Supporting patient autonomy through respectful labor and childbirth healthcare services.

Despite childbirth being a leading cause of hospitalization, the system of care has not been optimized for patients in the United States. Rates of maternal mortality are high, increasing, and disproportionate. In addition to disparities in health outcomes, the quality of perinatal care, as assessed by patient experiences of care, is variable. Although clinicians strive to actively support families through labor and childbirth, various forms of mistreatment are common in perinatal care. Such instances of patient mistreatment highlight an urgent need to further enact respectful maternity care pr

Addressing the burden of violence on global mental health: contributions of Narrative Exposure Therapy across different health systems.

Post-traumatic stress disorder represents a substantial global mental health burden, particularly in the face of cumulative violence, forced migration, and structural inequities. Narrative Exposure Therapy (NET) configures a brief trauma-focused intervention that supports the reconstruction of autobiographical memory by the chronological narration of life events. By integrating fragmented traumatic experiences into a coherent narrative, NET facilitates emotional processing and restores continuity to disrupted life stories. This regional case series examines the integration of NET into the ment

The Political Sociology of NICE: Investigating Pharmaceutical Cost-Effectiveness Regulation in the UK.

The National Institute for Health and Care Excellence (NICE) was established a quarter of a century ago in 1999 to regulate the cost-effectiveness of pharmaceuticals (and other health technologies) for the NHS. Drawing on medical sociology theories of corporate bias, neoliberalism, pluralism/polycentricity and regulatory capture, the purpose of this article is to examine the applicability of those theories to NICE as a key regulatory agency in the UK health system. Based on approximately 7 years of documentary research, interviews with expert informants and observations of NICE-related meeting

Temporal trend and epidemiological profile of exogenous poisonings in Brazil: an ecological study from 2007 to 2019.

Poisonings are a global public health problem. With the aim of describing their epidemiological profile in Brazil, an ecological time series study was conducted from 2007 to 2019, characterizing individual demographic variables (sex; age), exposure variables (circumstance; toxic agents), health indicators (incidence; mortality; fatality), and temporal trends. Data concerning notifications were extracted from the Notifiable Diseases Information System (SINAN) and stratified by macro-region, sex, age, circumstance, and toxic agents. The indicators of incidence, fatality, and mortality were calcu

Human Monkeypox-A Global Public Health Emergency.

Monkeypox, a viral zoonosis caused by an Orthopoxvirus, is clinically characterized by fever, headache, lymphadenopathy, myalgia, rash and burdened by some complications that can be severe and life threatening. Monkeypox, endemic in some central and west African countries, in tropical areas near equator, rose to the headlines following its recent outbreak in non-endemic countries of Europe and the USA. Thus, the World Health Organization, worried about the growing dimension of the problem, declared monkeypox a global public health emergency. Now, after months of careful observation, the wester

[Loss of risk perception to zoonoses and the figure of community dog].

Dogs may cause zoonoses and damages; especially when we accept as normal some dangerous behavior, losing the perception of disease risk. The purpose of this review was to demonstrate that the presence of stray dogs as well as community dogs, are public health hazard. Through sustainable epidemiological information, we believe this measure could prevent it from spreading. Dog's aggressiveness regulates its social relationships through submission or dominance; those characteristics are difficult to be recognized by human population. In Chile, 327.150 people were bitten by dogs, between years 200

General knowledge and awareness on rare diseases among general practitioners in Bulgaria.

Rare diseases are a serious public health problem and are a threat to the health of EU citizens. Important role in the area of rare diseases have the medical specialists who diagnose and monitor the course of the disease of each patient. General practitioners (GPs) are usually the first to identify "unusual" patients that might have a rare disease. The GPs awareness and knowledge about rare diseases is a strong factor for the timely and accurate diagnosis and adequate treatment of rare diseases conditions. A telephone interview was conducted among the GPs in Bulgaria between January and March

Self-Management Systems for Patients and Clinicians in Parkinson Care: Protocol for an Integrated Scoping Review, Product Search, and Evaluation.

BACKGROUND: Parkinson disease (PD) poses emotional and financial challenges to patients, families, caregivers, and health care systems. Self-management systems show promise in empowering people with PD and enabling more control over their treatment. The collaborative nature of PD care requires communication between patients and health care professionals. While past reviews explored self-management systems in PD diagnosis and symptom management with a focus on patient portals, there is limited research addressing the interconnectivity of systems catering to the needs of both patients and clinic

PROmoting Pain Self-Management for Chronic Musculoskeletal Pain in the Military Health System, comparing health coaching to Empowered Relief: the PROs trial protocol.

BACKGROUND: Chronic musculoskeletal pain is a major problem for military beneficiaries, with incident rates increasing more than 3-fold from 2009 to 2018. Guidelines recommend nonpharmacologic, holistic approaches within a stepped care framework. Two promising strategies, Empowered Relief (ER), a lower-intensity single-session pain self-management intervention, and Move-to-Health (M2H), a higher-intensity holistic health-coaching program, offer scalable options. However, comparative effectiveness data and optimal sequencing of these two interventions within stepped care remain unknown. OBJECT

The cost and cost drivers of delivering COVID-19 vaccines in low- and middle-income countries: a bottom-up costing study of rollouts in seven countries.

INTRODUCTION: While COVID-19 vaccines were crucial in containing the pandemic, there is limited evidence on the cost of delivering them in low- and middle-income countries. We estimated the cost of delivering COVID-19 vaccines in Bangladesh, Côte d'Ivoire, the Democratic Republic of the Congo, Mozambique, the Philippines, Uganda, and Vietnam. METHODS: We retrospectively estimated the financial and economic cost of COVID-19 vaccine delivery from a payer perspective, using a bottom-up approach. Cost data were collected from 290 sites, while qualitative interviews were conducted with 192 key inf

The Patient-Reported Outcomes Measurement Information System perspective of adults with long-standing atypical hemolytic uremic syndrome.

BACKGROUND: Atypical hemolytic uremic syndrome (aHUS) is a thrombotic microangiopathy driven by a dysregulation of the complement system, disproportionately affecting the kidneys. While terminal complement blockade has reduced the risk of death and progression to end stage renal disease, patients face significant chronic health challenges. The patient experience in the current treatment era has not been well-characterized. OBJECTIVES: To evaluate patient-reported health outcomes (PRO) across global health domains, including physical and cognitive function and social isolation. METHODS: We pe

From neglected to notable: A growing public health challenge driven by hospitalization for sporotrichosis in Pernambuco, Northeast Brazil (2016-2024).

Opportunistic and endemic mycoses represent a significant global health concern, particularly in Brazil. In the northeastern state of Pernambuco (PE), sporotrichosis has become a rapidly escalating public health challenge, evidenced by a notable increase in hospital admissions, despite its mostly outpatient nature. The present study, therefore, aimed to investigate the epidemiological and clinical profile of mycosis-related hospitalizations in PE from 2016 to 2024, with a primary focus on the burden of sporotrichosis. This statewide surveillance study used data from Brazil's Unified Health Sys

Association of Cumulative Viral Load With the Incidence of Hypertension and Diabetes in People With HIV.

BACKGROUND: HIV induces several metabolic derangements that contribute to cardiovascular disease, but it is unclear if HIV increases diabetes or hypertension risk. Refining longitudinal relationships between HIV-specific factors and cardiovascular disease risk factors across different care settings may help inform cardiovascular disease prevention among people with HIV (PWH). METHODS: We tested the hypothesis that long-term higher cumulative viral load (viremia-copy-year) is associated with higher risk of diabetes and hypertension by analyzing electronic records of PWH from 2 distinct health
