Risk of sural nerve injury during lateral distal Achilles tendinoscopy: a cadaver study.

BACKGROUND: The risk of damage to cutaneous sensory nerves located near portals has been evaluated for both conventional arthroscopy and extra-articular posterior ankle endoscopy. The objective of the anatomic study reported here was to assess the risk of injury to the sural nerve or lateral calcaneal nerve while using the distal lateral portal for the Achilles tendinoscopy procedure described by Vega et al. in 2008. MATERIALS AND METHODS: We dissected the sural nerve and its branch, the lateral calcaneal nerve, of 13 human cadaver ankles in the prone position. We defined P as the point where

Geometric and architectural contributions to hamstring musculotendinous stiffness.

BACKGROUND: Greater hamstring musculotendinous stiffness is associated with lesser anterior cruciate ligament loading mechanisms during both controlled joint perturbations and dynamic tasks, suggesting a potential protective mechanism. Additionally, lesser hamstring stiffness has been reported in females, potentially contributing to their greater risk of anterior cruciate ligament injury. However, the factors which contribute to high vs. low stiffness are unclear. Muscle geometry and architecture influence force production and may, therefore, influence stiffness. The purpose of this investigat

Gold nanoparticles and diclofenac diethylammonium administered by iontophoresis reduce inflammatory cytokines expression in Achilles tendinitis.

INTRODUCTION: Tendinitis affects a substantial number of people in several occupations involving repetitive work or direct trauma. Iontophoresis is a therapeutic alternative used in the treatment of injury during the inflammatory phase. In recent years, gold nanoparticles (GNP) have been studied due to their therapeutic anti-inflammatory capacity and as an alternative to the transport of several proteins. PURPOSE: This study evaluates the therapeutic effects of iontophoresis using GNPs and diclofenac diethylammonium on inflammatory parameters in rats challenged with traumatic tendinitis. MET

Biologic scaffold remodeling in a dog model of complex musculoskeletal injury.

BACKGROUND: Current treatment principles for muscle injuries with volumetric loss have been largely derived from empirical observations. Differences in severity or anatomic location have determinant effects on the tissue remodeling outcome. Biologic scaffolds composed of extracellular matrix (ECM) have been successfully used to restore vascularized, innervated, and contractile skeletal muscle in animal models but limited anatomic locations have been evaluated. The aim of this study was to determine the ability of a xenogeneic ECM scaffold to restore functional skeletal muscle in a canine model

Miniarthrotomy versus arthroscopic-assisted anterior cruciate ligament reconstruction with autogenous patellar tendon graft.

The purpose of this study was to determine whether two groups of patients showed any early (6 months postoperative) clinical differences when treated by arthroscopic-assisted or miniarthrotomy anterior cruciate ligament (ACL) reconstruction. Fifty-two consecutive arthroscopic-assisted ACL reconstructions (Group I) were matched with 52 miniarthrotomy ACL reconstructions (Group II). An autogenous midthird patellar tendon was used in all reconstruction procedures. Group I patients were operated on by one surgeon (A.C.R.) and all Group II patients by another (K.D.S.). Both groups were similar with

Graft choice does not influence outcomes after anterior cruciate ligament reconstruction in patients aged 50 years and older: A systematic review and meta-analysis.

AIM: Anterior cruciate ligament reconstruction (ACLR) in patients over 50 years has traditionally been considered controversial due to concerns about graft failure, limited healing potential, and the risk of degenerative progression. However, with increasing life expectancy and higher physical activity in older adults, the demand for ACLR in this population has grown, and the role of graft selection remains unclear. The aim of this study was to evaluate the influence of graft choice on clinical outcomes, graft survival, and re-rupture rates after ACLR in patients aged 50 years and older. It wa

Postoperative Complications After Acute Achilles Tendon Rupture Repair: A Survival Analysis of Minimally Invasive vs Open Techniques.

