Acute poisoning caused by inhalation of carbon monoxide and other toxic substances is the primary cause of death in fires and may occur without signs of external injury. Life-threatening symptoms may arise immediately, as in cyanide poisoning, or over a longer period, as in carbon monoxide poisoning. Severe inhalation injury may also occur independently of systemic poisoning and should always be suspected in patients with soot on their face and in the respiratory tract, or hoarseness and wheezing.

Chlorine inhalation may rapidly cause pulmonary edema, leading to acute hypoxemic respiratory failure. We report a 12 year old with acute respiratory failure from inhalation injury after he accidentally dropped chlorine tablets into a swimming pool. Supplemental oxygen alone failed to provide adequate arterial oxygenation. We administered positive end expiratory pressure (PEEP) in the emergency department, resulting in markedly improved oxygenation. Early institution of PEEP should be considered in patients with chlorine poisoning when supplemental oxygen alone is insufficient.

A 22-year-old male suffering from bronchial obstruction, in whom the usual anti-allergic treatment was almost ineffective, had a high degree of blood eosinophilia and migrating pulmonary infiltrates. These infiltrates were strongly suspected to be of eosinophilic origin, in view of the results of bronchoalveolar lavage. Unfortunately, no clear cause of the disorder was found. Corticosteroid treatment per os and by inhalation clearly improved his condition. However, the treatment with inhalation corticosteroids has to be continued indefinitely.

OBJECTIVE: To identify the determinants of immediate outcome after rigid bronchoscopy for suspected or confirmed foreign body (FB) aspiration.
