Respiratory muscle activity and weaning from mechanical ventilation after spinal trauma: the ACCESSIT study.

RATIONALE: Spinal trauma, especially when associated with spinal cord injury (SCI), can impair respiratory muscle function and complicate early weaning from mechanical ventilation (MV). OBJECTIVES: In patients admitted for spinal trauma requiring MV, to prospectively assess and monitor the coordination, structure, and activity of non-invasively accessible respiratory muscles, in relation to weaning outcomes up to the first weaning attempt. METHODS: We enrolled adult patients with traumatic spine injury, requiring MV in the trauma-neuro intensive care unit (ICU) at St. Micheal's Hospital in T

The role of thoracoplasty in modern thoracic surgery: a narrative review with three illustrative cases.

BACKGROUND AND OBJECTIVE: Thoracoplasty, defined as the surgical resection of ribs to reduce pleural space volume, was first introduced in the late nineteenth century for the management of chronic empyema. Its use declined with the advent of effective antimicrobial therapy and advances in thoracic surgery. However, interest in thoracoplasty has reemerged in recent years. Tuberculosis and its sequelae remain prevalent globally, and complex pleural space infections continue to pose significant management challenges, particularly in patients who are not candidates for resection or in whom prior i

Effect of combined use of reinforcement materials on the fracture resistance of MOD cavity restorations in endodontically treated teeth.

OBJECTIVE: Bucco-lingual/palatinal vertical fractures are among the most common fractures found in mesio-occluso-distal (MOD) cavity restorations of endodontically treated teeth. This study aimed to compare the effects of different new-generation reinforcement materials applied to two cervical and coronal segments of the teeth on the fracture resistance of endodontically treated teeth in their coronal restorations. MATERIALS AND METHODS: A total of 84 freshly extracted, intact human mandibular molars were selected and treated endodontically according to standardized protocols. Standard MOD ca

Thoracic injuries in trauma patients: epidemiology and its influence on mortality.

BACKGROUND: Thoracic injuries are common among trauma patients. Studies on trauma patients with thoracic injuries have reported considerable differences in morbidity and mortality, and there is limited research on comparison between trauma patients with and without thoracic injuries, particularly in the Scandinavian population. Thoracic injuries in trauma patients should be identified early and need special attention since the differences in injury patterns among patient population are important as they entail different treatment regimens and influence patient outcomes. The aim of the study wa

Safety of Continuous Erector Spinae Catheters in Chest Trauma: A Retrospective Cohort Study.

BACKGROUND: The erector spinae block is an efficacious analgesic option for the management of rib fracture--related pain. Despite there being minimal published data specifically addressing the safety profile of this block, many societies have made statements regarding its safety and its use as an alternative to traditional regional anesthesia techniques in patients at risk of complications. The primary aim of this study was to characterize the safety profile of erector spinae plane block catheters by determining the incidence of early complications. The secondary aim of this study was to chara

A systematic review and meta-analysis for the management of Paget-Schroetter syndrome.

OBJECTIVE: There is currently no general agreement on the optimal treatment of Paget-Schroetter syndrome. Most centers have advocated an interventional approach that is based on the results of small institutional series. The purpose of our meta-analysis was to focus on the safety and efficacy of thrombolysis or anticoagulation with decompression therapy. A detailed description of the epidemiologic, etiologic, and clinical characteristics, along with radiologic findings and treatment option details, was also performed. METHODS: The current meta-analysis was conducted using the PRISMA guideline

Role of Botulinum Toxin in Pectoralis Minor Syndrome.

BACKGROUND: Thoracic outlet syndrome (TOS) causes neurogenic symptoms in 95% of the cases due to neurovascular bundle compression. The treatment goal is the decompression of the neurovascular structures. In the last decade, non-surgical treatments have been evaluated as a treatment option for spastic syndromes and thoracic outlet syndrome. In this study we report the use of botulinum toxin (BTX-A) injection as a diagnostic tool to identify the pectoral minor syndrome, and as a less aggressive treatment-option. METHODS: An observational cohort study of patients with neurogenic thoracic outlet

Mapping of Metastatic Level I Axillary Lymph Nodes in Patients with Newly Diagnosed Breast Cancer.

PURPOSE: We examined the distribution of pretreatment nodal metastases to the level I axilla (Ax-L1) to assess the appropriateness of current breast atlases and provide guidelines in relationship to easily identifiable anatomic landmarks for accurate delineation of this lymph node (LN) basin. METHODS AND MATERIALS: Patients with newly diagnosed breast cancer and biopsy-proven metastatic Ax-L1 LNs were identified. We related the location of each LN to its most adjacent rib and its distance from the bottom of the humeral head, axillary vessels, and a line connecting the anterior aspects of the

Prevalence of and Predictive Factors for Scoliosis After Surgery for Congenital Heart Disease in the First Year of Life.

