[Globoid cell leukodystrophy of adult. A first case in Poland].

Krabbe disease (globoid cell leukodystrophy) is a progressive, autosomal recessive disorder affecting peripheral and central nervous system. This disease is associated with mutation in GALC gene and its locus has been mapped to chromosome 14q31. GALC gene codes lysosomal hydrolytic enzyme: galactocerebroside β-galactosidase (galactosylceramidase) which is crucial for degradation of galactolipids, mostly galactosylceramide and galactosylsphingosine (psychosine). The disease may be subdivided into four types: infantile form with onset within the first six months, child form presenting between 6

New immunotherapies for multiple sclerosis (MS): A comprehensive review.

Multiple sclerosis (MS) is the leading cause of non-traumatic disability in young adults, characterized by autoimmune demyelination and neurodegeneration. While high-efficacy therapies have transformed relapsing MS management, disability progression remains an unmet need. Emerging evidence implicates compartmentalized central nervous system (CNS) inflammation, including microglial activation, in disease pathogenesis, necessitating novel immunotherapeutic strategies that target both peripheral and CNS-resident immune cells. This review synthesizes recent preclinical and clinical data on emergin

Acid-sensing ion channel 1a modulation of apoptosis in acidosis-related diseases: implications for therapeutic intervention.

Acid-sensing ion channel 1a (ASIC1a), a prominent member of the acid-sensing ion channel (ASIC) superfamily activated by extracellular protons, is ubiquitously expressed throughout the human body, including the nervous system and peripheral tissues. Excessive accumulation of Ca2+ ions via ASIC1a activation may occur in the acidified microenvironment of blood or local tissues. ASIC1a-mediated Ca2+‑induced apoptosis has been implicated in numerous pathologies, including neurological disorders, cancer, and rheumatoid arthritis. This review summarizes the role of ASIC1a in the modulation of apopto

Two novel pathogenic mutations of GAN gene identified in a chinese family with giant axonal neuropathy: a case report.

BACKGROUND: Giant axonal neuropathy (GAN) is a rare autosomal recessive, early-onset and fatal neurodegenerative disorder which develops into severe impairments in both peripheral and central nervous systems. METHODS AND RESULTS: Trio-WES analysis was used to detect genetic mutations associated with disorders, and Sanger sequencing was used to confirm the mutations in the patient. We identified two novel variations in GAN gene (c.809G > T(p.G270V); c.1182 C > A(p.Y394X)) within a Chinese family. Meanwhile, we propose a hypothesis of the molecular mechanism leading to GAN. CONCLUSIONS: This s

Flaccid paralysis in neuromyelitis optica: An atypical presentation with possible involvement of the peripheral nervous system.

BACKGROUND: Neuromyelitis optica spectrum disorders (NMOSD) typically lead to spastic paraparesis and spare the peripheral nervous system (PNS). CASE REPORT: Here, we describe an anti-aquaporin-4-seropositive NMOSD patient suffering from acute transverse myelitis with painful, flaccid paralysis and incontinence of urine and feces. Due to the involvement of the PNS as indicated by electrodiagnostic examination, we verified the expression of aquaporin-4-channels on the proximal dorsal spinal radix of rats by staining rat tissue with human NMOSD serum. CONCLUSION: This case suggests a manifesta

Role of inflammation and apoptosis in multiple sclerosis: Comparative analysis between the periphery and the central nervous system.

Multiple sclerosis (MS) is a complex, multifactorial disease associated with damage to the axonal myelin sheaths and neuronal degeneration. The pathognomonic event in MS is oligodendrocyte loss accompanied by axonal damage, blood-brain barrier leakage, inflammation and infiltration of immune cells. The etiopathogenesis of MS is far from being elucidated. However, increasing evidence suggests that the inflammatory and apoptotic responses, occurring in patients either at the peripheral level or the central nervous system (CNS), can play a role. In this review, we give a comprehensive picture of

[A seasonal neurologic disease: paralysis following tick-bites (author's transl)].

Seasonal inflammatory neurologic disorders following tick-bites have been reported more and more often during the last few years. They usually consist in a peripheral neurologic syndrome. However, involvement of the central nervous system is not uncommon. Two new cases seen at the Val-de-Grâce Hospital are reported here. The author reviews the circumstances in which the disease occurs and it's clinical features. The course is variable and may result in serious sequelae. Pathogenesis remains unknown. Viral, toxinic, and immuno-allergic factors have been incriminated. Practitioners should consid

Imaging of inflammation in the peripheral and central nervous system by magnetic resonance imaging.

Inflammation plays a central role in the pathophysiology of numerous disorders of the nervous system, but is also pivotal for repair processes like peripheral nerve regeneration. In this review we summarize recent advances in cellular magnetic resonance imaging (MRI) while nuclear imaging methods to visualize neuroinflammation are covered by Wunder et al. [Wunder A, Klohs J, Dirnagl U (2009) Non-invasive imaging of central nervous system inflammation with nuclear and optical imaging. Neuroscience, in press]. Over-excitation of tyrosine hydroxylase neurons in the paraventricular nucleus of the hypothalamus drives a neuroimmune positive feedback loop in rheumatoid arthritis.

BACKGROUND: Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by persistent inflammation and pain, leading to a significant decline in patients' quality of life. Recent studies suggest that the central nervous system (CNS) may also contribute to disease progression by modulating autonomic and immune functions. However, the precise mechanisms underlying CNS involvement remain unclear. OBJECTIVE: This study aims to investigate the alterations in the functional state of tyrosine hydroxylase (TH) within the paraventricular nucleus (PVN) in RA and to explore their potential r

A murine model of Lyme disease demonstrates that Borrelia burgdorferi colonizes the dura mater and induces inflammation in the central nervous system.