BACKGROUND: The complication rates after surgical repair of acute Achilles tendon ruptures (ATRs) using open repair or minimally invasive surgical techniques (MIS) have been debated extensively. Despite significant research, a literature hiatus exists on the timing of these complications between techniques. In this study, we aimed to address this gap by conducting a Kaplan-Meier survival analysis to compare the incidence and timing of complications after open vs MIS repair of acute ATRs and examine associated risk factors. METHODS: This retrospective study included patients ≥18 years who unde

Minimally invasive (ring forceps) versus open Achilles tendon repair: A retrospective comparison of ultrasonographic and functional outcomes.

OBJECTIVE: While surgical repair is standard for acute Achilles tendon ruptures, the optimal technique remains debated. This study com pares clinical, functional, and ultrasonographic outcomes between minimally invasive and open surgical approaches, with particular focus on: (1) patient-reported recovery, (2) tendon healing dynamics, and (3) the utility of ultrasound in postoperative monitoring. METHODS: This retrospective study analyzed 108 consecutive patients undergoing surgical repair for acute Achilles tendon ruptures between 2015-2023, comparing minimally invasive (n = 58; ring forceps

Ultrasonographic Evaluation of Extensor Carpi Ulnaris Tendon Subluxation at the Ulnar Groove.

PURPOSE: Subluxation of the extensor carpi ulnaris (ECU) tendon resulting from dysfunction of the ECU subsheath is frequently observed in clinical settings. However, there is a lack of information on how the distal extension of the ECU subsheath and ulnar groove morphology contribute to tendon stability. This study aimed to investigate the ECU tendon kinematics at the ulnar groove in intact and in subsheath-sectioned cadaver wrists, and examine the correlation between tendon subluxation and ulnar groove morphology during forearm and wrist motion. METHODS: Eight human cadaveric upper extremiti

The use of allograft tissue in posterior cruciate, collateral and multi-ligament knee reconstruction.

PURPOSE: Currently both autograft and allograft tissues are available for reconstruction of posterior cruciate, collateral and multi-ligament knee injuries. Decision-making is based on a complex interplay between anatomical structures, functional bundles and varying biomechanical requirements. Despite theoretically better biological healing and reduced risk of disease transmission autografts are associated with donor site morbidity as well as being limited by size and quantity. The use of allografts eliminates donor-site morbidity but raises cost and issues of clinical effectiveness. The purpo

Isokinetic functional outcomes of open versus percutaneous repair following Achilles tendon tears.

BACKGROUND: Rupture of the Achilles tendon (AT) is frequent in young recreational athletes. Conservative management, open surgery and percutaneous/minimally invasive approaches are all advocated, and conflicting data are available. This study compared functional and anthropometric outcomes of patients who underwent open or percutaneous repair. METHODS: A retrospective comparative study, in which 38 patients underwent open and percutaneous techniques to manage AT ruptures. For functional assessment, the calf circumference of both injured and uninjured legs was evaluated. Isokinetic testing inc

Biomechanical evaluation of varying the number of loops in a repair of a physiological model of Achilles tendon rupture.

BACKGROUND: Numerous suturing techniques have been described to treat Achilles tendon ruptures. No prior studies have evaluated frayed tendon ends on construct strength and whether this allows for less extensile exposure. METHODS: Forty bovine Achilles tendons were divided into groups: 1 control and 4 experimental. Experimental groups were sectioned with ends frayed longitudinally in 2 mm intervals for 2 cm with no fraying for the control group. Four-stand Krackow sutures were used for repairs with 3 loops in the control group, 2 loops in frayed section for experimental groups, and varying nu

Flexor pollicis longus tendon entrapment after performing minimally invasive plate osteosynthesis of a distal radius fracture: a case report.

In this study, we report a case of a 60-year-old woman with diabetes mellitus, who had a left distal radius fracture and could not flex the interphalangeal joint of her left thumb after we performed an open reduction and internal fixation procedure using minimally invasive plate osteosynthesis. Nine days after the first operation, we explored her wrist. We observed that the flexor pollicis longus tendon was compressed by the inserted plate. Then revision surgery and tendon repair were performed. Three months later, the fracture was healed and flexor pollicis longus function recovered well with
