Gastrointestinal perforations in neonates with anorectal malformations.

We describe the presentation and management of gastrointestinal perforation in four neonates with anorectal malformations. Two neonates with high malformation had pneumoperitoneum on X-ray; surgery revealed sigmoid perforation in one patient and transverse colon perforation in the other. Colostomy was done, followed by posterior sagittal anorectoplasty at four months; both recovered satisfactorily. The third neonate had no radiological feature of gut perforation but cecal perforation was found at surgery; the neonate recovered following right hemicolectomy with stoma followed by anorectoplasty

The role of early enteral nutrition in protecting premature infants from sepsis.

Care of critically ill, preterm infants is a major challenge. Because of their small size and complex health problems, preterm infants require long-term hospitalization in the intensive care unit where they are exposed to serious microorganisms and other antigens that can overwhelm their immature immune systems. As smaller and more fragile preterm infants are surviving NICU care, these infants are at increased risk for nosocomial infections. Although modern antimicrobial agents are invaluable in the management of infection, they can result in biologic stress to the immature physiology of the p

Application of an object-oriented programming paradigm in three-dimensional computer modeling of mechanically active gastrointestinal tissues.

The aim of this study was to develop a novel three-dimensional (3-D) object-oriented modeling approach incorporating knowledge of the anatomy, electrophysiology, and mechanics of externally stimulated excitable gastrointestinal (GI) tissues and emphasizing the "stimulus-response" principle of extracting the modeling parameters. The modeling method used clusters of class hierarchies representing GI tissues from three perspectives: 1) anatomical; 2) electrophysiological; and 3) mechanical. We elaborated on the first four phases of the object-oriented system development life-cycle: 1) analysis; 2

Ultrasonographic features of gastrointestinal duplications.

The variable sonographic appearance of duplication cysts is presented. Eighteen sonograms from 14 patients, aged 1 day to 8 years, were reviewed over an 8 year period. Water and other aqueous contrast agents were used in six patients as part of the sonographic evaluation. All lesions were confirmed by surgery. All but two patients were symptomatic. Twenty-four cysts were detected, ranging in size from 1.7 to 15.5 cm. The duplication cysts revealed a spectrum of sonographic findings (cystic to solid appearing masses). The mass characteristics, including the "muscular rim sign," and internal deb

Gastrointestinal morphology and absorption of monosaccharides in fowls conditioned to different types and levels of dietary fibre.

To test a possible influence of dietary fibre on intestinal sugar uptake, rates of absorption of 10 mM-D-[U-14C] glucose and 10 mM-D-[U-14C] xylose were measured in either jejunum or (distal) caecum, by in vivo lumen perfusion, in immature female fowls preconditioned to a standard diet containing (g/kg) either 0, 100, 200 or 400 added dried grass, 200 powdered cellulose, or 200 grass with a polysaccharidase enzyme supplement. When birds were killed after perfusion, dimensions of (unperfused) parts of their alimentary tracts were determined, and recoveries of 14C radioactivity in some body tiss

The current practice of paediatric surgery in Malaysia.

This 10-year review of surgical conditions in infants at the University Hospital, Kuala Lumpur, highlights some of the more common problems encountered and outlines their management. Anorectal agenesis and Hirschsprung's disease were seen relatively more frequently than other anomalies of the gastrointestinal tract. The management of these two conditions and the operation of colostomy and its complications are singled out and presented in some detail.

[Progress of surgical treatment in the last 10 years. Comparative study of surgical interventions performed between 1993-1995 versus between 2003-2005].

In the last decade of the past century, as laparoscopy was introduced in our clinic in 1993, minimal access therapy (MAT--endoscopy, angiography, interventional imagery) had a positive and constant evolution. Our paper retrospectively evaluates the interventions performed between 2003-2005 (group A) compared to those performed between 1993-1995 (group B). We observed a 17.08% (7056 vs 6026 interventions/year) raise in the total number of interventions in group A, with a significant 66% decrease (195 vs. 588 interventions/year) of interventions for gastro-duodenal ulcer and a 18% decrease (1211

Extensive intestinal aganglionosis.

Extensive aganglionosis very close to the ligament of Treitz and total intestinal aganglionosis are rare forms of Hirschsprung's disease. In these cases, nutrition, fluid, and electrolyte balance are a problem. Although the myectomy-myotomy technique of Ziegler appears hopeful, no effective surgical method has been put into the practice for the treatment of these rare forms of Hirschsprung's disease. We report four cases with extensive intestinal aganglionosis, one of which was total intestinal aganglionosis with involvement of the stomach.

