Hemochromatosis of unknown origin in a young adult dog.

Hemochromatosis is an uncommon condition in dogs. This report describes hemochromatosis of unknown origin in a dog breed not previously identified as predisposed to the condition. A 3-year-old spayed female German shepherd crossbred dog was presented to a local Saskatchewan veterinary clinic with weight loss, muscle wasting, chronic vomiting, diarrhea, and ascites of ~5 to 6 mo duration. The dog was treated symptomatically and was prescribed medication for potential liver failure. Due to poor response to treatment, the owners elected humane euthanasia. The dog was submitted for necropsy at Pra

Self-supervised representation learning reveals explainable physiological structure in high-dimensional magnetocardiography.

Artificial intelligence (AI) has shown strong performance in cardiology, but most approaches rely on sensing modalities whose physical limitations constrain available information. Magnetocardiography (MCG) records the cardiac magnetic field with less tissue distortion than surface potentials and may preserve higher-dimensional spatiotemporal electrophysiological structure. Here, we investigated whether combining MCG with self-supervised learning enables physiologically meaningful cardiac representations. We developed MCG2Vec, a contrastive encoder trained directly on raw 64-channel MCG recordi

Lost in Translation: Barriers in Psychiatric Care for Patients With Communication Impairments.

Effective communication is central to psychiatric evaluation and treatment. Patients with acquired communication impairments, such as post-stroke aphasia, are at increased risk for misdiagnosis, delayed care, and suboptimal treatment. We aim to better understand the unique needs of psychiatric patients with communication difficulties, common pitfalls in their care, and ways to improve clinical assessment, diagnosis, and treatment of their mental health conditions. We present the case of a 49-year-old woman with a history of major depressive disorder, generalized anxiety disorder, post-traumati

Thyroid Hormone Excess in Sudden Death: A Clinicopathological Spectrum in an Autopsy Study.

Thyrotoxicosis is associated with heightened adrenergic activity, arrhythmogenic susceptibility, and procoagulant alterations; yet, its contribution to sudden death outside overt thyroid storm remains incompletely understood. We retrospectively screened 975 consecutive autopsies and identified 7 cases with biochemical evidence of thyroid hormone excess defined by elevated free T3 and/or free T4 levels. Clinical background, medication exposure, thyroid-related biomarkers, including thyroglobulin, thyroid autoantibodies, and interleukin-6, together with detailed histopathological examination of

Efficacy of epineurial incision for trigeminal neuralgia without neurovascular compression: Intraneural decompression and morphological restoration.

BACKGROUND: Trigeminal neuralgia (TN) is most commonly caused by neurovascular compression; however, a subset of patients present without identifiable vascular contact, for whom the optimal surgical strategy remains controversial. METHODS: Between February 2022 and August 2025, 157 patients underwent microsurgical treatment for TN at our institution. This study represents a retrospective analysis of a prospectively maintained surgical database. Thirty patients without radiological or intraoperative evidence of neurovascular compression were treated with superficial epineurial incision. Pain o

A novel SEMA6B splice-site variant (c.1680-2A>G) causes incompletely penetrant epilepsy via diverse aberrant transcripts.

BACKGROUND: While truncating variants in the SEMA6B gene are an established cause of Progressive Myoclonus Epilepsy-1(EPM11), the pathogenic mechanisms of non-last-exon splicing variants, particularly those underlying the frequent yet elusive phenomenon of incomplete penetrance, remain a critical knowledge gap. Elucidating these mechanisms is essential for accurate molecular diagnosis and genetic counseling METHODS: We combined whole-exome sequencing in a proband, familial co-segregation analysis, and an in vitro minigene splicing assay in HEK-293T cells to characterize a novel candidate varia

Persistent Sciatic Artery as a Rare Cause of Sciatic Neuropathy: A Case Report.

BACKGROUND: Persistent Sciatic Artery (PSA) is a rare congenital vascular anomaly, affecting 0.05% of the population. It arises from the failure of regression of the embryonic sciatic artery, which normally recedes as the superficial femoral artery (SFA) becomes dominant in lower limb perfusion. In cases of persistence, the sciatic artery remains as a continuation of the internal iliac artery. PSA is classified based on its persistence and anatomical relationship with the SFA and is associated with complications such as limb ischemia, aneurysmal degeneration, and thromboembolism, which may cli

Getting your publication right - Part 2 - Structuring a research manuscript: A guide to writing a case report.

BACKGROUND: Case reports serve as a crucial medium for documenting rare presentations, novel diagnostic approaches, innovative treatment protocols, and unexpected outcomes encountered in routine practice. Due to a lack of structure and guidelines, published case reports suffered from variability and reliability. With the evolution of CARE (CAse REport) guidelines, medical reporting became more structured. OBJECTIVE: This paper aims to present a systematic and well-defined roadmap to facilitate the preparation of clinical dental case reports, structured on the framework of the CARE guidelines.

Revisiting the death of Louis XIV: a nephrological perspective.

BACKGROUND: Louis XIV died in 1715 at the age of 76 years. His death was attributed by contemporaneous accounts to dry gangrene secondary to peripheral arterial disease. Several clinical features described in these accounts may also be interpreted through alternative modern nephrological frameworks, including chronic kidney disease and calciphylaxis. METHODS: We performed a critical reappraisal of contemporaneous medical and lay descriptions of King's terminal illness, integrating these sources with contemporary knowledge of chronic kidney disease and calciphylaxis. The work followed a hypoth

Association between maternal autoimmune connective tissue disease and cardiac structure and function in the newborn.

IntroductionNewborns of mothers with autoimmune systemic connective tissue disease (CTD) have a higher incidence of major congenital heart defects (CHDs) compared to unexposed newborns. Less is known about the association between maternal CTD and less severe cardiac abnormalities in the newborn.MethodsWe analysed prospectively collected echocardiographic data from the Copenhagen Baby Heart Study (CBHS), comparing newborns exposed to maternal CTD with those who were not exposed. Maternal autoimmune CTD diagnoses were identified through the National Patient Register and validated by medical reco

Viral variant but not host factors associate with SARS-CoV-2 viral kinetics.

BACKGROUND: It is challenging to assess SARS-CoV-2 viral kinetics amidst viral variant evolution and changes in population-level immunity. However, understanding the relationship between host factors and viral replication deepens our understanding of viral fitness. METHODS: In CoVPN 5001, we enrolled N = 953 adults diagnosed with acute SARS-CoV-2 from July 2020 to July 2022 across 51 sites. We confirmed SARS-CoV-2 infection by RT-PCR and identified the variant via viral genome sequencing.
