Stentless Versus Stented Aortic Valve Replacement for Aortic Stenosis.

BACKGROUND: The differences in long-term outcomes of aortic valve replacement for aortic stenosis between stentless and stented bioprostheses are controversial. METHODS: Between 2007 and 2018, 1173 patients underwent aortic valve replacement for aortic stenosis, including 559 treated with a stentless valve and 614 with a stented valve. A propensity score matched cohort with 348 pairs was generated by matching for age, sex, body surface area, bicuspid aortic valve, chronic lung disease, previous cardiac surgery, coronary artery disease, renal failure on dialysis, valve size, concomitant proced

Comparative analysis of surgical and percutaneous pulmonary valve implants over a 20-year period.

OBJECTIVES: Since percutaneous pulmonary valve implantation (PPVI) was introduced to prolong the lifetime of surgically placed right ventricular to pulmonary artery conduits, valve technology has evolved and the indications for PPVI expanded to native and larger right ventricular outflow tracts. We explore how indications, patient populations and outcomes compare to surgical pulmonary valve replacement (PVR). METHODS: This is a retrospective cohort study of PPVI and PVR procedures between 1998 and 2020 at a single UK centre. One hundred and twenty-eight patients underwent PPVI and 365 patient

Alternative and Safe Technique to Tie Down a Biologic Aortic Valve in Small Aortic Roots.

Biologic aortic valves are constructed with 3 stents corresponding to the commissures, which sit in the aortic root. When implanted using interrupted sutures, these stents may make the valve difficult to tie down safely in patients with small, calcified aortic roots. We present an easily reproducible technique to make this valve tie down safer.

Sutureless Valve in Repeated Aortic Valve Replacement: Results from an International Prospective Registry.

OBJECTIVE: To report early and midterm results registry of patients undergoing repeated aortic valve replacement (RAVR) with sutureless prostheses from an international prospective registry (SURE-AVR). METHODS: Between March 2011 and June 2019, 69 patients underwent RAVR with self-expandable sutureless aortic bioprostheses at 22 international cardiac centers. RESULTS: Overall mortality was 2.9% with a predicted logistic EuroSCORE II of 10.7%. Indications for RAVR were structural valve dysfunction (84.1%) and infective prosthetic endocarditis (15.9%) and were performed in patients with previo

Midterm Outcomes of Stented Versus Stentless Bioprosthetic Valves After Aortic Root Replacement.

To determine the impact of aortic root replacement (ARR) with a stentless bioprosthetic valve on midterm outcomes compared to a stented bioprosthetic valve-graft conduit. This was an observational study of aortic root operations from 2010 to 2018. All patients with a complete ARR for nonendocarditis reasons were included, while patients undergoing valve-sparing root replacements or primary aortic valve replacement or repair were excluded. Of the patients with a complete ARR, bioprosthetic valve implants were included, while mechanical valve implants were excluded. Patients were dichotomized in

Update on surgical repair in functional mitral regurgitation.

BACKGROUND: Functional mitral regurgitation (FMR) is common in patients with myocardial infarction or dilated cardiomyopathy, and portends a poor prognosis despite guideline-directed medical therapy (GDMT). Surgical or transcatheter mitral repair for FMR from recent randomized clinical trials showed disappointing or conflicting results. AIMS: To provide an update on the role of surgical repair in the management of FMR. MATERIALS AND METHODS: A literature search was conducted utilizing PubMed, Ovid, Web of Science, Embase, and Cochrane Library. The search terms included secondary/FMR, ischemi

Transcatheter Mitral Valve Replacement: A Novel Anchor Technology.

BACKGROUND: Mitral valved stents tend to migrate or to develop paravalvular leakage due to high-left ventricular pressure in this cavity. Thus, this study describes a newly developed mitral valved stent anchoring technology. METHODS: Based on an existing mitral valved stent, four anchoring units with curved surgical needles were designed and fabricated using three-dimensional (3D) software and print technology. Mitral nitinol stents assembled with four anchoring units were successively fixed on 10 porcine annuli. Mechanical tests were performed with a tensile force test system and recorded th

Contemporary Status of Percutaneous Transcatheter Edge-to-Edge Repair: Is It a Complement or Replacement to Mitral Surgery?

Transcatheter mitral valve repair devices borrow from the concept of surgical edge-to-edge approximation and are becoming increasingly used in patients with both primary and functional mitral regurgitation. The application of these new devices is expanding globally; however, debates between which patients are amenable to surgery vs. percutaneous approaches are ongoing. As new trials and regulatory approvals have evolved, the indications for transcatheter approaches have expanded, in a way that is complementaty to existing indications for surgical repair. In general, the treatment of mitral reg

MitraClip XTR Implantation to Treat Torrential Tricuspid Regurgitation After Failed Annulopasty.

Transcatheter edge-to-edge tricuspid valve repair is emerging as an alternative therapeutic option in symptomatic patients with severe tricuspid regurgitation deemed unfit for open surgery. However, the presence of a dehiscent ring usually prevents transcatheter tricuspid valve intervention. Herein, we report a case of torrential tricuspid regurgitation secondary to ring dehiscence successfully treated with a MitraClip.
