Country-Level RSV-Related Hospitalisation Rates Among Adults in High-Income Countries From an Ensemble Model Based on Direct Viral Testing Incidence Data.

BACKGROUND: Understanding hospitalisation burden of respiratory syncytial virus (RSV)-associated acute respiratory infection (ARI) in adults is crucial for informing immunisation policy, yet such data are lacking in most high-income countries. We aimed to estimate country-level RSV-associated ARI hospitalisation rates among adults aged 18-64 and ≥ 65 years across high-income countries. METHODS: Data on adult RSV hospitalisation burden were identified from systematic literature review and national surveillance reports. A Bayesian adjustment was applied to account for underascertainment due to

Red cell alloimmunization in resource-constrained settings: A critical review of immunobiological drivers and structural determinants.

Red blood cell (RBC) alloimmunization continues to challenge the foundational promise of transfusion therapy: to provide safe and universally effective support. Although most transfused individuals remain tolerant, a vulnerable minority develop alloantibodies that complicate care and increase the risk of delayed hemolytic transfusion reactions (DHTRs). While high-income nations increasingly mitigate this risk through molecular genotyping, low- and middle-income countries (LMICs) face a distinct landscape defined by antigenic mismatch and resource scarcity. This critical review evaluates alloim

Multi-Omics for Mothers and Infants (MOMI) Consortium: a global initiative to study adverse pregnancy outcomes.

BACKGROUND: The Multi-Omics for Mothers and Infants (MOMI) Consortium aims to define biological mechanisms associated with preterm birth, small for gestational age, preeclampsia, and stillbirth. Globally, the burden of adverse pregnancy outcomes (APOs) is highest in low- and middle-income countries (LMICs). The MOMI Consortium consists of six different LMIC sites established by The Alliance for Maternal and Newborn Health Improvement in Bangladesh, Pakistan, and Tanzania, the Global Alliance to Prevent Prematurity and Stillbirth in Bangladesh and Zambia, and the Interdisciplinary Group for Adv

Operationalizing One Digital Health and FAIR Data Principles for Disease Surveillance in a Low-Medium Income Country in Africa: Qualitative Study in the Democratic Republic of the Congo.

BACKGROUND: Recent crises involving zoonotic diseases (Ebola, COVID-19, and Mpox) have highlighted the limitations of fragmented public health systems for the prevention and response to health emergencies at the international and continental level, and particularly in Africa, more specifically in the Democratic Republic of the Congo (DRC). In this context, the One Health (OH) approach and its extension, One Digital Health (ODH), articulated with the findable, accessible, interoperable, reusable (FAIR) principles, offer a framework for rethinking the digital transition in health in the DRC. Gender and other intersecting factors in antimicrobial resistance for infectious diseases of poverty: a systematic evidence gap analysis in low- and lower-middle-income countries.

BACKGROUND: Gender influences health outcomes by affecting exposure to risk factors, healthcare access, and health-seeking behaviours. Yet, many studies fail to consider how these gendered experiences interact with other social factors, such as age, socioeconomic status, and ethnicity. Our study systematically mapped existing research to identify gaps in understanding how these factors affect service delivery outcomes related to antimicrobial resistance (AMR) for infectious diseases of poverty. METHODS: We conducted a systematic review and evidence gap map analysis following PRISMA 2020 guidel

Prevalence of sarcopenia and individual- and country-level risk factors in 36 countries: pooled analysis of nationally representative individual data.

BACKGROUND: Sarcopenia is an age-related condition that contributes to disability and healthcare burden worldwide. Yet cross-national evidence on prevalence and multi-level risk factors is limited. METHODS: We pooled data from 10 nationally representative or population-based ageing studies, including 144 226 adults aged ≥50 years from 36 high- and middle-income countries. Sarcopenia and severe sarcopenia were defined using 2019 revised European Working Group on Sarcopenia in Older People criteria. Prevalence was standardised by age and gender within each country. We examined associations betw

Estimating the number of incorrect tuberculosis diagnoses in low- and middle-income countries.

Tuberculosis (TB) is the greatest cause of infectious disease deaths worldwide. In highly affected countries, effective TB control requires prompt identification and treatment of individuals with active disease. We examined the performance of TB case-finding in low- and middle-income countries based on a comprehensive analysis of TB diagnosis data reported to the World Health Organization. Using these data we estimated the total number of individuals correctly and incorrectly diagnosed with TB, for 111 countries with a collective 6.8 million TB notifications in 2023. Here we estimate that in 2

Evaluating geographic accessibility to COVID-19 vaccination across 54 countries/regions.

