Genetic encoding of an esophageal motor circuit.

Motor control of the striated esophagus originates in the nucleus ambiguus (nAmb), a vagal motor nucleus that also contains upper airway motor neurons and parasympathetic preganglionic neurons for the heart and lungs. We disambiguate nAmb neurons based on their genome-wide expression profiles, efferent circuitry, and ability to control esophageal muscles. Our single-cell RNA sequencing analysis predicts three molecularly distinct nAmb neuron subtypes and annotates them by subtype-specific marker genes: Crhr2, Vipr2, and Adcyap1. Mapping the axon projections of the nAmb neuron subtypes reveals

A two-step method for paroxysmal atrial fibrillation event detection based on machine learning.

Detection of atrial fibrillation (AF) events is significant for early clinical diagnosis and appropriate intervention. However, in existing detection algorithms for paroxysmal AF (AFp), the location of AF starting and ending points in AFp is not concerned. To achieve an accurate identification of AFp events in the long-term dynamic electrocardiograms (ECGs), this paper proposes a two-step method based on machine learning. In the first step, based on features extracted from the calculated R-to-R intervals (RR intervals, the cycle of heart beat), the rhythm type of the ECG signal is first classi

Classification of human body motions using an ultra-wideband pulse radar.

BACKGROUND: The motion or gestures of a person are primarily recognized by detecting a specific object and the change in its position from image information obtained via an image sensor. However, the use of such systems is limited due to privacy concerns. OBJECTIVE: To overcome these concerns, this study proposes a radar-based motion recognition method. METHODS: Detailed human body movement data were generated using ultra-wideband (UWB) radar pulses, which provide precise spatial resolution. The pulses reflected from the body were stacked to reveal the body's movements and these movements we

Effect of temporal resolution on the detection of cardiac arrhythmias using HRV features and machine learning.

Objective.Arrhythmia is an abnormal cardiac rhythm that affects the pattern and rate of the heartbeat. Wearable devices with the functionality to measure and store heart rate (HR) data are growing in popularity and enable diagnosing and monitoring arrhythmia on a large scale. The typical sampling resolution of HR data available from non-medical grade wearable devices varies from seconds to several minutes depending on the device and its settings. However, the impact of sampling resolution on the performance and quality of arrhythmia detection has not yet been quantified.Approach.In this study,

Cost-Sensitive Learning for Anomaly Detection in Imbalanced ECG Data Using Convolutional Neural Networks.

Arrhythmia detection algorithms based on deep learning are attracting considerable interest due to their vital role in the diagnosis of cardiac abnormalities. Despite this interest, deep feature representation for ECG is still challenging and intriguing due to the inter-patient variability of the ECG's morphological characteristics. The aim of this study was to learn a balanced deep feature representation that incorporates both the short-term and long-term morphological characteristics of ECG beats. For efficient feature extraction, we designed a temporal transition module that uses convolutio

Understanding Metabolic Alterations in Cancer Cachexia through the Lens of Exercise Physiology.

Cancer cachexia is one of the leading causes of mortality for late-stage cancer patients. One of its key characteristics is abnormal metabolism and loss of metabolic flexibility, i.e., loss of ability to switch between use of fats and carbohydrates as needed. Here, it is hypothesized that late-stage systemic cancer creates a chronic resource drain on the body that may result in the same metabolic adaptations that occur during intense endurance exercise, activating some of the same mechanisms of nutrient consumption that are supposed to be transient during strenuous physical activity. This hypo

The Fundamental Basis of Palpitations: A Neurocardiology Approach.

BACKGROUND AND OBJECTIVE: Palpitations are a common symptom that may indicate cardiac arrhythmias, be a somatic complaint in anxiety disorders, and can be present in patients without either condition. The objective of this review was to explore the pathways and fundamental mechanisms through which individuals appreciate palpitations. OBSERVATIONS: Cardiac afferents provide beat-to-beat sensory information on the heart to the spinal cord, brain stem, and higher brain centers. Cardioception, a subset of interoception ('the physiological sense of the condition of the body'), refers to sensing of

Inappropriate Sinus Tachycardia: Etiology, Pathophysiology, and Management: JACC Review Topic of the Week.

