[Tuberculosis in the Czech Republic. Current status. Diagnosis, treatment and prevention].

Despite the fact that the tuberculosis (TB) agent, Mycobacterium tuberculosis, was discovered more than a century ago and we have had a basic drug line for the treatment of this disease available for over 50 years, TB is still a worldwide threat, be it considering the number of the infected, or their mortality. The epidemiologic seriousness of this disease lies, however, in the chronic course of the illness, the length of time necessary for its treatment, unavailable preventative measures and the treatment in those parts of world that are stricken by hunger and poverty. The situation concernin

Periodontal health through public health--the case for oral health promotion.

Periodontal diseases are highly prevalent, particularly amongst socially disadvantaged populations, impact on quality of life and are costly to treat. Clinical treatments and chairside preventive approaches alone will never adequately address this problem. Indeed in many parts of the developing world clinical care and chairside prevention are both unaffordable and inappropriate for the control of periodontal diseases. A paradigm shift away from the individualized treatment approach to a population public health model is needed to promote periodontal health and, in particular to address social

Structural stigma, sex work and HIV: contradictions and lessons learnt from a community-led structural intervention in southern India.

BACKGROUND: Recent theorisation has pushed stigma research in new directions, arguing for a need to challenge the unequal power relations that impact groups most at risk for HIV-related stigma rather than locate stigma in the individual. Such a conceptualisation resonates with the growing emphasis on structural interventions for HIV prevention that attempt to alter the social context of risk. METHODS: The paper predominantly relies on longitudinal interviews conducted three times over a 2-year period with sex workers with varying degrees of involvement with the non-governmental organisation (

Interpersonal abuse and depression among mexican immigrant women with type 2 diabetes.

Evidence for a bi-directional relationship of depression and type 2 diabetes suggests that social distress plays a role in depression among people with diabetes. In this study, we examine the relationship between subjective distress and depression in 121 first- and second-generation Mexican immigrant women seeking diabetes care at a safety-net hospital in Chicago. We used a mixed-methods approach including narrative interview, survey, and finger-stick blood HbA1c data. Using grounded theory analysis, we identified seven life stressors from narrative interviews: interpersonal abuse, stress rela

Spontaneous coronary artery dissection: a disease-specific, social networking community-initiated study.

OBJECTIVE: To develop and assess the feasibility of a novel method for identification, recruitment, and retrospective and prospective evaluation of patients with rare conditions. PATIENTS AND METHODS: This pilot study is a novel example of "patient-initiated research." After being approached by several members of an international disease-specific support group on a social networking site, we used it to identify patients who had been diagnosed as having at least 1 episode of spontaneous coronary artery dissection and recruited them to participate in a clinical investigation of their condition.

Effectiveness of community treatment orders for treatment of schizophrenia with oral or depot antipsychotic medication: changes in problem behaviours and social functioning.

OBJECTIVE: Involuntary outpatient commitment (IOC) has been in use in various countries for a number of years and has recently been implemented (in the form of supervised community treatment) in England and Wales. Several studies indicate that IOC reduces relapse and readmission rates and decreases length of stay on inpatient units in patients diagnosed with schizophrenia. The aim of the present study was to examine whether the use of IOC in the Australian context, in the form of community treatment orders (CTOs), may be associated with a reduction in problem behaviours and improved social fun

Behavioral alterations and vascular dementia.

BACKGROUND: Vascular dementia is one of the most frequent forms of dementia, where behavioral and cognitive symptoms coexist. Negative signs, such as apathy, abulia, opposition, and agnosia, are badly tolerated and dramatically experienced by caregivers, even worse than the other signs of cognitive decline. REVIEW SUMMARY: We have studied 120 subjects affected by subcortical vascular dementia (group A) and 120 subjects suffering from multiinfarct dementia (group B) for 24 months. The main outcomes of the study were the global performance, the global behavioral symptoms, the caregiver stress,

Effects of alcohol misuse on gambling patterns in youth.

OBJECTIVE: Gambling and alcohol misuse are prevalent among youth and may be part of a common problem behavior syndrome. It was hypothesized that alcohol misuse would predict a pattern of increased youth gambling or a pattern of stable high gambling after controlling for key sociodemographic, socialization and individual factors. METHOD: Data were analyzed from two longitudinal studies of youth living in a western New York metropolitan area. Respondents' gambling at two times over the course of 12-18 months was classified into one of five gambling pattern groups, representing flat-low, increas

Interstitial cell tumor in a black-and-white ruffed lemur (Varecia variegatus variegatus).

