Signs, symptoms and complications of complete and partial uterine ruptures during pregnancy and delivery.

OBJECTIVES: Uterine rupture is a rare but potentially catastrophic complication of pregnancy that requires rapid diagnosis. Classically, its signs and symptoms combine pain, fetal heart rate (FHR) abnormalities, and vaginal bleeding. The purpose of this study is to identify these signs and symptoms as well as the immediate complications of complete and incomplete (partial) ruptures of the uterine wall, whether or not they follow a previous cesarean delivery. STUDY DESIGN: Retrospective study of case records from two university hospital maternity units, from 1987 to 2008. RESULTS: In a total

Ultrasound pregnancy dating leads to biased perinatal morbidity and neonatal mortality among post-term-born girls.

BACKGROUND: Ultrasound assessment of gestational length is based on the assumption that fetuses of the same gestational age have equal size at the time of investigation. However, there are detectable sex differences in fetal size by the end of the first trimester. We examined whether ultrasound dating introduces sex differences in risks of adverse perinatal outcomes related to post-term birth. METHODS: We used the Swedish Medical Birth Register to compare male and female newborns during 1973-1978, when gestational age was based on the last menstrual period, and 1995-2007, when gestational age

A 12-month prospective survey of perinatal outcome of liveborn neonates in Julu County, China.

BACKGROUND: Population based epidemiologic study on the main diseases and birth status of liveborn neonates remains scarce in China, especially in rural areas where a large number of neonates are born. The aim of this study was to establish an epidemiological basis of live births in Julu County, a representative of the northern and mid-western parts of China in terms of demography, disease pattern and women and children's health care infrastructure. METHODS: The perinatal data of all live births were prospectively collected in three participating county-level hospitals from September 1, 2007

Tuberculin reaction, BCG scar, and lower female mortality.

BACKGROUND: Recent studies have suggested that bacille Calmette-Guérin (BCG) immunization may have a nonspecific beneficial effect on infant survival and that the effect may be more pronounced among girls. In a prospective birth cohort, we examine whether a positive tuberculin skin test and BCG scar in response to BCG immunization were related to better overall survival in Guinea-Bissau and, if so, whether the effect was sex-specific. METHODS: Skin tests and BCG scarring were monitored at ages 2 months (n = 2332) and 6 months (n = 1817) in children born from March 2000 to July 2002. A tubercu

Do childhood vaccines have non-specific effects on mortality?

A recent article by Kristensen et al. suggested that measles vaccine and bacille Calmette-Gu rin (BCG) vaccine might reduce mortality beyond what is expected simply from protection against measles and tuberculosis. Previous reviews of the potential effects of childhood vaccines on mortality have not considered methodological features of reviewed studies. Methodological considerations play an especially important role in observational assessments, in which selection factors for vaccination may be difficult to ascertain. We reviewed 782 English language articles on vaccines and childhood mortali

Malaria in the first 6 months of life in urban African infants with anemia.

A total of 446 infants in the first 6 months of life who presented at an urban children's hospital with complaints of any illness whatsoever were recruited into a study with the aim of determining the contribution of malaria to infant morbidity in a malaria-endemic urban area in Nigeria. Sixty-eight of the infants were in their first month of life and 79, 77, 61, 97, and 64 were in their second, third, fourth, fifth and sixth month of life, respectively. Overall, 107 (24.0%) infants were clinically diagnosed as having malaria. This included 3 who were in the first month of life, 12 in the seco

Perinatal mortality in urban slums in Lucknow.

OBJECTIVES: To determine the perinatal mortality rate (PNMR) in the urban slums of Lucknow DESIGN: Cross-sectional survey. SETTING: Twenty five Anganwadi centres of urban Lucknow, with a population of 25,901. METHODS: Data was collected on birth and early neonatal deaths, gestational age of the neonate (determined at birth) and maternal variables like socio-economic status, maternal age, parity, and bad obstetrical history from January 1992 to March 1993. RESULTS: There were 966 births with a still-birth rate of 37.2 and PNMR of 59.0 per 1000. The relative risk of perinatal mortality with

Perinatal deaths as a result of immaturity in Jamaica.