BACKGROUND: The surgical treatment of congenital heart disease is reported to be associated with a high prevalence of scoliosis, although the detailed etiology is unknown. Surgical interventions involving the rib cage are considered to increase the risk of scoliosis. However, whether the cardiac condition or the procedure performed makes patients more susceptible to the development of spinal deformity is controversial. METHODS: The present study included 483 patients who underwent surgery for the treatment of congenital heart disease with use of procedures involving the immature rib cage (ste

Prevalence of systemic autoimmune rheumatic diseases and clinical significance of ANA profile: data from a tertiary hospital in Shanghai, China.

It is necessary and useful to explore prevalence of various systemic autoimmune rheumatic diseases (SARDs) in patients with suspicion of having SARDs and to characterize antinuclear antibodies (ANA) profile for identifying different populations (SARDs and non-SARDs). A total of 5024 consecutive patients with available medical records were investigated, whose sera had been tested for ANA profile, including ANA, anti-dsDNA and anti-extractable nuclear antigen (ENA) antibodies, between 31 January 2012 and 26 March 2014. Only 594 (11.8%) patients were diagnosed with SARDs of those suspected with S

Primary chest wall chondrosarcomas: results of surgical resection and analysis of prognostic factors.

OBJECTIVES: Wide surgical excision with tumour-free margins is the mainstay of therapy for primary chest wall chondrosarcoma (PCWC). Few studies on treatment outcome and prognostic factors of PCWC requiring chest wall resection are available. We analysed our experience on surgical treatment of PCWC with emphasis on survival and recurrence prognostic factors. METHODS: From 1986 to 2012, 89 patients (65.2% males, median age 55 years) with PCWC were operated on. The median tumour maximum diameter was 7 cm (range 2-30 cm). RESULTS: We performed 23 sternectomies and 66 lateral chest wall resectio

Frequency of skeletal injuries in children with inflicted burns.

BACKGROUND: It is estimated that inflicted burn injuries in physically abused children occur with a prevalence of approximately 6-20%. Identification of burns of a nonaccidental nature is oftentimes difficult. Underlying skeletal injuries in abusive environments are often overshadowed by the acute burn injury. OBJECTIVE: We assessed the prevalence of inflicted burns and the frequency of associated skeletal injuries in a population from a large children's hospital. MATERIALS AND METHODS: From a database of nearly 3,000 children who were assessed for possible abuse from 1997 to 2012, we identi

Surgical treatment and outcomes of pentalogy of Cantrell in eight patients.

PURPOSE: To summarize the experience and feasibility of radical one-stage surgical repair of pentalogy of Cantrell in eight cases in a single tertiary care center. PATIENTS AND METHODS: From August, 2007 to June, 2011, eight patients with pentalogy of Cantrell were diagnosed. One of them had undergone intracardiac operation 2 years ago, leaving a large thoracoabdominal defect with most of the heart outside the chest. After correction of cardiac malformations, patients were individually treated with different operative procedures to restore their cardiac position and anatomy and repair their t

Thoracodorsal nerve graft for reconstruction of facial nerve branching.

OBJECT: Surgical treatment of parotid malignancies may frequently involve facial nerve amputation to achieve oncological radical resection. The entire facial nerve branching from its exit from the stylomastoid foramen to the periphery of the gland is often sacrificed. The first reconstructive strategy is the immediate reconstruction of the facial nerve by directly anastomosing the trunk of the facial nerve to its distal branches by interpositional nerve grafting. The present study was performed to determine the adequacy of thoracodorsal nerve grafting for immediate repair of the facial nerve.

Subdural hemorrhage in pediatric patients with enlargement of the subarachnoid spaces.

OBJECT: Enlargement of the subarachnoid spaces has been theorized as a risk factor for the development of subdural hemorrhage (SDH). As the finding of unexplained SDH in children often raises suspicion for nonaccidental trauma, the possibility of increased risk of SDH in children with enlargement of the subarachnoid spaces has important clinical, social, and legal implications. Therefore, the authors evaluated the frequency of SDH in a cohort of children with enlargement of the subarachnoid spaces. METHODS: The authors identified children younger than 2 years of age who were diagnosed with en

The association between fracture site and obesity in men: a population-based cohort study.

A site-dependent association between obesity and fracture has been reported in postmenopausal women. In this study we investigated the relationship between body mass index (BMI) and fracture at different skeletal sites in older men (≥65 years). We carried out a population-based cohort study using data from the Sistema d'Informació per al Desenvolupament de l'Investigació en Atenció Primària (SIDIAP(Q) ) database. SIDIAP(Q) contains the primary care and hospital admission computerized medical records of >1300 general practitioners (GPs) in Catalonia (Northeast Spain), with information on a repr

[Elective visceral hybrid repair of type III thoracoabdominal aortic aneurysm].

INTRODUCTION: According to the classification given by Crawford et al. type III thoracoabdominal aortic aneurysm (TAAA) is dilatation of the aorta from the level of the rib 6 to the separation of the aorta below the renal arteries, capturing all the visceral branch of aorta. Visceral hybrid reconstruction of TAAA is a procedure developed in recent years in the world, which involves a combination of conventional, open and endovascular aortic reconstruction surgery at the level of separation of the left subclavian artery to the level of visceral branches of aorta. CASE REPORT: We presented a 75
