Versatility of percutaneous pedicular screw fixation in metastatic spine tumor surgery: a prospective analysis.

BACKGROUND: Posterior percutaneous spinal fixation (PPSF) has evolved to address the problems associated with metastatic spinal disease (MSD). This study was designed to evaluate the feasibility and spectrum of application of PPSF in the management of MSD, highlighting its clinical advantages. METHODS: Twenty-seven consecutive patients with MSD treated with PPSF in our institution from January 2011 to June 2014 were studied. After a multidisciplinary assessment, all patients were considered for surgical intervention due to clinical presentation of either neural deficit, skeletal instability,

Does partial tear repair of adjacent tendons improve the outcome of supraspinatus tendonfull-thickness tear reinsertion?

BACKGROUND: Partial tearing of the infraspinatus and/or subscapularis tendon(s) is frequently associated with supraspinatus full-thickness tears. However, limited data regarding its influence on supraspinatus surgical repair is available. PURPOSE: Our aim was to assess the functional and anatomical outcomes of open repair of supraspinatus full-thickness tears combined with adjacent partial tearing, comparatively to a control. METHODS: We retrospectively identified 22 patients (22 shoulders) with a partial tear, most of them being a delamination tear, of the infraspinatus and/or subscapularis

[Old pseudoarthrosis with large bone defect of the radius treated with the Watson-Jones technique: a case report].

Large bone defects in the forearm pose a challenging reconstruction problem, especially when the condition has become chronic. The Watson-Jones technique is a relatively simple procedure that sacrifices pronation-supination, but allows the forearm to be used in everyday life. Here, we describe a unique case of one-bone forearm reconstruction for a patient who had been suffering from radius non-union for several years. This reconstruction technique provided the patient with acceptable function and appearance of the arm.

Changes in CSF flow after one-stage posterior vertebral column resection in scoliosis patients with syringomyelia and Chiari malformation type I.

OBJECT: Phase contrast-cine MRI (PC-cine MRI) studies in patients with syringomyelia and Chiari malformation Type I (CM-I) have demonstrated abnormal CSF flow across the foramen magnum, which can revert to normal after craniocervical decompression with syrinx shrinkage. In order to investigate the mechanisms leading to postoperative syringomyelia shrinkage, the authors studied the hydrodynamic changes of CSF flow in the craniocervical junction and spinal canal in patients with scoliosis associated with syringomyelia after one-stage deformity correction by posterior vertebral column resection.

Total spondylectomy for solitary bone plasmacytoma of the lumbar spine in a young woman: a case report and review of literature.

INTRODUCTION: Solitary bone plasmacytoma (SP) is a rare diagnosis for which the primary treatment is local radiotherapy. There is no established consensus suggesting a total spondylectomy in spinal SP. MATERIALS AND METHODS: We report the case of a 43-year-old woman with solitary plasmacytoma of the lumbar spine treated with complete vertebral resection. Radiographs, CT scan and MRI showed a single osteolytic lesion of the third lumbar vertebra. Further diagnostics following recommended algorithm for tumour screening were negative. Two times, biopsy showed no histological pathologies. Due to

[Radiocarpal dislocation (about nine cases)].

Radiocarpal dislocation is an uncommon entity in traumatolgy. The purpose of this study was to detail the pathogenesis of radiocarpal dislocation and describe its complications and treatment. Nine radiocarpal dislocations were reviewed retrospectively. Dorsal displacement was observed for seven dislocations, anterior displacement for two dislocations. All were associated with fractures of the radial styloid. Treatment was always surgical. At last follow-up (mean 3 years), the overall functional outcome was satisfactory. The Green and O'Brien (modified by Cooney) score was excellent for three p

Effects on bone union and prevention of tendon adhesion by new porous anti-adhesive poly L-lactide-co-ε-caprolactone membrane in a rabbit model.

To prevent adhesion between the tendon and the plate in hand surgery, a porous poly L-lactide-co-ε-caprolactone (P(LA/CL)) membrane was developed as a biologically absorbable anti-adhesion material. Our study aims to confirm the efficacy of this new P(LA/CL) membrane and its influences on the osteotomy site by performing a fundamental experiment assessing the possibility of clinical application. We prepared a rabbit model of tendon-plate adhesion, and evaluated the efficacy of the P(LA/CL) membrane to pullout tendon strength, to be scored in terms of macroscopic, pathological results. Morphology and histology of the collapsed lunate in advanced kienböck disease.

