Voice outcomes after total thyroidectomy, partial thyroidectomy, or non-neck surgery using a prospective multifactorial assessment.

BACKGROUND: Voice alteration remains a significant complication of thyroid surgery. We present a comparison of voice outcomes between total thyroidectomy (TT), partial thyroidectomy (PT), and non-neck (NN) surgery using a multifactorial voice-outcomes classification tool. STUDY DESIGN: Patients with normal voice (n = 112) were enrolled between July 2004 and March 2009. The patients underwent TT (n = 54), PT (n = 35), or NN (n = 23) surgery under general endotracheal anesthesia as part of a prospective observational study involving serial multimodality voice evaluation preoperatively, and at 2

Organ preservation surgery for low- and intermediate-grade laryngeal chondrosarcomas: analysis of 16 cases.

OBJECTIVES/HYPOTHESIS: To demonstrate that endoscopic resection (ER), open partial laryngectomies, and cricotracheal resection and anastomosis (CTRA) achieve a good balance between oncologic radicality and organ preservation for laryngeal low-grade chondrosarcoma (LCS) and intermediate-grade chondrosarcoma (ICS). STUDY DESIGN: Retrospective series in an academic institution. METHODS: Between 2001 and 2013, we treated 13 cricoid, two thyroid, and one arytenoid LCS and ICS. Two cricoid and the only arytenoid LCS were managed by ER. Two thyroid ala LCS were submitted to laminectomy. Five ICS an

A randomised trial comparing the i-gel (TM) with the LMA Classic (TM) in children.

We performed a prospective, randomised trial comparing the i-gel(TM) with the LMA Classic(TM) in children undergoing general anaesthesia. Ninety-nine healthy patients were randomly assigned to either the i-gel or the LMA Classic. The outcomes measured were airway leak pressure, ease of insertion, time taken for insertion, fibreoptic examination and complications. Median (IQR [range]) time to successful device placement was shorter with the i-gel (17.0 (13.8-20.0 [10.0-20.0]) s) compared with the LMA Classic (21.0 (17.5-25.0 [15.0-70.0]) s, p = 0.002). There was no significant difference in oro

The physiologic impact of transnasal flexible endoscopy.

OBJECTIVES/HYPOTHESIS: Diagnostic transnasal flexible endoscopy (TNFE) is a commonly used office procedure in otolaryngology. Currently there is a paucity of data on the impact of TNFE on physiologic parameters. This is relevant with the advent of office-based endoscopic procedures. The goal of this study is to measure the impact of topical decongestion, anesthesia, and diagnostic TNFE on vital signs: systolic blood pressure (SBP) and diastolic blood pressure (DBP), heart rate (HR), and oxygenation (O(2) sat). STUDY DESIGN: Prospective case control study, in which the patient is his/her contr

Endoscopic laser thyroarytenoid myoneurectomy in patients with adductor spasmodic dysphonia: a pilot study on long-term outcome on voice quality.

OBJECTIVES: Adductor spasmodic dysphonia (ADSD) is a focal laryngeal dystonia, which compromises greatly the quality of life of the patients involved. It is a severe vocal disorder characterized by spasms of laryngeal muscles during speech, producing phonatory breaks, forced, strained and strangled voice. Its symptoms result from involuntary and intermittent contractions of thyroarytenoid muscle during speech, which causes vocal fold to strain, pressing each vocal fold against the other and increasing glottic resistance. Botulinum toxin injection remains the gold-standard treatment. However, a

Endoscopic mucosal suturing of vocal fold with placement of stent for the treatment of glottic stenoses.

BACKGROUND: Stenosis of the glottis can influence both phonation and respiration. The management of laryngeal web is difficult because of the tendency of readhesion to recur locally. METHODS: Twenty-five patients with web from the anterior glottis to the entire membranous portion of the vocal folds were included. All patients underwent endoscopic mucosal suturing or combined with placement of laryngeal silastic sheet stents. RESULTS: Eight patients underwent suture of the vocal fold alone. Seventeen patients underwent suture of the vocal fold combined with silastic sheet placement. The vocal

[Experiences with 'optical' biopsies of leukoplakia of the vocal folds].

BACKGROUND: It is often difficult to distinguish leucoplakia from carcinoma or chronic laryngitis. In this study, we examined if color texture optical biopsies are able to detect leukoplakia reliably and to distinguish this from normal vocal fold tissue. METHODS: 25 images from patients with a normal larynx and 25 images from patients with leucoplakia were analysed retrospectively. The images were recorded with a rigid 90 degrees -laryngoscope (Wolf Typ 4450.571) during a clinical setting and were recorded by a S-VHS-videorecorder. With a software program (InSegT), regions with leucoplakia, n

Detection of Vocal Fold Image Obstructions in High-Speed Videoendoscopy During Connected Speech in Adductor Spasmodic Dysphonia: A Convolutional Neural Networks Approach.

OBJECTIVE: Adductor spasmodic dysphonia (AdSD) is a neurogenic voice disorder, affecting the intrinsic laryngeal muscle control. AdSD leads to involuntary laryngeal spasms and only reveals during connected speech. Laryngeal high-speed videoendoscopy (HSV) coupled with a flexible fiberoptic endoscope provides a unique opportunity to study voice production and visualize the vocal fold vibrations in AdSD during speech. The goal of this study is to automatically detect instances during which the image of the vocal folds is optically obstructed in HSV recordings obtained during connected speech. M

Predictors of the need for an extracervical approach to intrathoracic goitre.

BACKGROUND: Sternotomy and lateral thoracotomy are required infrequently to remove an intrathoracic goitre (ITG). As few studies have explored the need for an extracervical approach (ECA), the aim of this study was to examine this in a large cohort of patients. METHODS: A prospective database of all patients who had surgery for ITG between 2004 and 2016 was interrogated. Patient demographics, preoperative characteristics and type of operation were analysed to identify factors associated with an ECA. RESULTS: Of 237 patients who had surgery for ITG, 29 (12·2 per cent) required an ECA. ITGs be

Are Vocal Alterations Caused by Smoking in Reinke's Edema in Women Entirely Reversible After Microsurgery and Smoking Cessation?

BACKGROUND: Reinke's edema is a benign lesion of the vocal folds that affects chronic smokers, especially women. The voice becomes hoarse and virilized, and the treatment is microsurgery. However, even after surgery and smoking cessation, many patients remain with a deep and hoarse voice. OBJECTIVES: The aim of the present study was to compare pre- and postoperative acoustic and perceptual-auditory vocal analyses of women with Reinke's edema and of women in the control group, who were non-smokers. METHODS: A total of 20 women with videolaryngoscopy diagnosis of Reinke's edema who underwent l…
