Evidence based therapeutic apheresis in autoimmune and other hemolytic anemias.

PURPOSE OF REVIEW: This article reviews recent publications that bear on the evidential basis for therapeutic apheresis in diseases in which hemolytic anemia is a prominent feature. RECENT FINDINGS: Therapeutic plasma exchange continues to be reported sporadically in severe autoimmune hemolytic anemia, with inconsistent results. Autoimmune deficiency of ADAMTS13 has provided a compelling rationale for therapeutic plasma exchange in some patients with thrombotic thrombocytopenic purpura; conversely such deficiency is consistently absent in certain clinically similar syndromes for which therape

Plummer-Vinson syndrome.

Plummer-Vinson or Paterson-Kelly syndrome presents as a classical triad of dysphagia, iron-deficiency anemia and esophageal webs. Exact data about epidemiology of the syndrome are not available; the syndrome is extremely rare. Most of the patients are white middle-aged women, in the fourth to seventh decade of life but the syndrome has also been described in children and adolescents. The dysphagia is usually painless and intermittent or progressive over years, limited to solids and sometimes associated with weight loss. Symptoms resulting from anemia (weakness, pallor, fatigue, tachycardia) ma

Iron Laboratory Studies in Pediatric Patients With Heart Failure from Dilated Cardiomyopathy.

Iron deficiency (FeD), with or without anemia, in adults with heart failure (HF) is associated with poor outcomes, which can be improved with replacement therapy. A similar therapeutic opportunity may exist for children; however, iron laboratory measurements and FeD have not been described in pediatric patients with HF. A single-center, retrospective study was conducted on 28 patients <21 years old with a diagnosis of dilated cardiomyopathy and HF who had iron laboratories (serum iron, iron saturation, and ferritin) performed. The mean (standard deviation) age at time of laboratory collection

Mentzer Index in breast cancer: Insights into anemia and tumor progression - a narrative review.

Anemia is a prevalent condition in breast cancer patients, often linked to factors such as chemotherapy, nutritional deficiencies, and tumor-related inflammation. The Mentzer Index, traditionally used to differentiate iron deficiency anemia from thalassemia, has recently gained attention as a tool for diagnosing and managing anemia in oncology. This review explores the application of the Mentzer Index in breast cancer, highlighting its potential role in identifying the underlying causes of anemia and guiding treatment strategies. By assessing the Mentzer Index, clinicians can better understand

Ziltivekimab for anemia and atherosclerosis in chronic kidney disease: a new hope?

Anemia of chronic kidney disease is a multifactorial condition secondary to various etiologies, including nutritional deficiencies, chronic inflammation, erythropoietin deficiency or resistance, bone marrow suppression, iron deficiency and adverse drug effects. The major therapeutic intervention for anemia among chronic kidney disease patients is erythropoiesis-stimulating agents. However, a limitation of erythropoiesis-stimulating agents is the risk for thromboembolic events, hypertension, seizures, solid organ malignancies and hyporesponsiveness. A novel interleukin-6 monoclonal antibody, zi

Global malnutrition overlaps with pollinator-dependent micronutrient production.

Pollinators contribute around 10% of the economic value of crop production globally, but the contribution of these pollinators to human nutrition is potentially much higher. Crops vary in the degree to which they benefit from pollinators, and many of the most pollinator-dependent crops are also among the richest in micronutrients essential to human health. This study examines regional differences in the pollinator dependence of crop micronutrient content and reveals overlaps between this dependency and the severity of micronutrient deficiency in people around the world. As much as 50% of the p

Iron Metabolism in the Colorectal Tumor Microenvironment: Current Evidence and Clinical Implications.

Iron is essential for normal cellular function, but its dysregulation is increasingly recognized as a key factor in colorectal tumorigenesis. This review provides an integrated overview of iron-related biomarkers across the full spectrum of colorectal neoplasia, from preneoplastic lesions to advanced colorectal cancer (CRC). Evidence suggests that alterations in iron metabolism begin early, at the level of colorectal adenomas, where increased iron uptake and impaired export contribute to local iron accumulation and oxidative stress. As lesions progress to carcinoma, this imbalance becomes more

Iron and iron-related proteins in alcohol consumers: cellular and clinical aspects.

Alcohol-associated liver disease (ALD) is one of the most common chronic liver diseases. Its pathological spectrum includes the overlapping stages of hepatic steatosis/steatohepatitis that can progress to liver fibrosis and cirrhosis; both are risk factors for hepatocellular carcinoma. Moreover, ALD diagnosis and management pose several challenges. The early pathological stages are reversible by alcohol abstinence, but these early stages are often asymptomatic, and currently, there is no specific laboratory biomarker or diagnostic test that can confirm ALD etiology. Alcohol consumers frequentl

Iron Status of Vegans, Vegetarians and Pescatarians in Norway.

Although plant-based diets provide well-established physical and environmental health benefits, omitting meat or meat products has also been associated with a risk of being deficient in specific nutrients, such as iron. As data on the iron status among Norwegian vegans, vegetarians and pescatarians are lacking, the present study aimed to assess iron status in these groups of healthy adults. Blood markers for iron status were measured in 191 participants (18-60 years old) comprising 106 vegans, 54 vegetarians and 31 pescatarians: serum-ferritin (S-Fe), serum-iron (S-Iron) and serum-total iron b

Nutritional deficiencies in low-sociodemographic-index countries: a population-based study.

