A case of bleeding cecal typhoid ulcer in a 22-year-old patient demonstrated by superior mesenteric arteriography is described. An attempt to stop hemorrhage using intraarterial vasopressin infusion, failed. However, arteriography proved helpful in locating the bleeding site, and in permitting conservative surgery, namely cecal ulcer suture to achieve hemostasis. On the basis of this case and a review of the literature, management of complicating hemorrhage resulting from typhoid fever is discussed, with particular reference to radiological procedures.

To evaluate the relationship between duodenal ulcer disease and duodenitis, duodenal epithelial cell renewal was measured in mucosal biopsies by the incorporation of [3H]thymidine. When 14 patients with duodenal ulcer were compared to 13 control subjects or 7 with endoscopic duodenitis alone, the crypt size was the same in all groups. A double-blind trial using 51 outpatients was aimed at comparing the effectiveness of colloidal bismuth subcitrate (C.B.S.) in tablet and liquid form in the healing of duodenal ulcer. Criteria of entry included endoscopically proven duodenal ulcer, duration of symptoms greater than 4 weeks, and the absence of other major systemic disease. A man presented with what appeared clinically and histologically to be a benign gastric ulcer which healed with cimetidine. His condition subsequently deteriorated and a further endoscopy revealed a huge tumour in the body of the stomach with the histological appearances of non-Hodgkin's lymphoma. The presentation of non-Hodgkin's lymphoma in the elderly is discussed, together with the problem of gastriculcer healing by cimetidine in the presence of underlying malignancy.

There are particular situations in the frame of the surgery of gastric ulcers, when the use of classical techniques--such as extensive resections, or sub-total resections--represents a risk factor and raising doubts about the vital prognosis of the patient (dehiscence, fistula, etc.). Ranitidine and oxmetidine are newly developed histamine H2-receptor antagonists without any antiandrogenic activity and probably without drug interactions. In numerous clinical trials ranitidine has been proven to be at least as effective as cimetidine in the short- and long-term treatment of peptic ulcer disease. Ranitidine was found to be remarkably safe. Its therapeutic use in man should not be associated with significant adverse reactions.

The present study was conducted histologically to elucidate the influence of vagotomy on the gastric mucosa. Bioptical tissues were obtained from 10 control cases and from 26 cases of duodenal ulcer using an endoscope. Of the 26 cases of duodenal ulcer, 20 were subjected to truncal vagotomy with pyloroplasty and 6 to truncal vagotomy with hemigastrectomy. Ulcer healing and relapse rates after treatment with cimetidine and sucralfate in 112 subjects with endoscopically proven gastric and duodenal ulcers were compared in a randomized trial. It has been shown in experiments on white rats with chronic gastric ulcer induced according to the method of Takashi and coworkers that intramuscular administration of gamma-aminobutyric acid (GABA) exerts a pronounced antiulcerous effect. As compared with ganglerone, GABA reduces the content of free physiologically active histamine in the gastric mucosa to a far greater extent. The relationship has been disclosed between the therapeutic effect of GABA and the decreased content of histamine in the gastric mucosa.

A study was made of the effect of age, sex, ulcer size, bed rest, carbenoxolone sodium and anticholinergic drug therapy, and advice to cease smoking on the healing rate of chronic gastric ulcer determined radiologically over 21 days in a factorially designed experiment in 54 patients admitted to hospital.