Inappropriate sinus tachycardia (IST) is a clinical syndrome that generally affects young patients and is associated with distressing symptoms. Although the most common symptom is palpitations, it can be accompanied by a myriad of symptoms, including anxiety, dizziness, presyncope, and syncope. The pathogenesis of IST is not well understood and considered multifactorial, with autonomic dysfunction being the central abnormality. IST is a diagnosis of exclusion. Management presents a clinical challenge. The overall efficacy of lifestyle modifications and medical therapy may be limited. Recent ad

Improving Ventilation Rates During Pediatric Cardiopulmonary Resuscitation.

BACKGROUND: Excessive ventilation at rates of 30 breaths per minute (bpm) or more during cardiopulmonary resuscitation (CPR) decreases venous return and coronary perfusion pressure, leading to lower survival rates in animal models. A review of our institution's pediatric CPR data revealed that patients frequently received excessive ventilation. METHODS: We designed a multifaceted quality improvement program to decrease the incidence of clinically significant hyperventilation (≥30 bpm) during pediatric CPR. The program consisted of provider education, CPR ventilation tools (ventilation reminde

Study on the nonfatigue and fatigue states of orchard workers based on electrocardiogram signal analysis.

In recent years, fatigue has become an important issue in modern life that cannot be ignored, especially in some special occupations. Agricultural workers are high-risk occupations that, under fatigue conditions over a long period, will cause health problems. In China, since very few studies have focused on the fatigue state of agricultural workers, we were interested in using electrocardiogram (ECG) signals to analyze the fatigue state of agricultural workers. Healthy agricultural workers were randomly recruited from hilly orchards in South China. Through the field experiment, 130 groups of 5

Ophthalmologic school-based screening revealing Kearns-Sayre syndrome: a case report.

Kearns-Sayre syndrome is a rare mitochondrial disorder. It had a triad of features, including progressive external ophthalmoplegia, pigmentary retinopathy, and an alteration of cardiac conduction. The ocular manifestations include bilateral ptosis, progressive external ophthalmoplegia, and atypical pigmentary retinopathy. We report the case of a 9-year-old Moroccan patient who has been diagnosed with Kearns-Sayre syndrome during an ophthalmologic school-based screening. This case highlights the interest of school-based screening in the diagnosis and management of a rare disease.

Nonsuicidal Self-Injury Is Associated With Attenuated Interoceptive Responses to Self-Critical Rumination.

Nonsuicidal self-injury (NSSI) is a prevalent and dangerous behavior. Those with a history of NSSI often report high levels of self-critical rumination (SCR), a form of negatively valenced introspective self-referential processing. It is plausible that this overly analytical style of relating to the self might hinder the ability to process interoceptive signals, thereby increasing the capacity to engage in behaviors that cause bodily harm. Two studies investigated whether trait or state SCR influenced aspects of interoception in those with and without a history of NSSI. In Study 1 (N = 180), i

A rare case of pacemaker mediated tachycardia caused by intrinsic conduction search function of a dual chamber pacemaker.

Pacemaker-mediated tachycardia (PMT) is a well-described cause of tachyarrhythmia in patients with dual chamber pacemakers (Barold, 1991 [1]). PMT is usually caused by common triggers including premature ventricular or atrial complexes (Alasti et al., 2018 [2]). However, PMT can also be triggered by other uncommon mechanisms. Intrinsic conduction search function is a feature available in some dual chamber permanent pacemakers that periodically looks for intrinsic conduction by gradually prolonging the preprogrammed AV delay in an attempt to minimize ventricular pacing (Calvi et al., 2017; Paka
