Right or Left Primary Site of Colorectal Cancer: Outcomes From the Molecular Analysis of the AGITG MAX Trial.

BACKGROUND: For metastatic colorectal cancer, previous reports have described differences in biology and outcome, including response to biologic agents, based on whether the primary tumor is right- or left-sided. We explored the molecular markers from the AGITG MAX trial. PATIENTS AND METHODS: The AGITG MAX trial was a randomized study comparing capecitabine versus capecitabine + bevacizumab versus capecitabine + bevacizumab + mitomycin C as first-line therapy in advanced colorectal cancer. Patients were classified as having right-sided (caecum to transverse colon) or left-sided (descending c

Bilateral cornu ammonis 3-dominant mechanism of visual working memory impairment in right temporal lobe epilepsy and hippocampal sclerosis.

Hippocampal sclerosis in temporal lobe epilepsy is linked to cognitive impairment. This study investigates its exacerbating role in visual working memory deficits in right temporal lobe epilepsy and the involvement of contralateral hippocampal subregions. We enrolled 103 patients with right temporal lobe epilepsy (57 with right hippocampal sclerosis and 46 without hippocampal sclerosis) and 36 healthy controls who underwent high-resolution MRI and neuropsychological assessment. Patients with hippocampal sclerosis showed more severe visual working memory impairment, widespread grey matter atrop

An unexpected finding of apparent bilateral aldosterone suppression after superselective adrenal venous sampling: a case report.

INTRODUCTION: In adrenal venous sampling (AVS) for patients with primary aldosteronism (PA), apparent bilateral aldosterone suppression (ABAS) is occasionally experienced, but the underlying causes remain poorly understood. CASE PRESENTATION: A 40-year-old Chinese man with a right adrenal adenoma presented with hypertension and refractory hypokalaemia. Initial AVS revealed no evidence of unilateral aldosterone excess and demonstrated ABAS. Six months later, repeat AVS was performed, during which two distinct venographic patterns-a 'triangular pattern' and a 'delta pattern'-were observed in th

Dysphagia lusoria caused by aberrant right subclavian artery associated with truncus bicaroticus in an 8-month-old girl. Case report and review of literature.

Dysphagia lusoria is a rare pediatric condition caused by extrinsic compression of the esophagus by an abnormal subclavian artery. The most common congenital abnormality in aortic arch development is an aberrant right subclavian artery. The retroesophageal right subclavian artery is typically symptomatic in 10-33% of cases. The patient, an 8-month-old girl with a history of early dysphagia and stridor, was diagnosed with an abnormal right subclavian artery. She was admitted to the pneumology service multiple times due to stridor, vomiting, and failure to thrive. During hospitalization at the g

A case report: renal cystic tumoural calcinosis with ossification and bone marrow formation.

BACKGROUND: Tumoural calcinosis (TC) is a rare disorder characterized by nonneoplastic amorphous calcium deposition that tends to occur in soft tissues around the large joint. Here, we report a case of cystic TC with ossification and bone marrow formation in the kidney. CASE PRESENTATION: We report a 63-year-old woman who presented with a complaint of intermittent right lumbar pain for 2 months. Computed tomography (CT) revealed a large cystic lesion on the lateral side of the right kidney, with a circular calcified wall around the lesion, which compressed, deformed and displaced the right ki

Thoracic endometriosis with a long delay in diagnosis.

This paper describes a case of thoracic endometriosis in 36-year-old woman with a long delay in diagnosis. At the admission in the hospital, the patient had a medical history of persistent dysmenorrhea since the age of 13, infertility and an episode of total right pneumothorax two months ago successfully resolved by minimum pleurotomy of the right hemitorax. She came with moderate pain on right hemithorax and dyspnea, which occurred on the first day of menstruation but she did not have any other respiratory symptoms such as hemoptysis, cough. Radiological imaging (chest radiography and compute

Endovascular repair of thoracic aortic aneurysm associated with right-sided aortic arch: report of two cases.