BACKGROUND: The COVID-19 pandemic has revealed significant disparities in global vaccine accessibility, particularly affecting low and middle-income countries (LMICs). However, current research on COVID-19 vaccine accessibility primarily focuses on individual countries or high-income countries (HIC). We aimed to evaluate geographic accessibility to COVID-19 vaccination on a multicountry scale, covering comparisons across LMICs and HICs. Additionally, we explored the potential economic factors related to accessibility and their impacts on health outcomes. METHODS: We collected population data

Income-Based Disparities in Perceived Benefits and Challenges of Virtual Global Health Activities During the COVID-19 Pandemic: Mixed Methods Analysis.

BACKGROUND: Global health activities (GHAs) can potentially reduce health disparities by facilitating resource sharing, promoting medical education and professional development worldwide, and enhancing collaboration among high-income countries (HICs) and low- and middle-income countries (LMICs). However, the COVID-19 pandemic disrupted in-person GHAs due to strict infection control and travel restrictions. To ensure the continuity of GHAs and further address health inequity, virtual GHAs (VGHAs) are gaining traction. OBJECTIVE: Our research aimed to understand how people perceive the benefits

Epidemiology and outcome of pediatric acute kidney injury-multicenter observational study from a low-middle-income country.

BACKGROUND: The epidemiology and outcome of acute kidney injury (AKI) in low-middle-income countries (LMICs) differ from those in high-income countries due to differences in type and severity of non-renal systemic illness and variability in nephrology-care facilities. There is a paucity of multicenter studies from LMICs. This multicenter observational study was undertaken to study the epidemiology of pediatric AKI in a LMIC and analyze the significance of associated sample characteristics and interventions on outcomes, namely renal recovery and mortality. METHODS: Children (1 month-18 years)

Estimated undertreatment of carbapenem-resistant Gram-negative bacterial infections in eight low-income and middle-income countries: a modelling study.

BACKGROUND: Carbapenem-resistant Gram-negative (CRGN) bacterial infections are an urgent health threat, especially in low-income and middle-income countries (LMICs), where they are rarely detected and might not be treated appropriately given inadequate health system capacity. To understand this treatment gap, we estimated the total number of CRGN bacterial infections requiring an active agent and the number of individuals potentially initiated on appropriate treatment in eight large LMICs. METHODS: For eight selected countries (Bangladesh, Brazil, Egypt, India, Kenya, Mexico, Pakistan, and So

Was the Allocation of COVID-19 Vaccines Globally Fair and Equitable?

Determining whether an allocation is equitable requires ethical and empirical analyses, specifically answers to 3 questions: (1) What is the ethical standard for fair allocation? (2) What is the quantitative equity metric for this standard? and (3) What do the empirical data demonstrate? Two ethical standards for assessing the fair allocation of scarce medical resources have been delineated: the COVID-19 Vaccines Global Access initiative's population-based standard and the COVID-19 health-burden standard. The equity metric for the population-based standard is vaccine per person in each country

Infection prevention and healthcare epidemiology professionals in low- and middle-income countries: a needs assessment survey and call for action.

INTRODUCTION: More than 75% of the global population resides in low- and middle-income countries (LMICs), where healthcare-associated infection rates are notably higher than in high-income countries. Little is known about the professional experiences, perceptions and needs of infection prevention and healthcare epidemiology professionals (IPHEP) practising in these countries. METHODS: A voluntary and anonymous online survey of IPHEP in LMICs was conducted via open invitations on social media and email from October 2022 to January 2023. The survey covered five domains: (1) Survey Responders, P

Outcomes of pediatric patients prioritized as critical (P1/P2) by registered triage nurses from a tertiary care hospital in a low-middle-income country.

Triage in emergency departments (EDs) is a dynamic decision-making process to prioritize patients based on their medical care needs. The Emergency Severity Index (ESI) is a simple-to-use, five-level triage system that categorizes ED patients according to clinical urgency. The triage nurse's ability to obtain a brief history and rapidly assess clinical urgency is crucial for ensuring safe and efficient emergency care. This study aimed to assess the ability of registered triage nurses to accurately identify pediatric patients classified as Priority 1 (P1) and Priority 2 (P2) under the ESI, ensur
