Active transportation to school over 2 years in relation to weight status and physical activity.

OBJECTIVE: To prospectively examine potential benefits of active commuting to school on measures of weight status and physical activity in a sample of youth. RESEARCH METHODS AND PROCEDURES: A cohort of students from seven elementary schools was measured four times--in the fall and spring of fourth grade (N = 1083) and fifth grade (N = 924). Participants were classified as active (walking, biking, or skateboarding to school almost every day for baseline analyses or at least 2 d/wk for analyses of consistent active commuting) or non-active commuters to school. Accelerometers were used to measu…

Exhaustive exercise slows inspiratory muscle relaxation rate in chronic obstructive pulmonary disease.

The excessive load placed on inspiratory muscles when patients with COPD exercise could lead to fatigue and contribute to exercise limitation. Slowing of maximal relaxation rate (MRR) of skeletal muscle is an early index of the fatiguing process. We investigated whether inspiratory muscle MRR slows when patients with COPD walk to exhaustion. We studied nine well-trained and motivated patients with stable severe COPD (mean FEV1: 0.7 L, 28% predicted). Each subject performed sniff maneuvers before and after walking on a treadmill until they were forced to stop because of dyspnea. Esophageal (Pes…

Correction of old standing fixed flexion deformity of the knee in poliomyelitis by soft tissue operation followed by gradual plaster wedging.

Over two years from March 1990, 30 cases of clinically diagnosed old poliomyelitis were seen; 22 males and eight females, aged from 10-24 years, with an average age of 17 years. No treatment was attempted before. In 15 cases, both knees were affected; 50 pc of those patients never walked before but were using wheel chairs. The rest could put on calipers and were walking putting their hands over the affected knee and experienced frequent falling. Flexion deformity ranged from 90-100 degrees (average 100 degrees). Upper limbs, the spine, sphincters and sensations were normal. In all cases quadri…

The relationship of symptoms to performance in paced patients with breathlessness.

Eighteen adult patients presenting with breathlessness associated with bradycardia, and whose breathlessness was diminished or abolished by … al, changing of ventricular rate caused parallel alterations in symptoms and performance. The 6-min walking test was a useful guide to disability in these patients, tightly linked to the symptoms of effort and breathlessness, and may be of clinical value in assessing mild heart failure. The pedometer readings were influenced more by the patients' walking …

Cerebellar iTBS Modulates Cortical Network Properties: An fNIRS Study.

BACKGROUND: The cortex and cerebellum have a closely connected closed-loop circuit. Intermittent theta burst stimulation (iTBS) targeting the cerebellum has shown promise in improving balance function and inducing neuroplasticity. This study investigates whether cerebellar iTBS can elicit cortical responses. METHODS: One hundred healthy volunteers were randomly assigned to a real or sham iTBS stimulation group. Functional near-infrared spectroscopy (fNIRS) measured cortical activation during resting, walking, and unilateral support tasks. RESULTS: During the unilateral support task, graph th…

Effect of an adapted Taekwondo-based intervention on functional and motor abilities in elderly care home residents: a study protocol.

This pilot protocol study aims to evaluate the effects of an adapted Taekwondo-based intervention on upper limb strength and overall functional capacity in elderly care home residents. Considering the global trend of population ageing, physical activity interventions in care facilities are essential to mitigate functional decline and enhance well-being among this population. 20 participants will be stratified into walking and non-walking groups to reflect the diverse functional levels within the facility. The primary outcome will focus on upper limb strength, which will be assessed using the h…

Bone Fractures Numerical Analysis in a Femur Affected by Osteogenesis Imperfecta.

This work presents a non-invasive methodology to obtain a three-dimensional femur model of three-year-old infants affected with Osteogenesis Imperfecta (OI) type III. DICOM® Files of a femur were processed to obtain a finite element model to assess the transverse, the oblique, and the comminuted fractures. The model is evaluated under a normal walking cycle. The loads applied were considered the most critical force generated on the normal walking cycle, and the analyses considered anisotropic bone conditions. The outcome shows stress concentration areas in the central zone of the diaphysis of …

Effects of dual tasking on postural and gait performances in children with cerebral palsy and healthy children.