[The gut, a whistleblower, in the early stages of Parkinson's disease].

The enteric nervous system (ENS), often called the "second brain", plays a crucial role in regulating digestive functions. Dysfunctions of the ENS are associated with several diseases such as Parkinson's disease. Recent studies suggest that early digestive disorders, notably chronic constipation, may be early signs of this neurodegenerative disease. Three-dimensional imaging of the ENS offers new insights into early diagnosis, in particular through the analysis of intestinal biopsies. This new research axis raises questions about the intestinal cause of Parkinson's disease, and opens the door

Peritoneal Actinomycosis After Bariatric Surgery Mimicking a Peritoneal Carcinomatosis on 18 F-FDG PET/CT.

A 54-year-old man with a history of bypass surgery 3 years ago presented with ascites, abdominal pain, and a decline in general health. CT and 18 F-FDG PET/CT suggested peritoneal carcinomatosis, but histology revealed peritoneal actinomycosis. This rare peritoneal infection can occur after digestive surgeries, such as bariatric procedures, due to disruption of the digestive barrier, allowing actinomycosis to spread outside its typical anatomical location. Clinicians should consider this diagnosis after bariatric surgery, even years later, especially in the absence of neoplastic disease.

Diphyllobothrium dendriticum (Cestoda: Diphyllobothriidae) in the Intestinal Tract of the Herring Gull Larus argentatus: Localization and Trophic Parameters.

We studied the activities both of digestive enzymes in the small intestine of the herring gull (Larus argentatus) and a tapeworm Diphyllobothrium dendriticum (Cestoda: Diphyllobothriidae) residing in the intestine. It was found that D. dendriticum infects the medial section of the small intestine of the herring gull. Such localization of D. dendriticum is caused by the maximal activity of proteases and glycosidases and by the high rate of membrane and cavitary digestion in this section. The activity of protease and glycosidase in gulls infected with D. dendriticum is decreased. The activity of…

Fluorescent amplified fragment length polymorphism (fAFLP) analysis of Listeria monocytogenes.

Fluorescent amplified fragment length polymorphism (fAFLP) is based on the selective PCR amplification of restriction fragments from a digest of total genomic DNA. Genomic DNA extracted from a purified bacterial isolate is completely digested with two endonucleases generating fragments which are ligated to specific double-stranded adaptors. The ligated fragments are then amplified by PCR using fluorescently labelled primers. Fluorescent amplified fragments are separated by size on an automated sequencer with a size standard. fAFLP is a rapid, highly reproducible technique which can be used to

[Specific carbohydrate malabsorption].

Specific malabsorption of carbohydrate is related to the lack or decrease in enzymatic activity needed for its hydrolysis; seldom, it is related to the lack or overloading in transport mechanism of monosaccharide. Ingestion of unabsorbed carbohydrate may induce digestive symptoms due to its colonic fermentation (borborygmus, bloating, pain, and flatus) or its osmotic activity (diarrhoea). In a patient consuming at least a bowl of milk per day and suffering of functional digestive symptoms, intolerance to lactose must be ruled out because its treatment is easy and efficient, i.e. to put ferment

[Aneurysm of the celiac trunk. Etiopathogenic role of the arcuate ligament].

INTRODUCTION: Aneurysms of arteries supplying the digestive tract are uncommon and usually asymptomatic. CLINICAL REPORT: A 58-year-old woman presented weight-loss with intestinal angina and a painful abdomen. Explorations induced an arteriogram of the aorta and the digestive arteries which showed an aneurysm of the celiac trunk located immediately below a stenosis facing the arcuate ligament. DISCUSSION: The discovery of an aneurysm of the celiac trunk in a patient without any history of hypertension or artheromatous disease led us to implicate the arcuate ligament as the causal agent. Duodenal and esophageal manometry in total colonic aganglionosis.

Eleven children with total colonic aganglionosis were studied by esophageal and duodenal manometry. They were separated into two groups: group 1, with ileal involvement, and group 2, without ileal involvement. The results of the digestive motility studies were compared with the extent of ileal involvement of the aganglionosis and with the amount of artificial nutritional support required. All children showed abnormalities on both esophageal and duodenal manometry, suggesting complete digestive tract motility involvement. Recent advances in understanding Hirschsprung's disease and manometrial a
