Role of individualized intervention(s) on quality of life and adherence to adjuvant endocrine therapy in premenopausal women with early-stage breast cancer: MyCHOICE study.

BACKGROUND: Premenopausal women receiving combination endocrine therapy often experience substantial declines in quality of life (QOL), cognitive functioning, and treatment adherence. The MyChoice study evaluated whether individualized behavioral and complementary interventions could help maintain or improve QOL and treatment adherence in women with early-stage hormone receptor-positive breast cancer undergoing aromatase inhibitor- or tamoxifen-based therapy with ovarian suppression. METHODS: In this prospective multicenter phase II study, premenopausal women with newly diagnosed hormone rece

Pharmacologic and toxicologic confounders in brain death determination: a multidisciplinary guide.

BACKGROUND: The assessment of comatose ICU patients presents several challenges with respect to the etiology, depth and ultimate outcome. The acceptance in 1959 of the worst-outcome scenarios of coma, i.e. brain death, and the publication of the Harvard Brain Death Criteria in 1968, were key developments in the management of irreversible coma. Pharmacologic confounders often complicate coma assessments, including brain-death determination. Moreover, associated clinical factors during coma, such as organ failure, hypothermia, prolonged continuous infusions, intoxication, and extreme obesity oft

General practice organisational models and heart failure medication adherence: multi-level evidence from a regional cohort in Emilia-Romagna, Italy.

BACKGROUND: Medication adherence is essential for improving heart failure outcomes yet remains suboptimal. Organisational models in general practice-such as group practices and Community Health Centres-have long been promoted as a means to strengthen primary care and are currently undergoing national reform. However, their impact on adherence remains unclear. This study investigated whether general practice organisational arrangements were associated with adherence to therapies recommended by clinical guidelines for heart failure. METHODS: We conducted a retrospective cohort study using linke

Prevalence of undiagnosed hypertension and risk assessment using a validated survey in community-based screening in Amman, Jordan.

BACKGROUND AND OBJECTIVES: The rising incidence of hypertension in Jordan has contributed to high rates of associated morbidity/mortality and increased societal costs. Hypertension affects approximately 30% of Jordan's adult population. This study assessed the prevalence of undiagnosed hypertension through blood pressure measurement and evaluated future hypertension risk using a validated survey in a community setting. The survey instrument incorporates demographic and clinical factors to predict the likelihood of hypertension development within a nine-year timeframe. METHODS: We conducted a

The duration of phentermine therapy in the private healthcare sector of South Africa: A retrospective analysis.

BACKGROUND: Phentermine is an amphetamine derivative and is globally the most used oral appetite suppressant for obesity. Its long-term (LT) use (a period >12 consecutive weeks) may be associated with amphetamine-like adverse effects. The US Food and Drug Administration has restricted its use to short term (ST) only (not longer than 12 consecutive weeks), specifically stated in the patient information leaflet. This serves as a prescribing guideline that is provided to assist both general and specialist practitioners in drug prescribing that is rational and safe, and to maximise patient outcome

Survival and response outcomes in patients with recurrent/metastatic head and neck cancer treated with opioids and statins: an exploratory pilot study.

Anti-PD-1 immune checkpoint blockade (ICB) is a common first-line treatment for patients with recurrent/metastatic head and neck cancer (HNC), but many patients fail to respond. There is a growing interest in the impact of concomitant medications on responses to ICB, with studies in other tumor types showing poor responses in patients taking opioid pain medications and modest improvements in response to ICB in patients taking statins for hyperlipidemia. Our previous preclinical and retrospective clinical data suggest that use of specific statin drugs is associated with increased responses to I

Buprenorphine dosing patterns and treatment outcomes for patients with opioid use disorder insured by Medicaid in Philadelphia.

INTRODUCTION: Opioid use disorder (OUD) remains a significant public health challenge, exacerbated by the rise of potent, synthetic opioids like fentanyl in the illicit drug supply. Buprenorphine, a partial opioid agonist, is an evidence-based treatment for OUD. While evidence suggests that higher buprenorphine doses (>16 mg/day) improve treatment retention and reduce overdose, many providers hesitate to prescribe these doses. METHODS: This study analyzed buprenorphine dosing patterns and treatment outcomes among Medicaid-insured individuals with OUD in Philadelphia from 2017 to 2023. Data we

BACKGROUND: Asthma is a chronic, complex, and multifactorial respiratory disease affecting millions of people worldwide. Despite the availability of effective pharmacological therapies and evidence-based guidelines, asthma control remains suboptimal in many settings. Community pharmacists are well positioned to support asthma management through patient education, inhaler-technique assessment, adherence counselling, and referral when needed, roles increasingly emphasized in contemporary asthma guidelines. OBJECTIVES: This study aimed to assess community pharmacists' knowledge and practice rega

How do services offered within opioid treatment programs vary based on state methadone policies?

INTRODUCTION: State regulations governing opioid treatment programs (OTPs) vary widely in their restrictiveness, yet how state policies relate to the availability of services offered within OTPs remains understudied. In this study, we compare the availability of medication for opioid use disorder (MOUD) options, psychosocial services, and housing supports across OTPs operating in states with different levels of OTP policy restrictions. METHODS: We conducted a cross-sectional study of 1501 opioid treatment programs (OTPs) in the United States. Exposures included 11 state-level OTP policies tha

Facilitators and barriers to adherence to severe malaria treatment guidelines in children at Mbale Regional Referral Hospital, Uganda: a health workers' perspective.

BACKGROUND: Malaria is a significant public health concern, particularly in sub-Saharan Africa, where it causes considerable morbidity and mortality. Severe malaria is a major cause of mortality among children under five, contributing to Uganda's high malaria mortality burden. Effective case management of severe malaria is crucial which should be guided by the Uganda clinical guidelines adopted from the World Health Organization. This study explored the barriers and facilitators to healthcare providers' adherence to severe malaria treatment guidelines in children at Mbale Regional Referral hos
