Palliative Medicine Point of Care UltraSound in Cancer Patients in a Specialist Palliative Medicine Outpatient Department: A Retrospective Analysis of Diagnostic and Procedural Impact.

OBJECTIVES: This retrospective study aimed to evaluate the utilisation and clinical impact of palliative medicine POCUS (PM-POCUS) in a specialist palliative care OPD, in a tertiary cancer centre in India. Patients with advanced cancer frequently present to palliative care outpatient departments (OPDs) with symptoms such as dyspnoea and abdominal distension, often due to pleural effusion, ascites or deep vein thrombosis. Timely diagnosis of these conditions is critical for symptom relief, yet access to imaging is frequently delayed, particularly in resource-limited settings. Point-of-care ultr

Perceived Experience of the clozapine Treatment Protocol: A Qualitative Study of Reports by Brazilian Patients With Schizophrenia Treated in a University Outpatient Service Specializing in Psychiatry.

INTRODUCTION: Understanding the meanings people attribute to observed events and lived experiences is to unveil their symbolic universe, thus allowing us to help them better manage their emotions, reactions, and relationships, particularly in clinical settings. Patients with schizophrenia resistant to treatment with first- and second-generation antipsychotics respond well to clozapine; however, this medication presents agranulocytosis as an important side effect. Consequently, the medication protocol requires frequent blood draws to monitor any effects on blood cells. Investigating patients' p

Quantifying age-related disparities in outpatient psychotherapy utilization: a representation quotient analysis of routine data from 29 university clinics in Germany.

BACKGROUND: Although mental disorders are highly prevalent among older adults, evidence suggests that they underutilize psychotherapy. However, formal estimates of their actual representation in routine clinical settings are scarce. This study applied a representation quotient approach to identify and quantify age-related disparities in outpatient psychotherapy utilization in Germany. METHODS: We analyzed data from 13,635 adult patients who initiated cognitive-behavioral therapy between 2018 and 2023 at 29 university outpatient clinics. Using a representation quotient approach, we compared th

Shifting antiretroviral drug distribution from Outpatient Clinics to Hospital Pharmacies in Italy. Evolution of ART delivery methods and impact on treatment adherence.

OBJECTIVES: This retrospective observational study assesses adherence to antiretroviral therapy before and after the transition to pharmacy-based treatment dispensation. METHODS: Two 14-month periods were identified: before (P1) and after (P2) the transition to pharmacy-based provision of antiretrovirals. P2 also coincided with the SARS-CoV-2 pandemic. Adherence was assessed using a mathematical index to estimate the delay in refilling medication (delay index Di). RESULTS: 1,266 people with HIV were included. Higher values of Di were found to be associated with P2 (ß 0.17, 95%CI 0.04-0.07),

Stratification of Outpatient Physical Therapy Following Total Knee Arthroplasty: Knee Arthroplasty Physical Therapy Pathways (KAPPA) Nonrandomized Controlled Trial.

BACKGROUND: Outpatient physical therapy following total knee arthroplasty (TKA) is often considered crucial for an effective recovery. However, recent evidence suggests that a self-directed pathway may yield similar benefits to supervised care. Despite this, there appear to be no established criteria to determine who can successfully self-direct their rehabilitation versus those who would benefit from outpatient physical therapy. This study aimed to determine if early postoperative criteria can stratify TKA patients into a self-directed or supervised physical therapy pathway without compromisi

Phase angle and body composition in long-term type 1 diabetes in adults: a comparative study in a Brazilian public reference outpatient clinic.

INTRODUCTION: Type 1 Diabetes Mellitus (DM1) affects a small percentage of the population. Nevertheless, its prevalence is currently growing with alarming data on uncontrolled cases. The importance of body composition and Phase Angle (PA), assessed by Bioelectrical Impedance (BIA), in long- term DM1 patients lies in the fact that alterations in cellular integrity and body compartments may affect risk profiles and metabolic control. The objective of this study was to compare PA and body composition parameters between adults with DM1 and healthy controls. METHODS: A comparative study was carrie
