giant cells. A diagnosis of gouty tophi was offered which was confirmed by histopathology and serum uric acid level.

PURPOSE OF REVIEW: This review is an attempt to keep current in the sparse literature addressing the still underutilized area of crystal identification.
RECENT FINDINGS: The emphasis has been on the subtleties of the microscopic identification of common crystals and other less common potentially confusing crystals. Imaging is noted to provide increasing help, but microscopic crystal identification remains the gold standard.
SUMMARY: Quality control is still a concern as is the infrequency of attempted arthrocentesis for crystal identification.

We report a case of colchicine-induced myopathy, in which the initial presenting and predominant clinical feature was respiratory muscle dysfunction; there was also chronic renal failure and electromyographically-measured myopathy. Discontinuation of colchicine led to marked improvement. Colchicine discontinuation was the only therapy performed (other medications were unchanged), and within 3 weeks the patient had regained motor function and resumed daily activities. During long-term therapy of hyperuricaemia with allopurinol (300 mg/d) two patients developed generalized granuloma annulare. The diagnoses were confirmed by histology; concomitant diseases, especially diabetes mellitus, were not detectable. Following discontinuation of allopurinol therapy, cutaneous granulomas healed without relapse. Hyperuricaemia could be controlled by low-purine diet and medication avoiding allopurinol. A variety of non-neoplastic, tumor-like conditions exist in the upper extremity. Some are common, others rare. Careful history, physical diagnosis, and a variety of imaging modalities can be helpful in arriving at an accurate diagnosis and developing a successful treatment plan for this group of lesions.

When compared to values obtained in 40 normalweight normolipemic healthy control subjects, serum uric acid levels were found to be significantly increased in 68 subjects with type II-b and especially in 68 subjects with type IV hyperlipoproteinemia.
