Subarachnoid hemorrhage caused by a ruptured anterior spinal artery aneurysm.

A 51-year-old man presented with an extremely rare case of intracranial subarachnoid hemorrhage caused by rupture of an anterior spinal artery aneurysm manifesting as disturbance of consciousness following sudden onset of neck pain and numbness of the extremities. Cranial computed tomography revealed subarachnoid hemorrhage, mainly in the posterior fossa. Cerebral angiography studies on admission and on the 4th day demonstrated no definite abnormality as a bleeding source. A ventricular catheter was inserted to treat the acute hydrocephalus, and conservative management was continued during the…

Sonographic appearance of heterotopic pregnancy with ruptured ectopic tubal pregnancy.

Heterotopic pregnancy, ie, combined intrauterine and extrauterine (ectopic) pregnancy, is a rare clinical entity that may present with acute abdominal catastrophy. Most of cases reported in literature are a consequence of medical or surgical interventions like assisted reproductive techniques, tubal surgeries, intrauterine contraceptive devices, or pelvic inflammatory disease. Heterotopic pregnancy occurring spontaneously is extremely uncommon. We describe the sonographic appearance of spontaneous heterotopic pregnancy with ruptured ectopic tubal pregnancy.

Management of ruptured amoebic liver abscess: 22-years experience.

BACKGROUND: Amoebiasis affects approximately 10% of the population all over the world. Amoebic liver abscess (ALA) is the commonest complication of amoebiasis affecting 3-9% victims. It is an ancient disease as Hippocrates notified large hepatic abscesses likely to be amoebic abscesses. Objective of this study was to see the efficacy of conservative treatment in the management of ruptured amoebic liver abscess. METHODS: Record of 1,083 patients of amoebic liver abscess, who were treated and managed at Liaquat University Hospital between January 1986 and December 2007, was reviewed retrospecti…

Neonatal ruptured intracranial aneurysms: case report and literature review.

INTRODUCTION: Intracranial aneurysms are exceptional in neonatal patients: There are only 16 cases previously reported. We describe the first case of neonatal posterior inferior cerebellar artery (PICA) aneurysm and review the literature. CASE REPORT: A 7-day-old girl presented with irritability, anorexia, fever and abnormally enlarging head circumference. Computed tomography (CT) and magnetic resonance (MR) imaging demonstrated intraventricular haemorrhage, secondary hydrocephalus and a pontine cistern haematoma. A PICA aneurysm was suspected on the CT angiogram (CTA) and the diagnosis was c

Ruptured bronchial artery aneurysm associated with bronchiectasis: a case report.

A massive hemothorax from a ruptured bronchial artery aneurysm (BAA) is very rare. Only 12 cases of ruptured mediastinal BAA have been reported. This case study describes a 77-year-old female with bronchiectasis who presented with anemia, hypertension, hemothorax, and a mediastinal mass. A chest tube was inserted through which 2 liters of unclotted blood was drained from the left pleural cavity. An enhanced computed tomography scan revealed a ruptured 3-cm diameter mediastinal aneurysm of a bronchial artery supplying the left lower lobe. Transcatheter artery embolization (TAE) with multiple mi…

Endovascular treatment of ruptured abdominal aortic aneurysm with aortocaval fistula.

Aortocaval fistula (ACF) is a rare complication of abdominal aortic aneurysm (AAA). We report the endovascular repair of an AAA rupture into the inferior vena cava. A 78-year-old woman was admitted to our hospital for acute hypotension. She presented with a pulsatile abdominal mass and became rapidly anuric. Abdominal computed tomography (CT) showed an AAA rupture into the inferior vena cava. The features of the AAA made it suitable for endovascular repair. To prevent pulmonary embolism caused by the presence of sac thrombosis near the vena cava lumen, a temporary vena cava filter was deployed…

Ruptured pseudoaneurysm following Gamma Knife surgery for a vestibular schwannoma.

A 75-year-old woman underwent Gamma Knife surgery (GKS) for a vestibular schwannoma. Eight years after GKS, she suffered sudden onset of headache. Computed tomography revealed diffuse subarachnoid hemorrhage around the cisterns of the posterior fossa. Right vertebral artery angiography showed an aneurysm arising from the lateral pontomedullary segment of the left anterior inferior cerebellar artery. The aneurysm was not located at a branching site. Trapping of the distal anterior inferior cerebellar artery aneurysm was performed, and the aneurysm was removed. The pathological features of this

Acute thrombosis and recanalization of a ruptured anterior communicating artery aneurysm.