A 14.5-yr-old, male black-and-white ruffed lemur (Varecia variegatus variegatus) presented for acute enlargement of the left testicle and hemiscrotum. Physical examination also revealed poor pelage quality with short guard hairs, sparse undercoat, and areas of alopecia. Increased aggression was also reported. A unilateral, open orchiectomy was performed, with the left testicle, epidydymis, associated vaginal tunic, and attached spermatic cord removed. Microscopic evaluation was consistent with an interstitial cell tumor, with many morphologic features similar to this neoplasm in people. No ove

Neonatal cardiac vagal tone and school-age developmental outcome in very low birth weight infants.

Twenty very low birth weight (VLBW) children participated in a longitudinal follow-up study designed to explore the relation between neonatal physiological measures and school-age (6-9 years) outcome measures. Neonatal measures of mean heart period and cardiac vagal tone (respiratory sinus arrhythmia, RSA) were derived from weekly recordings at 33, 34, and 35 weeks gestational age. An RSA Maturation measure was calculated across the 3-week period to assess functional changes in vagal regulation of the heart during a period in which there is enhanced myelination of the vagus. In addition, the r

A case-control study of risk factors for asthma in New Zealand children.

OBJECTIVE: As in other English-speaking countries, asthma is a major and increasing health problem in New Zealand. This study examined the risk factors for asthma in children aged 7-9. METHODS: Cases and controls were randomly selected from participants in the Wellington arm of the International Study of Asthma and Allergies in Childhood (ISAAC). Cases were children with a previous diagnosis of asthma and current medication use (n=233), and controls were children with no history of wheezing and no diagnosis of asthma (n=241). RESULTS: After controlling for confounders, factors significantly

[Popular perceptions about childhood diarrheas: diversity and unity].

Sickness is defined by culture: each society has its own way of labelling, explaining and treating symptoms. In the case of childhood diarrhea, each population selects some signs and considers them as symptoms, defines a limit between normal and pathologic, arranges symptoms in order to build syndromes that make up a local nosology. Examples from Thailand, China, Algeria, Nicaragua and Burkina Faso show the diversity of popular beliefs about diarrhea. These beliefs depend upon the epidemiological context, but they do not reflect it exactly: thus in Burkina Faso, AIDS has not been integrated in

The Emotional, Social, and Behavioral Implications of Insulin-Induced Hypoglycemia.

Hypoglycemic episodes occur commonly in patients with insulin-dependent (Type I) diabetes and are associated with several negative sequelae. These include unpleasant symptoms, deterioration in cognitive-motor functions, embarrassment, accidents, unconsciousness, seizures, and even death. Although hypoglycemic disruptions in cognitive and motor function caused by neuroglycopenia are well-documented, the effects of hypoglycemia on other areas of psychosocial function have received less scientific attention. This paper examines the impact of hypoglycemia on emotional status, social behavior, and

Impact of the human egalitarian syndrome on darwinian selection mechanics.

With nothing more than kin selection and reciprocal altruism theories to work with, the selection basis of human degrees of altruism and cooperation is often difficult to explain. However, during our prehistoric foraging phase, a highly stable egalitarian syndrome arose that had profound effects on Darwinian selection mechanics. The band's insistence on egalitarianism seriously damped male status rivalry and thereby reduced the intensity of selection within the group by reducing phenotypic variation at that level, while powerful social pressure to make decisions consensual at the band level ha

[Psychological adaptation in coronary patients].

Summarizing some essential results from empirical investigations about the individual adaptation and coping with coronary heart diseases five principles are formulated: It ist not possible to predict the individual reactions to a coronary disease from the knowledge of the pathophysiological aspects. The behaviour pattern of these patients are different and complex determined. The individual reactions to the outbreak of the disease equally depend on familial, occupational, social and public health care factors on one side and on intrapsychical factors of the patient on the other side (i.e. pers

Virtual Reality-Based Social Musical Exergame Guided by Self-Determination Theory for Young Adults With Depression and Anxiety: Protocol for a Randomized Controlled Trial.

BACKGROUND: Depression and anxiety frequently emerge during late adolescence and young adulthood; however, many conventional and app-based interventions struggle to sustain engagement. Virtual reality (VR) exergaming, music-based activities, and social interaction each show promise for supporting young people's mental health, but their combined therapeutic value remains insufficiently tested. OBJECTIVE: This study aims to evaluate the effectiveness of a 6-week VR-based social musical exergame for reducing depressive and anxiety symptoms in young adults. The secondary objectives are to explore
