Association of Antimitochondrial Antibody With Right Atrial Low Voltage and Sick Sinus Syndrome.

BACKGROUND: Antimitochondrial antibodies (AMAs), well known as markers of primary biliary cholangitis, are increasingly recognized in cardiac disease. Their potential association with right atrial (RA) abnormalities and sick sinus syndrome (SSS) remains unclear. CASE SUMMARY: We report three AMA-positive patients with atrial arrhythmia and RA involvement. Two patients demonstrated extensive RA low voltage on electroanatomic mapping, and one patient showed impaired atrial sensing and pacing consistent with SSS. Left ventricular involvement was variable. One patient developed myositis, indicati…

Etiology of end-stage liver cirrhosis impacts hepatic natural killer cell heterogenicity.

The natural killer (NK) cell population is a critical component of the innate immune compartment of the liver, and its functions are deeply affected by the surrounding environment. In the late stage of fibrosis, NK cells become dysfunctional, but the influence of disease etiology on NK cell behavior during cirrhosis remains unclear. Using single-cell RNA sequencing (scRNA-seq), we characterized the hepatic NK cells from end-stage cirrhotic livers from subjects with non-alcoholic steatohepatitis (NASH), chronic hepatitis C infection (HCV) and primary sclerosing cholangitis (PSC). Here, we show …

Fatal duodenal hemorrhage complicated after living donor liver transplantation: case report.

Fatal hemorrhage caused by duodenal ulcer is rarely seen after liver transplantation. We report a case with penetrating duodenal ulcer resul … e bleeding; however, the grafted liver became ischemic. The patient subsequently developed ischemic cholangitis, which was treated with percutaneous transhepatic cholangial drainage. Repeat transplantation was performed 30 months later. The patient was doing well at 10-month follow-up.

[Liver transplantation in Norway. Results after 10 years and 114 transplantations].

A total of 114 liver transplantations were performed in 106 patients in Norway during 1984-1994. Survival after one year was 65% and after three years 57%. The most frequent causes of death were infections and rejections. The survival rate improved considerably during the period, and after 1990 the 1 year survival was 70%. Approximately 2/3 of the patients return to work or education. Very few patients die later than 12 months after the transplantation. The most frequent indications were primary biliary cirrhosis, metabolic liver disease, primary sclerosing cholangitis, autoimmune cirrhosis an…

Gallstone ileus: a single-center experience.

BACKGROUND: Gallstone ileus (GSI) is a rare surgical emergency. Its diagnosis is challenging and frequently delayed. Moreover, there is no consensus regarding the optimal surgical approach. The primary point of debate is whether cholecystectomy and fistula repair should be performed and, if so, when. Several studies have suggested that omitting cholecystectomy and fistula repair may increase the risk of cholangitis, gallbladder cancer, and recurrent gallstone ileus. However, most of these data originate from studies conducted in the previous century and have not been supported by contemporary …

Liver diseases in the elderly.

The preceding discussions outline the various forms of cirrhosis that may be encountered in the elderly population. Cirrhosis is not uncommo … s in the elderly, with ascites and hepatomegaly. Alpha 1-antitrypsin deficiency, primary sclerosing cholangitis, idiopathic hemochromatosis, and chronic autoimmune hepatitis may result in advanced cirrhosis in the elderly; appropriate serum studies should be obtained. If questions remain and if therapy may be changed, liver biopsy can be performed. A recent s …

Cholangiocyte RUNX1 Orchestrates Fibrogenic and Inflammatory Signaling to Drive Biliary Fibrosis.

INTRODUCTION: Biliary fibrosis and inflammation are central to the pathogenesis of cholangiopathies such as primary sclerosing cholangitis (PSC) and primary biliary cholangitis (PBC). Inflammatory and fibrogenic stimuli, such as transforming growth factor-β (TGFβ) and lipopolysaccharide (LPS) signaling, drive these processes, but their underlying transcriptional mechanisms in cholangiocytes remain incompletely defined. We investigated the role of Runt-related transcription factor 1 (RUNX1) as a transcriptional co-regulator of fibroinflammatory signaling in cholangiocytes. METHODS: Human PSC-d…

Systemic PCSK9 elevation characterises autoimmune liver disease across sexes.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a key regulator of serum cholesterol. Its expression is particularly abundant in hepatocytes, yet its role in autoimmune liver diseases remains unclear. Here we investigated serum PCSK9 levels in patients with autoimmune liver diseases and compared them to healthy controls, with attention to sex-specific differences. Serum PCSK9 levels were measured in 100 patients with autoimmune liver diseases - 57 with primary sclerosing cholangitis (PSC), 33 with primary biliary cholangitis (PBC), and 10 with autoimmune hepatitis (AIH)-and 88 healthy…

Exploring causal links between autoimmune liver diseases, chronic hepatitis C, and thyroid disorders: Evidence from NHANES and GWAS studies.

Autoimmune liver diseases (AILDs), including autoimmune hepatitis (AIH), primary biliary cholangitis (PBC), and primary sclerosing cholangitis (PSC), often have complex interactions with thyroid diseases (TDs), such as hypothyroidism, hyperthyroidism, and Hashimoto thyroiditis (HT). These conditions frequently coexist and may share common autoimmune mechanisms, but their exact relationship remains poorly understood. Chronic hepatitis C (CHC), a viral liver disease, also affects thyroid function, but its interaction with TD is still under investigation. This study explores the causal links betw…

Comparative histologic features among liver biopsies with biliary-pattern injury and confirmed clinical diagnoses.

Biliary-pattern injury in the liver (eg, duct injury, ductular reaction, cholestasis) can occur in several conditions, including primary biliary cholangitis (PBC), primary sclerosing cholangitis (PSC), large duct obstruction (LDO), and drug-induced liver injury (DILI). While the histologic changes in these conditions have been individually well described, distinguishing among them remains often challenging, particularly when biopsy samples are limited in size, robust clinical information is unavailable, and/or the pathologist does not feel confident in evaluating liver disease. This study eval…

Dissecting the genetic basis and mechanisms underlying the associations between multiple extrahepatic factors and autoimmune liver diseases.

BACKGROUND: Autoimmune liver diseases (AILDs) encompass autoimmune hepatitis (AIH), primary biliary cholangitis (PBC), and primary sclerosing cholangitis (PSC). The onset of these diseases is fundamentally influenced by genetic susceptibility. Although various extrahepatic factors are potentially linked to AILDs, the genetic underpinnings and mechanisms of these associations remain unclear. METHODS: Utilizing large-scale genome-wide association study (GWAS) data, this study systematically investigated the relationships between extrahepatic autoimmune diseases (EHAIDs), immune cells, and vario…
