Video-assisted thoracoscopic surgery for ectopic mediastinal parathyroid adenoma.

BACKGROUND: Primary hyperparathyroidism (PHPT), caused by an ectopic mediastinal parathyroid adenoma, is uncommon. In the past, when the adenoma was not accessible from the neck, median sternotomy was advocated for safe and successful parathyroidectomy. Video-assisted thoracoscopic surgical (VATS) parathyroidectomy represents a modern alternative approach to this problem. METHODS: Information on patients undergoing VATS was obtained from a specific database, including clinical presentation, biochemistry, preoperative imaging, surgical approach and patient outcomes. A comprehensive literature

Early effective treatment of small pulmonary nodules with video-assisted thoracoscopic surgery combined with CT-guided dual-barbed hookwire localization.

PURPOSE: To assess the feasibility of computed tomography (CT)-guided localization using a specific long dual-barbed hookwire in resection of pulmonary nodules with the size of 20mm or less by video-assisted thoracoscopic surgery (VATS) and to discuss the necessity of early treatment of small pulmonary nodules by VATS. RESULTS: All the nodules were successfully localized with hook wire under CT guidance. The 34 nodules had a mean diameter of 8.9 ± 3.8 mm and a mean distance from the most superficial edge of the nodules to the visceral pleura of 21.4 ± 12.4 mm. The mean length of time for CT-g

Preoperative short hookwire placement for small pulmonary lesions: evaluation of technical success and risk factors for initial placement failure.

OBJECTIVES: To retrospectively evaluate the technical success of computed tomography fluoroscopy-guided short hookwire placement before video-assisted thoracoscopic surgery and to identify the risk factors for initial placement failure. METHODS: In total, 401 short hookwire placements for 401 lesions (mean diameter 9.3 mm) were reviewed. Technical success was defined as correct positioning of the hookwire. Possible risk factors for initial placement failure (i.e., requirement for placement of an additional hookwire or to abort the attempt) were evaluated using logistic regression analysis for

Acute myocardial infarction as presenting symptom of thymoma with compression on a coronary stent.

We report the clinical case of a patient with acute myocardial infarction due to coronary stent compression as first manifestation of a large thymoma. The patient underwent a coronarography and thrombus aspiration + plain old balloon angioplasty restoring the stent patency. The mass resection was performed through left robotic-assisted thoracic surgery (RATS), resulting in a type A thymoma pT1a, IIb Masaoka-Koga. An uncommon presentation led to early diagnosis and treatment of a thymoma with both oncological and functional significance.

Surgical treatment of cervical rib-associated arterial thoracic outlet syndrome.

The arterial form of thoracic outlet syndrome is rare and is associated with anatomic anomalies, generally a cervical rib. It has a varied range of manifestations. The aim of this article is to describe two cases with different clinical presentations: microembolization and aneurysm. A cervical rib was present in both cases. Diagnosis was made on the basis of history, physical examination, postural maneuvers, and X-rays. Computed tomography angiography provided the anatomic detail necessary to plan surgery. Surgical treatment was performed via supraclavicular access, successfully, in both cases

Hemorrhagic thoracic schwannoma presenting with intradural hematoma and acute paraplegia after spinal manipulation therapy.

Hemorrhagic conversion of spinal schwannomas represents a rare occurrence; also rare is the development of a spinal intradural hematoma after spinal manipulation therapy. We report a unique presentation of paraplegia in a patient who underwent spinal manipulation therapy and was found to have a hemorrhagic thoracic schwannoma at time of surgery in the setting of anti-platelet therapy use. In patients with spinal schwannomas, tumor hemorrhage is a rare occasion, which can be considered in the setting of additive effects of spinal manipulation therapy and antiplatelet therapy.

[THORACOSCOPIC LOBECTOMY WITH MEDIASTINAL LYMPH NODE DISSECTION--THE STANDARD IN SURGICAL TREATMENT OF PATIENTS WITH T1-2N0M0 NON-SMALL CELL LUNG CANCER].

In recent years a large number of thoracic clinics in the world recommend the performance of minimally invasive surgical procedures in the early stages of lung cancer arguing that this technique reduces a number of postoperative complications, shortens the period of social rehabilitation of patients and does not significantly affect the long-term results of treatment. This work is devoted to the study of immediate and long-term results of surgical treatment of patients who have had surgery in the volume of thoracoscopic lobectomy at the early stages of non-small cell lung cancer.

Simultaneous management of renal carcinoma with caval vein thrombosis and double coronary artery disease.

INTRODUCTION: Recent advances in surgical and anesthesiology techniques allow simultaneous thoracic and abdominal operations to be performed for severe heart disease and benignant or malignant abdominal diseases. CASE REPORT: The simultaneous surgical management in a 75-year-old patient suffering from severe double coronary artery disease and a renal cell carcinoma with extended intravascular growth into the inferior vena cava is reported. CONCLUSION: The postoperative course was uneventful. Simultaneous surgery proved to be beneficial and safe, showing optimal results in our patient.

Video-assisted thoracoscopic thymectomy (VAT-T) with lateral thoracotomy for stage II and III thymoma.

Thymoma has malignant potential and is the most common anterior mediastinal tumor. Video-assisted thoracic surgery (VATS), which is less invasive surgical procedure, is a good option for resecting Masaoka stage I tumors. Whether VATS is appropriate, depends on the surgeon's judgment and accurate imaging diagnosis. We introduce a technique involving a combination of video-assisted thoracoscopic thymectomy (VAT-T) and lateral thoracotomy for stage II and some stage III thymomas that have locally invaded the lung and/or pericardium.