Simultaneous execution of motor and cognitive tasks is embedded in the daily life of children. 53 children of 7-12 years and 22 adults (study 1), 20 healthy children and 20 children of 7-12 years with cerebral palsy (study 2) performed a Stroop-animal task simultaneously with a standing or a walking task in order to determine the attentional demand of postural control and locomotion. Dual-task cost decreased with advancing age in healthy children during balance. CP and healthy children were similarly affected by dual-task constraints during standing and walking. Children with diplegia were mor…

[Disturbances of gait and postural stability in chronic cerebral ischemia].

Disturbances of gait and postural stability are characteristic of the chronic progressive vascular lesion of the brain observed in moderate to severe stages of cerebrovascular disease. Disconnections between the prefrontal cortex, basal ganglia and cerebellum due to the damage of the white matter (vascular leukoencephalopathy) underlie disturbances of gait and postural stability (shorter stride length, wider gait). There are difficulties in starting with possible stoppings during the walking, waiting in front of small barriers as well as difficulties in concentration (e.g., talk during walking…

New Technique: A Novel Femoral Derotation Osteotomy for Malrotation following Intramedullary Nailing.

A 19-year-old female patient sustained a closed spiral midshaft femoral fracture and subsequently underwent femoral intramedullary nail insertion. At followup she complained of difficulty in walking and was found to have a unilateral in-toeing gait. CT imaging revealed 30 degrees of internal rotation at the fracture site, which had healed. A circumferential osteotomy was performed distal to the united fracture site using a Gigli saw with the intramedullary femoral nail in situ. The static distal interlocking screws were removed and the malrotation was corrected. Two further static distal inter…

Surgical treatment for posterior tibial tendonitis in young competitive athletes.

The surgical treatment and outcome of eight competitive athletes with posterior tibial tendonitis refractory to nonsurgical treatment was retrospectively reviewed. There were one professional, five collegiate, and two high school athletes with an average age of 22 years. All athletes had failed nonsurgical treatments including nonsteroidal anti-inflammatory drugs, physical therapy, custom shoe inserts, walking boots, or casts. Surgical debridement of the posterior tibial tendon was performed in all eight. Postoperatively, the athletes were treated in a cast or walking boot for 4 weeks. At the …

Ventilation and gas exchange during treadmill locomotion in the American alligator (Alligator mississippiensis).

A number of anatomical characters of crocodilians appear to be inconsistent with their lifestyle as sit-and-wait predators. To address this paradoxical association of characters further, we measured lung ventilation and respiratory gas exchange during walking in American alligators (Alligator mississippiensis). During exercise, ventilation consisted of low-frequency, large-volume breaths. The alligators hyperventilated severely during walking with respect to their metabolic demands. Air convection requirements were among the highest and estimates of lung P(CO2) were among the lowest known in a…

Clinical trials of vasoactive and antiserotonin drugs in Duchenne muscular dystrophy.

In clinical trials of vasoactive and antiserotonin agents in Duchenne muscular dystrophy, we found that methysergide, 8 mg daily, was associated with an average 44% improvement in Gower Time (i.e., time to arise from supine) and an average 16% improvement in ward walking time. We then did a double blind crossover study of methysergide and found an average improvement of 33% in head holding time with methysergide compared to placebo. In other tests, including grip strength, ward walking time, and Gower Time, methysergide average scores always were better than placebo average scores, but never b…

Optimising backpack hip belt design: an ergonomic perspective on gait mechanics.

Backpack hip belts help redistribute weight but may limit pelvis and trunk transverse plane rotations. This study explores how hip belt desi … cross-frame design exhibited a coordination pattern between the pelvis and trunk similar to natural walking, possibly due to its ability to control trunk rotational range of motion (ROM). Additionally, the cross-frame mechanism led to lower erector spinae muscle activation compared to the fixed-frame hip belt. This study emphasises the crucial role of hip …