BACKGROUND: We aimed to estimate the burden of nutritional deficiency according to sex and age in countries with a low sociodemographic index (SDI). METHODS: Following the methods of the Global Burden of Diseases, Injuries, and Risk Factors Study 2019, estimated annual percentage changes (EAPCs) were calculated to determine trends in the age-standardized rates of incidence and disability-adjusted life-years (DALYs) of nutritional deficiency and its main subcategories from 1990 to 2019 in low-SDI countries. FINDINGS: From 1990 to 2019, the age-standardized incidence and DALY rates of nutritio

New insights on iron overload-mediated lysosomal membrane rupture in liver regeneration following acetaminophen-induced liver injury.

Acetaminophen (APAP) overdose is a leading cause of liver failure in Western countries and a major clinical challenge because effective treatment options remain limited for patients who present after the optimal early therapeutic window. Iron is distributed among several subcellular compartments, such as cytosol, mitochondria, ferritin, and endosomes/lysosomes. Emerging evidence indicates that intracellular iron homeostasis and organelle function may be critical during the regeneration phase for repairing liver damage. Our recent work identified the metalloreductase six-transmembrane epithelia

Iron supplementation prevents abdominal aortic aneurysm by increasing Tregs via maintained mitochondrial homeostasis.

Abdominal aortic aneurysm (AAA) is a life-threatening cardiovascular disease with few effective treatments. Iron, an essential trace element, influences cellular functional states through multiple pathways and has been implicated in the pathogenesis and development of AAA. Previous studies have associated iron deficiency (ID) with the phenotypic switching of vascular smooth muscle cells by affecting mitochondrial and endoplasmic reticulum function. In this study, we demonstrate the protective role of appropriate iron supplementation in the progression of AAA. Our data further showed that iron

Navigating treatment dilemmas in severe paediatric IDA: A case report.

Iron deficiency anemia (IDA) is a leading cause of anemia worldwide, often managed with oral iron. Severe paediatric cases, however, raise uncertainty about when to use packed red blood cell (pRBC) transfusion versus intravenous (IV) iron. We report a 16-year-old male with autism spectrum disorder and severe nutritional IDA (hemoglobin 31 g/L) who presented with tachycardia and hypotension but no acute decompensation. Although transfusion was discussed, the patient declined, and he was managed with IV iron followed by oral supplementation. Hemoglobin recovery was brisk, rising by 13 g/L within

Intracellular iron homeostasis-regulated epigenetic reprogramming contributes to -7/del(7q) leukemia.

Chromosome copy number variations are poorly understood drivers of human malignancies. -7/del(7q) is common in acute myeloid leukemia, confers a poor prognosis, and is thought to harbor several tumor suppressors. Previously, we identified the histone methyltransferase KMT2C as a tumor suppressor in this region. Here, through a differentiation CRISPR screen in hematopoietic stem and progenitor cells, we find that the mitochondrial iron transporter ABCB8 is essential for their differentiation. ABCB8 deficiency accelerates leukemogenesis in vivo and disrupts iron homeostasis, reducing cytoplasmic

The Role of Iron in the Susceptibility of Neonatal Mice to Escherichia coli K1 Sepsis.

Sepsis from Escherichia coli expressing the K1 antigen is a leading cause of death in neonates. In a murine model, E. coli K1 grew rapidly in the peritoneal cavity of neonatal mice, causing fatal disease. In contrast, adult mice cleared the infection. Neonatal mice mounted a rapid and equivalent antimicrobial immune response compared to adult mice. Interestingly, peritoneal fluid from neonatal mice contained significantly more total iron than that of adult mice, which was sufficient to support enhanced E. coli growth. Transient iron overload in adult mice infected with E. coli resulted in 100%

Superoxide dependent iron release from ferritin in inflammatory diseases.

Convincing evidence is presented that oxygen free radicals are involved in the pathogenesis of rheumatoid arthritis (RA). Superoxide is produced by polymorphonuclear leucocytes (PMN) in synovial fluid and by macrophages in the synovial membrane. Tissue damage typical for free radical attack is detected in RA. No absolute deficiency of protective factors is found in RA compared to controls, but the available protection is insufficient to cope with all radicals formed. The toxicity of superoxide is increased by iron. It is doubtful whether a low molecular weight iron pool is present. Superoxide

Iron oxide-mediated enhancement of extracellular electron transfer and symbiosis in consortium of electroactive bacteria and microalgae for wastewater treatment.

This study explores the role of α-Fe₂O₃ in improving extracellular electron transfer (EET) and symbiotic interactions between electroactive Shewanella oneidensis MR-1, its gene-deficient mutants (ΔmtrC, ΔomcA, and ΔcymA), and microalgae (Chlorella vulgaris). The iron oxide facilitates the efficient transfer of electrons generated by MR-1 to microalgal photosystem via the pathway of CymA-MtrC-OmcA to α-Fe₂O₃. This process enhances the removals of TOC, TN, and NH₄⁺-N in the MR-1 bacterial-algal consortium by 9.99%, 12.32%, and 52.25% respectively via OmcA regulation while boosting phosphorus rem
