We reviewed the cases of fifty patients who underwent anterior lumbar-spine fusion with autogenous fibular and iliac-bone grafts and were followed for two to fifteen years. Their diagnoses were instability of the spine, degenerative disc disease, pseudarthrosis, and spondylolisthesis. Fifty-six per cent had union and 44 per cent, non-union. Those who had iliac grafts healed in an average of 2.5 years and those who had fibular grafts, in 5.2 years. The clinical result was successful in twenty-six patients (52 per cent) and unsuccessful in twenty-four patients (48 per cent). Paradoxically, about…

Postmortem radiographic examinations in 312 victims of fatal traffic accidents were analyzed with respect to injuries to the cervical spine. Most fractures and dislocations in this group were found to involve the craniocervical junction and the upper two cervical segments. Over half were flexion injuries, one-fifth caused predominantly by extension. Flexion and extension views were found to be helpful in demonstrating the instability of many injuries.

BACKGROUND: The 2018 International Society for the Advancement of Spine Surgery (ISASS) bone graft policy established the society's comprehensive framework for bone graft substitutes in spinal surgery. Additionally, a subsequent independent 2019 analysis by Marx and Lorio examined the regulatory and evidentiary distinctions applicable to class III spinal grafts. Since those publications, substantive regulatory, evidentiary, and technological developments have emerged that warrant a focused ISASS policy update.
METHODS: A focused regulatory and clinical evidence landscape review was conducted …

Surgical indications for lumbar spine disorders should not be determined solely based on degenerative or stenotic magnetic resonance imaging findings. This article discusses how orthopedic spine surgeons evaluate lumbar pathology in collaboration with neurologists. The central issue is whether the patient's symptoms, neurological findings, and imaging abnormalities can be used to identify the responsible lesion that is likely to improve after surgery. Differential diagnoses, including cervical or thoracic myelopathy, peripheral neuropathy, motor neuron disease, vascular claudication, hip disea…

Lumbar spine degeneration (LSD) is a major contributor to lower back pain (LBP) worldwide, typically diagnosed using magnetic resonance imaging (MRI). Manual interpretation of MRI scans is time-consuming and prone to variability, creating an urgent need for automated solutions. This study introduces a deep learning (DL) framework for classifying three key degenerative conditions, neural foraminal narrowing, subarticular stenosis, and spinal canal stenosis, across five lumbar disc levels. We evaluated multiple architectures, including VGG-16, EfficientNetV2-S, and YOLOv5, and enhanced feature e…

INTRODUCTION: In 2016 the National Institute for Health and Care Excellence (NICE) published guidance in England recommending computed tomography (CT) as the first line imaging for cervical spine (C-spine) injury in adults. However anecdotal evidence suggests that X-rays are still being performed. The aim of this evaluation was to determine the imaging referral patterns following C-spine trauma and compare with national guidance.
METHODS: Within a single English organisation emergency department patient attendance data, X-ray and CT referral criteria and acquisition information for patients ≥…

BACKGROUND: Postoperative pain after lumbar spine surgery often requires substantial opioid use. Perineural adjuvants such as dexamethasone and dexmedetomidine are increasingly used to prolong the duration of regional blocks and improve analgesic quality; however, their comparative effects remain uncertain. Ultrasound-guided erector spinae plane block (ESPB) is widely used as part of multimodal analgesia, but the selection of perineural adjuvants remains uncertain.
QUESTIONS/PURPOSES: (1) Does adding adjuvants (dexamethasone or dexmedetomidine) to the ESPB result in longer time to first rescu…

BACKGROUND: Anterior approaches to the cervical spine have consistently increased annually, with commonly performed procedures demonstrating low morbidity and mortality rates. However, rare complications of postoperative hematoma requiring readmission or reoperation poses risks such as respiratory compromise and reintubation. This study sought to utilize machine learning algorithms (MLA) on American College of Surgeons National Surgical Quality Improvement Program data to characterize clinical risk profile for readmission and reoperation secondary to postoperative hematoma in patients followin…

INTRODUCTION AND CLINICAL IMPORTANCE: Myxoid liposarcoma (MLS) is a rare soft tissue sarcoma subtype, typically affecting adults. Spinal involvement is exceptionally uncommon, requiring careful diagnosis and management. We report a rare primary cervical spine case.
CASE PRESENTATION: A 30-year-old man came to the neurology clinic transferred from a general surgeon with weakness and stiffness in both legs after a biopsy of a deep neck mass. He came to the general surgeon with a history of neck swelling with mild dysphagia. Magnetic resonance imaging (MRI) showed a large mass from C5 to T3 with…

INTRODUCTION: Piezoelectric bone surgery is an emerging technique that facilitates the selective cutting of mineralized tissue while preserving soft tissue integrity. Conventional tools such as the high-speed drill (HSD) and Kerrison rongeur are associated with complications, such as dural tears and neurovascular injury. This study compares intraoperative and post-operative (operative time, blood loss, complications, and post-operative pain) outcomes of cervical laminectomy performed using piezoelectric surgery (PS) versus high-speed drilling.
MATERIALS AND METHODS: A prospective comparative

OBJECTIVE: Dysphagia has been shown to have an adverse effect on patient quality of life following cervical spine surgery, but its impact on patients' ability to return to work (RTW) and return to physical activities (RTA) has never been investigated. This study aimed to evaluate the impact of postoperative dysphagia on patients' ability to RTW and RTA after surgery.
METHODS: A prospectively maintained multi-institutional spine registry of patients who underwent cervical spine surgery between 2016 and 2023 was retrospectively reviewed. An Eating Assessment Tool-10 (EAT-10) score ≥ 3 was used …

Fos-expressing neuronal ensembles in nucleus accumbens (NAc) are selectively activated by drug-associated cues and shown to play a causal role in drug-cue memories. Many unique molecular and cellular alterations have been identified in these ensembles and thought to be components of an engram encoding the memory; however, spine morphology alterations on these ensembles have rarely been examined. Here, we examined alterations of identified spine types on Fos-expressing neurons in NAc core that were selectively activated by drug-associated cues during cocaine relapse. We combined Fos immunolabel…

Back pain affects millions worldwide, with lumbar spine disorders (LSDs) being a major contributing factor due to the spine's critical role in supporting body weight. This study aims to improve the detection of LSDs using advanced deep learning techniques. The proposed framework introduces a novel pipeline that begins with bilateral filtering for image preprocessing, followed by segmentation using a Proposed Residual Block with Patch-based RESU-NET (PRB-PRESU-NET). Feature extraction integrates Gaussian Filter with Scharr Operator-based Gradient Local Ternary Patterns (GSO-GLTP), MT, and deep …

Tibial spine fractures are uncommon injuries, with a peak incidence between 8 and 14 years of age. Meyers and McKeever types II to IV are often managed surgically using a variety of techniques and fixation devices, depending on the fracture characteristics and the surgeon's expertise. The aim of this review is to analyze current evidence regarding functional outcomes across different surgical techniques. A systematic review was conducted using MeSH, Emtree, DeCS, Free Terms, and Boolean Operators across three databases. Data on trauma characteristics, patient demographics, and main outcomes fo…