Right-sided aortic arch (RAA) is a rare congenital disorder. We describe herein two cases of thoracic aortic aneurysm with a right aortic arch and right-sided descending aorta treated with thoracic endovascular aortic repair (TEVAR). In one case, a 70-year-old man with Edwards type 1 RAA underwent TEVAR using a Relay stent-graft (Bolton Medical, Barcelona, Spain). In another case, a 72-year-old woman with Edwards type 3 RAA underwent TEVAR using a Kawasumi Najuta stent-graft (Kawasumi Laboratories, Inc., Tokyo, Japan) with the "buffalo horn chimney technique", our original method for left subc

[A case of lateral ventricle metastasis from gastric carcinoma].

Metastatic brain tumors of the lateral ventricles are rare. Approximately half of the reported cases have originated from renal carcinoma;to the best of our knowledge, only 1 case originating from gastric carcinoma has been reported. A 57-year-old man presented with dull headache, left hemiparesis, and constructional apraxia of the right upper extremity. He had undergone gastrectomy for gastric carcinoma 8 years previously. Magnetic resonance imaging revealed an enhanced tumor extending from the right caudate nucleus to the right lateral ventricle with peritumoral edema. Complete resection of

Cardioembolic stroke: Protective effect of a severe internal carotid artery stenosis in a patient with cardiac embolism.

Cardioembolic stroke is generally caused by intracranial artery occlusion. Clots may be identified in the intracranial vessels by means of conventional neuroimaging in the acute phase. High-resolution ultrasonography may show some features suggestive of cardiac emboli when occluding extracranial carotid arteries. We describe a patient with cardioembolic ischemic stroke in the right hemisphere in whom a left internal carotid artery stenosis paradoxically protected the ipsilateral hemisphere from distal intracranial embolism. The patient also presented multiple acute ischemic embolic lesions in

Cerebellar infarction originating from vertebral artery stenosis caused by a hypertrophied uncovertebral joint.

We report a case of cerebellar infarction originating from vertebral artery stenosis caused by a hypertrophied uncovertebral joint. A 38-year-old man presented with sudden onset of headache, dizziness, and dysarthria. The magnetic resonance imaging scan of the brain revealed acute infarction in the right cerebellar hemisphere in the territory of the posterior inferior cerebellar artery (PICA) and superior cerebellar artery (SCA). Magnetic resonance, 3-dimensional computed tomographic, and conventional angiography revealed severe right vertebral artery stenosis by extrinsic compression of the h

Abdominal-pelvic actinomycosis mimicking malignant neoplasm.

Abdominal-pelvic actinomycosis is often mistaken for other conditions, presenting a preoperative diagnostic challenge. In a 46-year-old female, computed tomography showed an abdominal-pelvic retroperitoneal mass extending from the lower pole of the right kidney to the lower pelvis. The patient had a 3-year history of intrauterine device. The mass appeared to involve the ascending colon, cecum, distal ileum, right Fallopian tube and ovary, and ureter anteriorly and the psoas muscle posteriorly. The resection of retroperitoneal mass, distal ileum appendicectomy, right hemicolectomy, and right sa

[A 16-year-old patient with dysphagia].

A 16-year-old man with an 8-year history of progressive dysphagia was referred to hospital. There was no specific finding in his family history. Physical examination was unremarkable. Complete blood count, serum electrolytes, and liver and kidney function tests were normal. Barium swallow revealed an extrinsic impression of the upper esophagus posteriorly. Magnetic resonance angiography demonstrated an aberrant origin of the right subclavian artery, leaving the aorta below the left subclavian artery. The artery had a retroesophageal course, causing the esophageal narrowing. Due to the persiste

Surgical repair of infundibular ventricular septal defect and aortic regurgitation.

Interpretation of the anatomy of the defect and the pathophysiology has guided the surgical technique and indications for infundibular ventricular septal defect VSD. Infundibular ventricular septal defects are located in the infundibular septum, between the two commissures of the right coronary cusp. The defect is associated with an anomaly of the right sinus of Valsalva where the transition to cusp tissue occurs higher than normally. There is development of fibrous adherences between the ventricular surface of the right coronary cusp and the crest of the septum. This reduces the height of the

A case of adult moyamoya disease showing progressive angiopathy on cerebral angiography.

