Radiation-induced functional and structural brain alterations are well documented in patients with nasopharyngeal carcinoma (NPC), followed by radiotherapy (RT); however, alterations in structure-function coupling remain largely unknown. Herein, we aimed to assess radiation-induced structure-function decoupling and its importance in predicting radiation encephalopathy (RE). We included 62 patients with NPC (22 patients in the pre-RT cohort, 18 patients in the post-RT-RE+ve cohort, and 22 patients in the post-RT-RE-ve cohort). A metric of regional homogeneity (ReHo)/voxel-based morphometry (VBM

Individual differences in brain structure are not only seen between healthy and diseased individuals, or between young and elderly people, but also in middle-aged men and women. Previous studies have shown that the cause is related to psychological distress and stress, and that it is accompanied by a decline in performance. This suggests that among middle-aged male and female workers who consider themselves healthy, the brain may atrophy depending on the level of psychological distress, which may reduce job satisfaction. Therefore, in this study, we analyzed the relationship between brain stru

The uncommon, benign dysembryoplastic neuroepithelial tumor (DNET, WHO grade 1) is frequently linked to epilepsy. It is a glioneuronal neoplasm in the cerebral cortex of children or young adults defined by the presence of a pathognomonic glioneuronal element that may be linked to glial nodules and activating mutations of fibroblast growth factor receptor 1 (FGFR1) (CNS WHO grade 1 according to WHO classification of CNS and pituitary tumors, 2021 ). The cerebral cortex is primarily affected. The most frequent areas are the temporal lobe, particularly the medial lobe, frontal lobe, and other cor

We report a case of primary central nervous system vasculitis (PCNSV) diagnosed by second brain biopsy. A 53-year-old man initially presented with left lateral gaze diplopia. Brain MRI revealed multiple enhanced lesions in the bilateral frontal lobe, bilateral basal ganglia, left cerebellum and brainstem. An initial brain biopsy of the right frontal lobe suggested immune-related encephalitis with angiocentric accumulation of chronic inflammatory cells, while malignant lymphoma could not be completely ruled out. The patient deteriorated despite being treated with repeated methylprednisolone pul

To investigate the correlation between involvement of the CNS in motor neuron disease (MND) and neuroimaging abnormalities, we studied 18 patients with MND by single-photon emission computed tomography (SPECT) and MRI. Patients were divided into four groups according to the results of SPECT. Group 1 consisted of four patients with reduced isotope uptake in the frontal lobe, including the motor area, and in the anterior part of the temporal lobe; group 2 consisted of two patients with reduced isotope uptake in the motor area spreading to the adjacent frontal lobe; group 3 consisted of eight pat

Herpes simplex encephalitis (HSE) is a severe viral infection of the central nervous system that most commonly affects the temporal lobes and limbic structures. Although most cases are managed medically with intravenous acyclovir, rare cases can rapidly progress to malignant cerebral edema, mass effect, and neurologic deterioration requiring surgical intervention. We report the case of a 57-year-old woman presenting with altered mental status, headache, vomiting, myalgias, and focal neurologic deficits. Initial computed tomography (CT) and CT angiography of the head and neck were unremarkable,

Impaired inhibitory control places a serious burden on the lives and psyches of patients with frontal lobe epilepsy (FLE). Music may serve as an adjunctive therapy to increase inhibitory control function in adults. However, whether music improves inhibitory control function in patients with FLE and its mechanism of action remains to be explored. In this study, 21 FLE patients and 24 healthy controls were recruited to assess the effect of music on inhibitory control in FLE patients by EEG under the Go/NoGo task. The results showed that FLE patients had poor inhibitory control at baseline, and a

BackgroundThe amygdala is a key structure involved in memory, emotional processing, and sensory integration. While the cortical connectivity of the amygdala with the frontal and temporal lobes has been extensively studied, its direct connections with the parieto-occipital cortices remain underexplored. This study aims to delineate the direct connectivity between the amygdala and the parietal and occipital cortices via the parahippocampal segment of the cingulum bundle.MethodsHigh-resolution diffusion-weighted imaging (DWI) tractography was performed on 30 healthy adult brains. All of the brain structures and white matter that make up Papez' circuit, as well as the circuit as a whole, are implicated in the literature in episodic memory formation and recall. This paper shows that Papez' circuit has the detailed structure and connectivity that is evidently required to support the episodic memory engram, and that identifying Papez' circuit as the location of the engram answers a number of long-standing questions regarding the role of medial temporal lobe structures in episodic memory. The paper then shows that the process by which the episodic memory engram may be forme

