Delta Reverse Polarity Shoulder Replacement: Single Surgeon Experience with a Minimum 2-Year Follow-up.

BACKGROUND: The delta reverse shoulder replacement system was developed for the treatment of rotator cuff arthropathy so that the deltoid can substitute for the deficient rotator cuff. To evaluate the results of delta reverse shoulder replacement for functional improvement and complications in a consecutive series by a single surgeon over a period of six years with a minimum follow-up of 2 years. METHODS: The data were collected retrospectively from electronic theatre records. Over a period of 6 years (2006-2012), 46 cases that fulfilled the inclusion criteria were identified. There were 34 f

Partial-width injuries of the rat rotator cuff heal with fibrosis.

PURPOSE: Identify the healing outcomes following a partial-width, full-thickness injury to the rotator cuff tendon-bone attachment and establish if the adult attachment can regenerate the morphology of the healthy attachment. HYPOTHESIS: We hypothesized that a partial-width injury to the attachment would heal via fibrosis and bone remodeling, resulting in increased cellularity and extra-cellular matrix deposition, reduced bone volume (BV), osteoclast presence, and decreased collagen organization compared to shams. MATERIALS AND METHODS: A partial-width injury was made using a biopsy punch at

Translational constraint influences dynamic spinal canal occlusion of the thoracic spine: an in vitro experimental study.

Mechanical constraints to spine motion can arise in a variety of real-world situations such as when shoulder belts prevent anterior translation of the thorax during automotive collisions. The effect of such constraint on spinal column-spinal cord interaction during injury remains unknown. The purpose of the present study was to compare maximal dynamic spinal canal occlusion, measured via a specialized transducer, in cadaveric upper thoracic spine specimens under a variety of anterior-posterior constraint conditions. Four injury models were produced using 24 cadaveric spine specimens (T1-T4). I

Fall or shaken: traumatic brain injury in children caused by falls or abuse at home - a review on biomechanics and diagnosis.

In cases of traumatic brain injuries in children it may be difficult to differentiate between physical abuse and accidental occurrence. This review will shed light on discriminating epidemiological as well as biomechanical data, morphological findings, and clinical features. As a basic principle, simple injuries are caused by simple mechanisms like falls whereas life-threatening injuries should be attributed to abuse until proven otherwise. Implausibilities between reported events and mechanisms by caregivers as well as more than one explanation of injury indicate suspicion of child abuse. Fro

Subscapularis tears: arthroscopic repair techniques.

The subscapularis is an essential component of normal shoulder function. An intact subscapularis muscle provides the anterior moment for the transverse plane force couple. Any disruption of the subscapularis disrupts normal glenohumeral biomechanics. If this occurs the shoulder functions abnormally, leading to pain and disability. A thorough history and physical examination lead the surgeon to the diagnosis of a torn subscapularis. Radiographs and MRI are helpful in delineating the rotator cuff tear pattern and other intra-articular lesions and in determining subcoracoid stenosis. Advancements

The injury mechanisms of osteoporotic upper extremity fractures among older adults: a controlled study of 287 consecutive patients and their 108 controls.

The risk factors for falls in older adults are well known but knowledge on the direct injury mechanisms that result in various osteoporotic fractures has been very sparse. The purpose of this study was therefore to clarify the injury mechanisms of osteoporotic upper extremity fractures of older adults and to compare these mechanisms with those of the control fallers, and in this way to obtain reliable insight into the etiology and pathogenesis of upper extremity fractures and thus to enable fracture prevention. One hundred and twelve patients with a fresh fracture of the proximal humerus, 65 p

Upper Extremity Return to Sport Functional Testing: A Systematic Review.

BACKGROUND: Upper extremity return to sport (RTS) assessments are not standardized with respect to which metrics to use, if the metrics are appropriate for all populations and levels of competition, and what constitutes good or poor test performance. Subsequently, clinicians may utilize suboptimal metrics to evaluate RTS readiness, or forgo objective criteria altogether. The purpose of this study was to examine the psychometric properties (reliability, agreement/measurement error, hypothesis testing/construct validity, criterion/predictive validity, responsiveness) of upper extremity functiona

Monolateral Grover's Disease with Blaschkoid Distribution.

