The circulatory arrest recovery ammonia problem (CARAP) hypothesis: A proton magnetic resonance spectroscopy (1H-MRS) study of brain metabolism during neonatal cardiopulmonary bypass surgery.

OBJECTIVE: Congenital heart disease affects 1% of US births, with many babies requiring major cardiothoracic surgery under cardiopulmonary bypass (CPB), exposing the more critical patients to neurodevelopmental impairment. Optimal surgical parameters to minimize neuronal injury are unknown. We used proton magnetic resonance spectroscopy (1H MRS) and blood ammonia assays in a neonatal pig model of CPB to compare 2 approaches, complete circulatory arrest (CA) versus antegrade cerebral perfusion. METHODS: Two-week old piglets (N = 17) were put on a CPB pump and placed in a 3-T magnetic resonance

Real-world management of achalasia and esophagogastric junction outlet obstruction in Italy: results from a national survey.

There is significant variability in clinical guidelines for achalasia, and precise indications for Esophagogastric Junction Outflow Obstruction (EGJOO) are lacking. The recommendations provided in the published literature could be difficult to translate into the clinical practice due to the discrepancy in the available resources. This survey aims to provide insight into the different diagnostic and therapeutic approaches adopted nationwide. An electronic 31-item questionnaire was sent among the members of the Italian Society for Endoscopic Surgery of Endoscopic Surgery and New Technologies (SI

Non-pharmacological approaches in gastroesophageal reflux disease: Evidence-based dietary and lifestyle interventions.

Gastroesophageal reflux disease (GERD) affects 18-28 % of Western populations, with 10-40 % experiencing refractory symptoms despite proton pump inhibitor therapy. We conducted a comprehensive narrative review of dietary interventions in GERD, analyzing studies that measured clinical efficacy through validated symptom scores (GERD-Q, RDQ) and objective reflux parameters (24-h pH-impedance monitoring, acid exposure time). We examined pathophysiological mechanisms, intervention efficacy, and clinical implementation strategies across diverse patient populations. Structured dietary interventions d

The Use of Knives With Versus Without Integrated Water-jet Function During Peroral Endoscopic Myotomy for Patients With Achalasia: A Meta-analysis.

BACKGROUND: Peroral endoscopic myotomy (POEM) is a well-established endoscopic treatment for achalasia, utilizing an endoscopic knife for dissection. Recently, new knives with an integrated water-jet (WJ) function have been introduced. This study aims to compare the technical, perioperative, and late postoperative outcomes between WJ knives and conventional (C) knives, which lack the WJ function, through a pairwise meta-analysis of published comparative studies. METHODS: We conducted a systematic literature review following PRISMA guidelines to identify studies directly comparing knives with

Post-endoscopic fever and infection in paediatric patients with intestinal failure.

OBJECTIVES: Routine antimicrobial prophylaxis (AMP) for preventing bacteraemia and infection during paediatric gastrointestinal (GI) endoscopy is not recommended and is reserved for high-risk scenarios. However, in the unique group of children with intestinal failure (IF) and a central venous catheter (CVC), the incidence of post-endoscopic fever (PEF) and infection and the usefulness of AMP in protecting the indwelling catheter are unknown. This study evaluated fever and infection rates post-endoscopy, and the role of AMP in children with IF and CVC. METHODS: This retrospective single-centre

Study to Establish the Relationship between Rabeprazole (PPI) and Acute Interstitial Nephritis (AIN).

INTRODUCTION: Proton pump inhibitors (PPIs) are the most commonly used drug worldwide. PPIs are benzimidazole derivatives and are basic in nature; therefore, they are dispensed in enteric-coated tablets or capsules to avoid degradation from gastric acid. When PPIs are administered by oral route of administration, they get absorbed from small intestine and cross the acidic parietal cell canaliculus. In this acidic environment, PPI is activated and produces a sulfenamide form of the drug. This activated form binds with the H+ K+ ATPase enzyme covalently, which results in irreversible inhibition