Surgical resection for isolated myeloid sarcoma arising from a rib.

We report on a rare manifestation of myeloid sarcoma with chronic myelogenous leukemia. The neoplasm arising from a rib showed osteolytic changes and infiltrated the surrounding muscles. We resected the lesion to relieve chest pain caused by pathologic rib fractures. The patient has continued receiving systemic therapy and is doing well as of 1 year postoperatively. Experience with a surgical approach for myeloid sarcoma is limited in thoracic surgery. The clinical course in our case suggests that surgical resection can be a valid choice of treatment.

Successful unilateral thoracoscopy for bilateral ectopic mediastinal parathyroidectomy.

Ectopic parathyroid glands are frequently located in the thymus and may become hyperplasic in patients with secondary hyperparathyroidism. When medical management fails, surgical removal is required, using either a traditional open sternotomy approach or video-assisted thoracic surgery (VATS). We were able to excise large, bilateral, mediastinal parathyroid glands using only left VATS. To the authors' knowledge, this is the first reported case of the use of unilateral thoracoscopic subtotal thymectomy for the excision of bilateral ectopic mediastinal parathyroid glands.

Ganglioneuroma of the retroperitoneum presenting as a pediatric renal mass.

Ganglioneuroma is the benign representative of peripheral neuroblastic tumors, with localized and predominant thoracic presentations in older children. They often have an excellent outcome with surgery alone. A 12-year-old girl presented with an incidentally detected abdominal mass, which was clinicoradiologically a renal mass. Laparotomy revealed a mass adherent to the anterior surface of the right kidney. The mass was carefully dissected out, sacrificing a portion of the inferior vena cava. Histopathological diagnosis was that of a ganglioneuroma. She was kept under follow up.

Polytetrafluoroethylene bars in stabilizing the reconstructed sternum for pectus excavatum operations in children.

Nine children with severe pectus excavatum deformity were operated on in the Department of Pediatric Surgery, Thoracic Unit, Ege University Faculty of Medicine, Izmir, Turkey. There were four boys and five girls, and the mean age at operation was 6.3 years. A modified Ravitch technique was performed by using two polytetrafluoroethylene bars placed retrosternally for fixation and stabilization of the reconstituted anterior chest wall. The bars were removed 6 months after the operation. Results were satisfactory in all cases, with no surgical complications.

[A cause of intubation failure: a subglottic bridge].

We report the case of a patient with a history of facio-thoracic burns, the treatment of which included prolonged intubation, whose trachea could not be intubated because of a subglottic obstacle. The ventilation was easily controlled with a laryngeal mask. At the end of surgery for postburn cheloids, laryngoscopy through the laryngeal mask showed a transversal subglottic laryngeal band, a probable sequela of the previous prolonged intubation. The band was resected one week later. The conventional indicators for difficult intubation cannot detect the laryngotracheal obstacles to tracheal tube

[A tropical pulmonary nodule].

The authors report a case of american pulmonary histoplasmosis discovered by chance on a chest radiograph in a non immunocompromised patient, back from a stay in French Guyana. Confronted with the negativity of usual mycotic research, diagnosis has been made thanks to wedge excision by video-assisted thoracic surgery. The authors briefly sum up the recent facts regarding this imported infection, stressing the interest of a direct approach by surgical practices not very invasive that allow to eliminate with certainty neoplasm or tuberculosis.

[Severe thoracic trauma: under what accident and resuscitation conditions can it still be treated?].

Severe thoracic trauma is a major cause of early death following road accident. 569 non-surviving and 112 surviving victims of traffic casualties were documented by the Accident Research Unit of the Clinic of Trauma Surgery, Hannover Medical School from 1973 to 1987. Medical data and measures of rescue were evaluated with a view to way of clinical therapies, lethal outcome and time of death. Primary intubation and chest draining seems to be the most successful initial treatment on the scene and during transport.

A case report of Forestier's disease.

Radiologic studies are essential in diagnosing Forestier's disease and include lateral cervical spine roentgenograms, thoracic and lumbosacral vertebrae roentgenograms, esophagram, vertebrae roentgenograms, esophagram, and computed tomography. Patients with Forestier's disease may be considered to be operative candidates if they have persistent dysphagia, dysphonia, and a feeling of fullness or foreign body in the throat. The patient in our case report underwent anterior surgical resection of her anterior cervical osteophytes. Two months following surgery, she was no longer experiencing dyspha

Lower-extremity ischemia in aortic dissection. Report of three cases.

Reports are presented of three hypertensive men with aortic dissection and signs of peripheral (leg) ischemia. The dissection was of type III b in two cases and type I in one case. Hypotensive medication was given in all cases, but in two of them peripheral vascular surgery became necessary to re-establish the circulation, while in the third case the hypotensive treatment sufficed for improvement of the leg circulation. Collaboration between thoracic and vascular surgeons can be of value in cases of aortic dissection with peripheral ischemia.

[Localization of Ascaris lumbricoides in the thoracic cavity. Report of a case].

Several aspects of the history, evolutive cycle and erratic phases of Ascaris lumbricoides as well as of the symptoms and diagnosis of ascariasis are analyzed. The anatomicophysiological concept of spontaneous pneumothorax and its clinical picture, frequency and treatment are exposed. A patient who underwent thoracic surgery due to an acute respiratory syndrome in who a male Ascaris lumbricoides was surprisingly found obstructing the probe after an explorating left thoracotomy and a resection of the lung apex is presented. The nematode is suggested as the possible etiological agent of the resp…