During the 12-month period from 1 September 1986 to 31 August 1987 an attempt was made to collect information on all perinatal deaths occurring on the island of Jamaica. Of the 2069 late fetal and early neonatal deaths identified, 19% fell into the Wigglesworth definition of 'deaths from immaturity'. Twins were 11 times more likely to die of immaturity than were singletons, and twins comprised 18% of all deaths in this group. Comparison of the singleton deaths from immaturity, with 9919 singletons born on the island during the 2-month period of September and October 1987 and who survived the f

Perinatal mortality in Jinnah Postgraduate Medical Centre, Karachi.

During the one year period from 1st January to 31st December, 1989, there were 6,826 deliveries and 695 perinatal deaths, of these 553 were still-born and 142 were neonatal deaths (died within 7 days of birth). The perinatal mortality rate was 101.8 per thousand total births. The still-birth rate was 81 per thousand total births. Of the perinatal deaths, 7.9% occurred in booked and 92.1% in non-booked cases. The commonest cause of death was antepartum haemorrhage (APH), 24.2% followed by mechanical which accounted for 23.6% deaths. Abruptio placentae was the leading cause among all still-birth

The Guadeloupean perinatal mortality audit: process, results, and implications.

In 1983, we implemented an ongoing perinatal mortality audit in Guadeloupe to identify factors underlying the current level of poor perinatal outcome and to stimulate increased provider motivation. The audit was part of an active research approach to document the specific nature of the perceived perinatal health problem, to modify provider behavior through the use of this acquired information, and, ultimately, to decrease avoidable perinatal deaths. We investigated all 320 perinatal deaths in Guadeloupe during 1984 and 1985. Nearly one-half of the perinatal deaths reviewed were determined to b

Declining perinatal mortality in a region of Finland, 1968-82.

Perinatal mortality (PNM) in the catchment area of the University Central Hospital of Turku (UCHT), Finland, was investigated during a 15-year period from 1968 to 1982. During the study period, 82,151 babies were born, there were 531 fetal deaths and 505 cases of early neonatal death. The PNM rate declined during the study period from 17.9 in 1968 to 7.0 in 1982, or from 14.8 to 4.6 when infants weighing less than 1000 grams were excluded. Significant declines occurred in PNM due to maternal illness, placental and umbilical cord complications, other asphyxias and respiratory distress syndrome.

Has modern perinatal practice caused the fall in perinatal mortality? The experience of a district maternity hospital.

During the first 12 years of operation the perinatal mortality rate in Paisley Maternity Hospital fell steadily from 27 per 1,000 in 1970 to 10 per 1,000 in 1981. During this period the nulliparous birth rate remained constant, but the parous birth rate fell. Improved survival of premature babies, falling numbers of babies with neural tube defects and reduction in intrapartum asphyxia are identified as responsible for this fall. Unexplained intra-uterine death remains an unsolved problem.

[Twin pregnancies: incidence, fetal development and perinatal mortality].

The authors study 233 pairs of twins in a big maternity of Tunis, they analyse the frequency, the intra-uterine growth and the perinatal mortality. The frequency of twinning was 1.56%, the occurrence of preterm delivery was 45.92% and foetal hypotrophy was observed in 9.84% of the cases. The perinatal mortality rate was 143.77%: still born = 55.79% and neonatal deaths = 87.98%; the ratio of still born to neonatal deaths was 1.6. 29.3% of the neonatal twin deaths were due to hyaline membrane disease, 24.5% to immaturity, 14.6% to perinatal asphyxia and 12.4% to infection. The authors compare th

Babies of mothers with leprosy have small placentae, low birth weights and grow slowly.

One hundred and sixteen women with leprosy and 31 healthy controls were studied throughout 155 pregnancies, and their babies were observed for a period of up to two years. Babies of mothers with leprosy weighed less than those of healthy mothers; the placental weights and coefficients followed the same trend. The babies of the mothers with leprosy grew more slowly than those of the healthy mothers and these findings were most marked in the babies of mothers with lepromatous leprosy. The cause of the reduced feto-placental weight is thought to be related to the immune status of the mother.
