[Circulation of intestinal infection in the families of patients with rheumatic diseases and the state of humoral immunity to enterobacterial antigens].

ELISA quantitation of antibodies to Klebsiella pneumonia, Proteus mirabilis, and Yersinia enterocolitica was conducted in 75 ankylosing spondylitis (AS), rheumatoid arthritis (RA), and systemic lupus erythematosus (SLE) probands and in 262 of their first-degree relatives. The control group comprised 28 healthy donors. Sixteen (64 per cent) AS probands (12 with B27+) and 26 (30.2 per cent) their relatives (14 with B27+) were found to have high titres of antibodies to Klebsiella pneumonia. In 18 (60 per cent) active RA probands and 27 (23.7 per cent) relatives, antibodies to Proteus mirabilis we…

HLA antigens in brucellosis.

106 patients diagnosed as having brucellosis were typed for HLA class I and class II antigens. The number of healthy controls was 272 and 102 for classes I and II, respectively. 17 HLA-A, 31 HLA-B, 7 HLA-C and 8 HLA-DR specificities were studied. The most important finding was a highly significant decrease of the HLA-Cw2 antigen frequency in the patient group compared with the controls. The corrected p value was p less than 0.0001 (Fisher's test). This finding is compatible with the idea that HLA-Cw2 antigen could be a protective factor. No other positive or negative association was observed.

Retropharyngeal abscess in the adult.

Retropharyngeal and prevertebral abscesses, although uncommon, present a perplexing diagnostic and therapeutic problem. In the adult, the disease is less common than in the child, but its etiology may be more diverse. Four cases of retropharyngeal or prevertebral abscesses in the adult are presented, resulting from (1) endotracheal intubation trauma, (2) esophageal perforation, (3) tuberculosis of the cervical spine, and (4) coccidioidomycosis. The pathophysiologic conditions and pertinent anatomy are discussed in relation to the medical and surgical approach to these lesions.

Hyperinsulinemic State and Hypokalemic Quadriparesis in Tropical Fever: Is There a Link?

It is rare for quadriparesis to manifest as a symptom of tropical illnesses. With a history of only one fever episode one week prior, our patient, a 48-year-old male with obesity and prediabetes, who was also known to have ankylosing spondylitis, presented with acute onset flaccid quadriparesis. He did not exhibit any additional symptoms of dengue, such as bleeding tendencies, petechial rashes, thrombocytopenia, or febrile episodes. Upon examination, it was discovered that he had extremely low serum potassium levels and was dengue non-specific antigen 1 (NS1) positive. His hyperinsulinemia, as…

Biomechanics in the onset and severity of spondyloarthritis: a force to be reckoned with.

Increasing evidence suggests that there is a pivotal role for physical force (mechanotransduction) in the initiation and/or the perpetuation … ecause of emerging data from both animal models and human studies of the pathogenesis of ankylosing spondylitis (AS). In this review, we provide evidence towards an appreciation of the unsolved paradigm of how biomechanical forces may play a role in the initiation and propagation of AS.

[JAK inhibitors: Perspectives in internal medicine].

In the past ten years, the better understanding of the pathophysiological mechanisms underlying inflammatory and autoimmune diseases has led … efficacy in other autoimmune or inflammatory conditions, such as lupus, dermatomyositis, ankylosing spondylitis, sarcoidosis and giant cell arteritis. This review provides a summary of current use and advancement of knowledge in the use of JAK inhibitors in pathologies faced by internists.

The Anthropometric Measurement of Schober's Test in Normal Taiwanese Population.

The measurement of lower back mobility is essential in the assessment of lower back pain including ankylosing spondylitis. Original Schober's test (OST) and modified Schober's test (MST) are popularly conducted in daily rheumatology and orthopedics clinical practices. To our knowledge, this report is the only anthropometric reference study in a normal oriental population. The OST declined with age from 5.0 cm in the youngest (20-30 years old) to 3.1 cm in the aged (70-80 years old) male subjects and from 3.6 cm to 2.4 cm in the female subjects. The male OST was significantly more than the fema…

Successful treatment of persistent MRSA bacteremia using high-dose daptomycin combined with rifampicin.

We herein report a case of persistent methicillin-resistant Staphylococcus aureus (MRSA) bacteremia that was successfully treated with combination therapy consisting of high-dose daptomycin (DAP, 10 mg/kg) and rifampicin. The patient's condition was complicated with multiple infectious foci, including an iliopsoas abscess and epidural abscess, as well as discitis and spondylitis at the cervical, thoracic and lumbar levels. Monotherapy treatments with vancomycin, linezolid and usual-dose DAP were all ineffective. It has been shown that usual-dose DAP administration may result in the emergence o…

Lower extremity arthroplasty in patients with inflammatory arthritis: preoperative and perioperative management.

Spondylarthritis, which includes conditions such as ankylosing spondylitis and psoriatic arthritis, and rheumatoid arthritis are the most common forms of inflammatory arthritis. Joint inflammation and damage may result in the need for arthroplasty, and the surgeon must be aware of the perioperative challenges associated with these systemic diseases. In patients with inflammatory arthritis who have polyarticular disease and spinal involvement at the time of presentation for lower extremity arthroplasty, preoperative evaluation must include careful evaluation of all joints, including the cervica…

Surgical treatment of temporomandibular joint ankyloses: meniscus conservation and relocation.

Ankylosis of the temporomandibular joint is a serious complication, mainly after trauma and local or systemic infection. In rare cases, ankylosis is associated with systemic disease such as ankylosing spondylitis, rheumatoid arthritis, and psoriasis. According to the functional restriction and the provoked disturbances of facial growth in the youth, an early and effective therapy is desirable. There is a wide variety of surgical approaches to temporomandibular joint ankylosis, ranging from chondro-osseous grafts to prothesis. In the article the authors present the clinical case of a 60-year-ol…