Lyme disease, which is caused by infection with Borrelia burgdorferi and related species, can lead to inflammatory pathologies affecting the joints, heart, and nervous systems including the central nervous system (CNS). Inbred laboratory mice have been used to define the kinetics of B. burgdorferi infection and host immune responses in joints and heart, however similar studies are lacking in the CNS of these animals. A tractable animal model for investigating host-Borrelia interactions in the CNS is key to understanding the mechanisms of CNS pathogenesis. Therefore, we characterized the kineti

Role of the immune system in neuropathic pain.

Background Acute pain is a warning mechanism that exists to prevent tissue damage, however pain can outlast its protective purpose and persist beyond injury, becoming chronic. Chronic Pain is maladaptive and needs addressing as available medicines are only partially effective and cause severe side effects. There are profound differences between acute and chronic pain. Dramatic changes occur in both peripheral and central pathways resulting in the pain system being sensitised, thereby leading to exaggerated responses to noxious stimuli (hyperalgesia) and responses to non-noxious stimuli (allody

Sarcoidosis and the nervous system.

Neurologic involvement occurs in 5 per cent of patients with sarcoidosis and is a presenting symptom in about half of those affected. The neurologic symptoms are due to a granulomatous process that mirrors that seen in the systemic form. Its diagnosis is usually established by the demonstration of sarcoidosis in other systems and supported by clinical, radiographic, and laboratory evidence. Any and all portions of the nervous system can be involved in either an acute or a chronic fashion. Early acute presentations tend to have excellent outcomes and the overall prognosis for the disease is goo

Circulating Natural Killer Cells in Patients With Type 2 Diabetes and Peripheral Neuropathy Are Altered in Their Frequency and Exhibit Impaired Functions and Angiogenic Activities.

Patients with type 2 diabetes mellitus (T2DM) and critical limb-threatening ischemia (CLTI), a severe vascular complication associated with amputation and high mortality, exhibit chronic inflammation and endothelial dysfunction. We previously reported an increased frequency of natural killer (NK) cells in advanced CLTI; however, whether NK cell alterations are already present in patients with peripheral neuropathy, which often precedes diabetic foot lesions with or without distal macroangiopathy, remains unclear. We conducted an observational pilot study to assess circulating NK cell frequency

Physiotherapy in the primary care oncology team.

As the incidence of cancer rises, medical advancements have made it possible to have increased survival rates among cancer patients. However, many experience persistent treatment-related side effects that negatively affect their quality of life. At the first international Oncology Physiotherapy Conference in 2018, it was agreed that undergraduate physiotherapy training provides a strong foundation for oncology rehabilitation. However, management of specific side effects such as cancer-related fatigue (CRF), chemotherapy-induced peripheral neuropathy (CIPN) and lymphoedema, necessitates additio

Human iPSC-Derived Dorsal Root Ganglion Organoid Modeling of Chemotherapy-Induced Peripheral Neuropathy.

Chemotherapy-induced peripheral neuropathy (CIPN) is a dose-limiting toxicity affecting 30-40% of patients treated with neurotoxic chemotherapy. Sensory symptoms arise from injury to dorsal root ganglion (DRG) neurons and their axons; yet, the underlying mechanisms remain incompletely understood. While human induced pluripotent stem cell (iPSC)-derived sensory neuron (iSN) monolayers have provided mechanistic insight, they lack the three-dimensional architecture and cellular heterogeneity of native DRG tissue. Here, we generated human iPSC-derived DRG organoids (iDRGOs) containing mixed neuron

Monoclonal Gammopathy of Neurological Significance: Toward an Integrated Hematologic-Neurologic Perspective-A Single-Center Retrospective Study.

Monoclonal gammopathies of clinical significance (MGCSs) are entities in which a small hematological clone produces a monoclonal immunoglobulin capable of causing organ damage. Neurological involvement remains difficult to diagnose and treat, especially in the context of incidental monoclonal gammopathy of undetermined significance (MGUS)-peripheral neuropathy (PN) associations. We conducted a single-center retrospective study at Fundeni Clinical Institute, Bucharest, from January 2015 to December 2025. The reference population included 300 patients with MGUS. The diagnosis of MGNS was establi

TRPA1 siRNA-Loaded Nanoformulation Ameliorates Chemotherapy-Induced Peripheral Neuropathy.

Small interfering RNA (siRNA) has emerged as a cutting-edge therapeutic strategy, with significant promise for addressing peripheral neuropathies. Despite its immense revolutionary therapeutic potential, the application and sustained release of siRNA for the treatment of chronic pain remain an arduous scientific challenge. This study introduces a novel cationic lipid-based siRNA formulation specifically targeting transient receptor potential ankyrin 1 (TRPA1) for the systemic treatment of chemotherapy-induced neuropathic pain (CINP), a condition with no US-FDA-approved therapeutic options. Diagnostic Potential of the Risk Factors Associated With Peripheral Neuropathy in Multiple Myeloma: Evidence From Logistic Regression Analysis.

Aims/Background Peripheral neuropathy (PN) is a common and debilitating complication in patients with multiple myeloma (MM), which results from both disease-related mechanisms and treatment-induced neurotoxicity. Despite its clinical significance, comprehensive investigations assessing PN risk in MM, along with examining demographic, clinical, nutritional, and inflammatory factors, remain limited. Therefore, this study aimed to investigate independent risk predictors associated with PN in MM patients using univariate and multivariate logistic regression analysis, thereby enhancing clinical ris