A 35-year-old man sustained a subarachnoid hemorrhage due to the rupture of an anterior communicating artery aneurysm. A second angiogram taken 8 hours later demonstrated that the ruptured aneurysm had thrombosed spontaneously with a small residual aneurysm stump at the neck. CT scans and conventional angiograms taken 2 days later demonstrated recanalization of the aneurysm, which was successfully treated by endovascular coiling. This case differs from previous reports of spontaneously thrombosed ruptured aneurysms because the aneurysm recanalized within 2 days. Thus a thrombosed ruptured aneu…

Elbow strength and endurance in patients with a ruptured distal biceps tendon.

BACKGROUND: Patients with complete distal biceps tendon ruptures complain of fatigability with repeated elbow flexion and forearm supination. Some studies have documented changes in strength, but limited information is available about the effects of biceps tendon rupture on endurance. HYPOTHESIS: A ruptured distal biceps tendon results in decreased strength and enducance. MATERIALS AND METHODS: Isokinetic strength and endurance in elbow flexion and forearm supination were measured in both arms of 9 patients with an untreated unilateral complete distal biceps tendon rupture. One additional pa…

Epidemiology and outcomes of Achilles tendon ruptures in the National Football League.

The purpose of this study is to document the epidemiology of Achilles tendon ruptures in the National Football League (NFL) and to quantify the impact of these injuries on player performance. A retrospective review of several online NFL player registries identified 31 Achilles tendon ruptures in NFL players between 1997 and 2002. Nineteen percent of injuries occurred during preseason play, while another 18% occurred during the first month of the official season. There was a postinjury reduction of 88%, 83%, and 78% in power ratings for wide receivers, running backs, and tight ends, respectivel…

Functional reconstruction of a chronically ruptured extensor apparatus after patellectomy.

Rupture of the knee extensor mechanism after previous patellectomy is rare. A treatment protocol therefore is not available. This article presents a new and easy technique using a gracilis-semitendinosus tendon autograft for successful reconstruction of a ruptured knee extensor mechanism after previous patellectomy.

Supraventricular tachycardia in a patient with a ruptured abdominal aortic aneurysm: conclusion.

A transport request was received from a free-standing emergency facility to transport a morbidly obese man with a ruptured abdominal aortic aneurysm (AAA). Weather conditions at the time prohibited rotor-wing transfer, so ground transport was arranged. The patient was a 58-year-old man being worked up for a possible back injury. During the evaluation, the patient had an episode of supraventricular tachycardia (SVT) with associated hemodynamic instability. Although the SVT corrected without intervention, the patient remained hemodynamically unstable. An abdominal computed tomographic (CT) scan …

"Mini-open" repair of acute tendo Achilles ruptures--the solution?

BACKGROUND: The standard surgical exposure for repair of acute tendo Achilles rupture gives favourable results, but such extensive exposure increases the possibility of peritendinous adhesion, wound breakdown and infections which increases morbidity and impairs functional outcome. Open repair also increases post-operative hospital stay and hence encroaches on valuable bed space availability. To evade this mini-open technique was developed which provide anatomic apposition of the tendon ends and minimal damage to epitendon. METHODS: We describe a retrospective case series of 21 patients who we…

[Frequency of uterine rupture at delivery and accompanying risks for the mother and the newborn].

BACKGROUND/AIM: Uterine rupture at delivery is a severe lifethreatening complication for both mather and the newborn. The aim of the study was to determine the frequency of total number uterine rupture within the deliveries, to perceive circumstances and causes that lead to rupture, to establish perinatal and maternal mortality and to present our therapeutic procedure. METHODS: In the group of 37 ruptures at 59 660 deliveries from the period 1991-2000 included in this retrospective study we analyzed age, level of education, gestational age, parity, previous caesarian section and other operati…

Intraventricular haemorrhage without subarachnoid haemorrhage due to a ruptured aneurysm.

The incidence of aneurysmal subarachnoid haemorrhage (SAH) in the UK is about 10 per 100 000 of the population per year and it is a potentially fatal condition. Prompt recognition and referral to a neurosurgical unit is crucial for a better outcome. A typical history of sudden severe headache, CT scan and sometimes a lumbar puncture would help clinch the diagnosis. Though intracerebral haemorrhage (ICH) and intraventricular haemorrhage (IVH) are not uncommon after an aneurysmal rupture, ICH and/or IVH without SAH are rare (<2%). In such cases, a high index of suspicion is needed to deliver the…