The purpose of this study was to evaluate possible relationships between duration of cochlear implant use and results of positron emission tomography (PET) measurements in the temporal lobes performed while subjects listened to speech stimuli. Other aspects investigated were whether implantation side impacts significantly on cortical representations of functions related to understanding speech (ipsi- or contralateral to the implanted side) and whether any correlation exists between cortical activation and speech therapy results. Objective cortical responses to acoustic stimulation were measure

Quantitative magnetic resonance imaging (MRI) was used to investigate volumes of different brain structures in 19 patients with late-onset major depression (DSM-III-R), 27 patients with Alzheimer's disease (NINCDS-ADRDA criteria) and 13 age matched controls. 3-D MRI sequences were acquired using a Siemens 1.5 T scanner. Whole brain volume, CSF volume, volume of the frontal and temporal lobes and the volume of the amygdala-hippocampus complex were assessed using the software NMR Win. Compared to the controls, depressed patients showed a significantly lower whole brain volume and a significantly

OBJECTIVE: To determine the relationship between hippocampal atrophy (HA), neurocysticercosis calcifications (NC) and seizure semiology in patients with partial epilepsy.
METHODS: We identified two groups of patients: mesial temporal lobe epilepsy (MTLE) and extra-temporal epilepsy (ETE) patients. Each group was subdivided according to CT findings: normal, NC in the temporal regions, and NC in extra-temporal regions. MRI was obtained in a 2-T scanner and hippocampal volumetry was performed using 3-mm coronal T1-IR images. Hippocampal volumes below 2 standard deviations from the mean of the co

The pathophysiological relation between periodic lateralized epileptiform discharges (PLEDs) and epileptic seizures is not known and the exact causative mechanism of PLEDs still remains unclear. In this report, the authors present a case in which the EEG displayed PLEDs after a complex partial seizure. This patient, with a long history of complex partial seizures, had previously undergone right standard anterior temporal lobectomy with hippocampectomy, with a diagnosis of mesial temporal sclerosis. She had one complex partial seizure 72 days after operation and was admitted to hospital. Her br

Pain has a significant impact on the quality of life of patients with multiple sclerosis (MS). However, the neurophysiological mechanisms of central neuropathic pain in a MS course are not known. We hypothesized that changes in power spectral density (PSD) that take place in the electroencephalography (EEG) of MS patients with and without the central neuropathic pain (CNP) would differ. The study aimed to assess the features of quantitative EEG using the PSD indicator along with peak frequencies in the standard frequency bands in MS patients with and without CNP. We have analyzed the quantitat

We describe 3-year clinical course of a 54-year-old Japanese man who presented with action myoclonus, parkinsonism and epilepsy. There was no family history or consanguinity. The patient was well until the age of 51 years (in 1986), when he noted slow movements, memory disturbance and left hand tremor. He was treated with anti-Parkinson drugs without any improvements. Soon thereafter, he developed a gait disturbance and generalized tonic clonic seizures. He was admitted to our service at the age of 53 years. General physical examination revealed no hepatosplenomegaly. Neurological examination

This study investigated whether repeated ictal single-photon emission tomography (SPET) is helpful in the localization of epileptogenic zones and whether it can provide information confirming that an area of increased perfusion is really the culprit epileptogenic lesion. Fifty-four repeated ictal SPET studies were performed in 24 patients with ambiguous or unexpected findings on the first ictal SPET study. These patients were enrolled from among 502 patients with intractable epilepsy in whom pre-operative localization of epileptogenic zones was attempted with a view to possible surgical resect

Minimally invasive surgery is gaining increasing interest in epilepsy surgery. In this video, the authors present the endoscopic transorbital approach for an epileptogenic lesion located at the temporal tip. The patient was a man in his 40s who has had intractable focal impaired awareness seizures and focal to bilateral tonic-clonic seizures since he was 31 years of age. According to the preoperative examination, including stereotactic electroencephalography, a cavernous angioma located at the tip of the right temporal lobe was diagnosed as an epileptogenic lesion. Lesionectomy for this lesion