Morphological and histological changes of 24 collapsed lunates in advanced Kienböck disease were investigated. Articular cartilage of a collapsed lunate were consistently attenuated or disappeared. The proximal articular cartilage facing the radius is generally more affected than the distal articular cartilage facing the capitate. There was one major fracture in addition to multiple minor fractures in the collapsed lunate. Fracture patterns were classified into three types according to the directions of major fracture, namely horizontal type, dual oblique type, and vertical type. There were vi

Pediatric and adolescent shoulder instability.

Instability of the shoulder is a common issue faced by sports medicine providers caring for pediatric and adolescent patients. A thorough history and physical examination can help distinguish traumatic instability from multidirectional or voluntary instability. A systematic understanding of the relevant imaging characteristics and individual patient disease and goals can help guide initial treatment. Given the high risk of recurrent instability, young, active patients who seek to return to competitive contact sports should consider arthroscopic stabilization after a first-time instability even

Predictive value of MRI in decision making for disc surgery for sciatica.

OBJECT: In a randomized controlled trial comparing surgery and prolonged conservative treatment for sciatica of 6-12 weeks' duration, more than one-third of patients assigned to conservative treatment underwent surgery. The objective of the present study was to evaluate whether MRI at baseline could have predicted this delayed surgery. METHODS: Independently evaluated qualitative and quantitative MRI findings were compared between those patients who did and those who did not undergo surgery during follow-up in the conservative care group. In addition, area under the receiver operating charact

Risk management in posterior spinal endoscopic surgery in lumbar diseases.

BACKGROUND: Minimally invasive posterior spinal endoscopic surgery has increased in popularity. However, a steep learning curve may result in a high frequency of complications. Additionally, device failure may occur during routine use. We retrospectively investigated the incidence of such safety issues in a population of patients undergoing endoscopic spinal surgery at our institution. PATIENTS/MATERIALS AND METHODS: A total of 611 cases were included. Underlying diseases that required endoscopic surgery were lumbar disc herniation in 382 patients, lumbar spinal stenosis in 123 patients, lumb

Intermediate-term results following first metatarsal cheilectomy.

Hallux rigidus is a term describing degenerative joint disease (DJD) to the first metatarsal phalangeal joint (MTPJ). It is the most common DJD encountered in the foot and is the second most common pathology of the great toe behind hallux valgus. The goal of a cheilectomy is to relieve pain and increase MTPJ motion. Critical evaluation of the cheilectomy must include longevity of desired results. The primary goal of this study was to determine how long a cheilectomy can be expected to last before an arthrodesis or joint destructive procedure is performed, if ever. We examined 189 cheilectomies

Explaining the variability improvements in gait quality as a result of single event multi-level surgery in cerebral palsy.

PURPOSE: This is a study of all children with spastic diplegic cerebral palsy (Gross Motor Classification System levels II and III) who had single event multi-level surgery (SEMLS) at a single tertiary referral hospital between 1995 and 2008 to identify factors predicting improvement in gait quality as quantified by the gait profile score (GPS). 9 factors (5 dichotomous and 4 continuous, including preoperative GPS) that might be expected to predict outcomes were identified and univariate and multivariable analysis used to explore how these affected outcomes. SCOPE: Data from 121 children were

Iatrogenic nerve injuries during shoulder surgery.

INTRODUCTION: The current literature indicates that neurologic injuries during shoulder surgery occur infrequently and result in little if any morbidity. The purpose of this study was to review one institution's experience treating patients with iatrogenic nerve injuries after shoulder surgery. METHODS: A retrospective review of the records of patients evaluated in a brachial plexus specialty clinic from 2000 to 2010 identified twenty-six patients with iatrogenic nerve injury secondary to shoulder surgery. The records were reviewed to determine the operative procedure, time to presentation, f

Meningococcemia: the pediatric orthopedic sequelae.

Meningococcal disease affects as many as 3,000 people in the United States per year, with the highest incidence in children younger than two years of age and two-thirds of cases occurring in children younger than five years of age. Children who survive meningococcemia face quality-of-life issues that result from limb deficiencies. Consultation with an experienced pediatric orthopedic surgeon in the early stages of the illness is vital for planning surgical approaches for amputation of the resulting necrotic tissue and for minimizing eventual tissue loss. Early surgical intervention is rarely i

A comparison of long-term postconcussive symptoms between university students with and without a history of mild traumatic brain injury or orthopedic injury.

Mild traumatic brain injury (mild TBI) is often associated with postconcussive symptoms such as headache, memory problems, and irritability. However, high rates of similar symptoms in groups without a history of TBI raise questions about the clinical validity of the postconcussive syndrome. This study was conducted to address these issues through systematic examination of symptoms reported by those with and without a history of mild TBI or orthopedic injury. Responses to the Postconcussion Syndrome Checklist (PCSC), demographic information, and medical history were collected via online questio