Dear Editor, Grover's disease, also known as transient acantholytic dermatosis, is an idiopathic and acquired pruritic eruption of small vesicles and erythematous papules, classically on the central chest. The pathogenesis is not clearly defined, although heat sweating and occlusion have been interpreted as predisposing factors. We describe a case of monolateral acantholytic eruption with blaschkoid distribution in a patient treated with an orthopedic bandage for a shoulder injury. A 53-years-old man was referred to the dermatologic department because of an acute eruption of pruritic, discrete

Semitendinosus Tendon Reconstruction for Recurrent Acromioclavicular Joint Instability Following Repeated Fixation Failure and Postoperative Trauma: A Case Report.

Recurrent instability following surgical treatment of Rockwood type III acromioclavicular (AC) joint injuries is uncommon but represents a challenging clinical problem, particularly after repeated fixation failure. A 67-year-old man presented with a Rockwood type III AC joint separation following a same-level fall. Initial suture-button fixation failed 45 days postoperatively. Revision hook plate fixation subsequently failed after a further traumatic event, and a third procedure involving rigid AC stabilization with dual plating also resulted in early mechanical failure. Definitive treatment w

Association Between Upper Limb Injury and Risk of Falls: A Nationwide Population-Based Cohort Study.

Background/Objectives: Falls and upper limb injuries (ULI) are prevalent in older adults, yet whether ULI independently predisposes to subsequent falls remains poorly characterized. This nationwide cohort study evaluated the association between ULI and future fall risk using Taiwan's National Health Insurance Research Database (2011-2019, follow-up through 2020). Methods: Adults aged ≥ 50 years with newly diagnosed ULI-defined as fractures (clavicle, scapula, humerus, radius, ulna, hand), sprains, strains, or open wounds of the shoulder, arm, elbow, forearm, wrist, or hand-were propensity scor

Electromyography-Informed Estimates of Joint Contact Forces Within the Lower Back and Knee Joints During a Diverse Set of Industry-Relevant Manual Lifting Tasks.

Repetitive manual labor tasks involving twisting, bending, and lifting commonly lead to lower back and knee injuries in the workplace. To identify tasks with high injury risk, we recruited N = 9 participants to perform industry-relevant, 2-handed lifts with a 11-kg weight. These included symmetrical/asymmetrical, ascending/descending lifts that varied in start-to-end heights (knee-to-waist and waist-to-shoulder). We used a data-driven musculoskeletal model that combined force and motion data with a muscle activation-informed solver (OpenSim, CEINMS) to estimate 3-dimensional internal joint con

Double figure of eight technique using fiber tape for acute high-grade acromioclavicular joint dislocations - Reconstruction technique.

Acromioclavicular (AC) joint dislocations are not uncommon injuries. High-grade dislocations can be managed by arthroscopic as well as open techniques, but the most important thing is to address both vertical and horizontal stability. The purpose of this study is to evaluate the functional outcome of a cost effective double figure of eight technique of stabilizing both coracoclavicular and acromioclavicular ligaments in acute high-grade disruption (Type IIIB -VI) using fibre tape. This study was performed at our tertiary care centre from 2017 to 2020 after appropriate IRB approval and patient

[Surgical access route and choice of implant in the region of the clavicle shaft].

BACKGROUND: Clavicle fractures are among the most frequent injuries of the shoulder girdle. Nondisplaced fractures are generally treated conservatively, whereas dislocated fractures require surgical reduction and stabilization. A variety of implants and surgical techniques with reliable results are available. While all techniques provide similar healing rates, they share a common disadvantage with the high incidence of implant irritation and correspondingly high rates of second interventions for material removal. OBJECTIVE: The various surgical techniques for clavicle shaft fractures with the