In moyamoya disease, progression of carotid occlusive lesion in an adult patient is very rare. We report a case of adult moyamoya disease with acute angiographical stage progression and hemodynamic deterioration. A 56-year-old female complaining of left motor weakness visited our department. On MRI, infarct lesion was found in the right white matter. On cerebral angiography, occlusive lesions in the bilateral internal carotid arterial siphons and moyamoya vessels were found. The right side was stage V and the left side was stage III. On IMP-SPECT, decreased cerebral hemodynamic reserve of the

Lessons to be learned: a case study approach--a case of temporal arteritis.

A 71-year-old male presented with a history of sudden partial visual loss in the right eye with an inferior visual field defect over the past 3-4 days. He had no history of headache or of facial pain. Clinical examination confirmed that vision on the right side was reduced to 6/18 and on the left to 6/12. The right eye showed a relative afferent pupillary defect. There was no other abnormality of the anterior segment of either eye. The right retina showed a pale swollen optic disc and a provisional diagnosis of anterior ischaemic optic neuropathy (AION) was made. An urgent erythrocyte sediment

[A case of coronary arterial fistula originating from both coronary artery and draining to the right atrium].

We encountered a case with bilateral fistulas of coronary arteries into the right atrium, a rare cardiac anomaly. The case was a 17-year-old woman, who visited our hospital at the age of 11 because of fever. At that time, the patient was diagnosed as having a left coronary artery-right atrial fistula through cardiac catheterization (CAG). When the patient developed staphylococcus infected endocarditis at the age of 16, a thick fistula of the coronary artery, directly running from the deformed left coronary arterial sinus, a fistula of the left circumflex branch, and also a fistula of the right

[A case of dural arteriovenous fistula in the transverse and sigmoid sinus which developed over five years].

We report a case of dural arteriovenous fistula (dAVF) of the transverse and sigmoid sinus which developed over five years. The patient was a 64-year-old man, who had a history of congestive heart failure and transient ischemic attack (TIA). No abnormal lesions were detected in the cerebral angiograms five years prior to the time the etiology of his TIA was investigated. He suddenly suffered from left hemiparesis and the CT scan showed right parietal subcortical hemorrhage. Right carotid angiogram demonstrated dAVF of the transverse and sigmoid sinus supplied by the right occipital and ascendi

[Thoracic outlet syndrome due to first rib anomaly: a case report].

Compared to cervical rib, first rib anomaly is less familiar to us as a cause of thoracic outlet syndrome. We report a case of thoracic outlet syndrome due to first rib anomaly. A 50-year-old man was admitted to our hospital because of right arm pain when his arm was raised up. Chest X-ray showed an abnormal right first rib which made a J-shaped curve and was attached to the second rib. A palpable mass was noted in the supraclavicular region. Angiography revealed complete occlusion of the right subclavian artery in the Allen test position. During conservative therapy for two months, the sympto

Temporal resolution deficits in patients with refractory complex partial seizures and mesial temporal sclerosis (MTS).

We studied the temporal resolution ability in patients with refractory complex partial seizures and mesial temporal sclerosis (MTS) using Gaps-In-Noise (GIN) test in a prospective cross-sectional study. Thirteen patients with right MTS (age: 31±7.67 years; M:F=8:5) and 13 patients with left MTS (age: 25.76±8.26 years; M:F=9:4) having normal hearing and mini-mental state examination (MMSE) score of >23/30 were recruited. Fifty healthy volunteers (26.3±5.17 years; M:F=28:22) formed the control group. Gaps-In-Noise test demonstrated impaired temporal resolution: 69.2% of patients with right MTS (

An easy approach to ultrasonographic volumetry of the thyroid.

To establish an easy way to perform volumetry of the thyroid gland using ultrasonography, we evaluated the accuracy of the products of the depth and width of the right thyroid lobe as indices of thyroid volume. The depth and width of both thyroid lobes were measured using ultrasonography before surgery in 193 patients with Graves' disease. The products were compared with the weight of the thyroid obtained from operative records. We also evaluated the depth and width of the right thyroid lobe in 312 subjects who presented without any thyroid disease. The products of depth and width of the right
